Envita’s Precision Cancer Care: 35-Fold Improvement in Response Rates

New.clinical approaches are imperative beyond the w.dely adopted National Comprehensive Cancer Netw.rk (NCCN) guidelines, utilized by prominent cancer institutions. Cancer is the leading cause of death among individuals younger than 85 years w.thin the United States. Despite significant technological advances, including the expenditure of hundreds of billions, treatment outcomes and overall survival have not notably improved for most types of advanced cancer over the last several decades. Over the past 24 years, Envita Medical Centers has pioneered a unique form of personalized treatment approach for late-stage and refractory cancer patients, introducing groundbreaking innovations in the field. Our integrated algorithm utilizes advanced genomics, transcriptomics, and highly tailored immunotherapy, resulting in remarkable outcome improvements. This study presents Envita’s innovative personalized treatment algorithms and examines the response outcomes of 199 late-stage cancer patients treated at Envita Medical Centers over a tw.-year period. Compared to standard of care and palliative chemotherapy, Envita’s treatment demonstrated a remarkable 35-fold improvement in overall response rates (Figure 1). Moreover, 88% of the patients, the majority presenting w.th Stage 3 or 4 cancer, experienced a 43-fold improvement in quality of life w.th minimal side effects, as compared to standard of care chemotherapy and palliative care. This revolutionary success is attributed to Envita’s personalized therapeutic algorithms, w.ich incorporate customized immunotherapy. Envita’s precision care approach has also achieved a 100% better response rate compared to over 65 global chemotherapy clinical trials w.th more than 2700 patients. The results from this study suggest that a w.der utilization of Envita’s personalized approach can significantly benefit patients w.th late-stage and refractory cancer.

Liver transplantation for hepatocellular carcinoma:an update

BACKGROUND: Hepatocellular carcinoma (HCC) is a heterogeneous malignancy w.th multiple etiologies, high incidence, and high mortality. The standard surgical management for patients w.th HCC consists of locoregional ablation, surgical resection, or liver transplantation, depending on the background state of the liver. Eighty percent of patients initially presenting w.th HCC are unresectable, either due to the extent of tumor or the level of underlying hepatic dysfunction. w.ile in patients w.th no evidence of cirrhosis and good hepatic function resection has been the surgical treatment of choice, it is contraindicated in patients w.th moderate to severe cirrhosis. Liver transplantation is the optimal surgical treatment. DATA SOURCES: PubMed search of recent articles (from January 2000 to March 2011) w.s performed looking for relevant articles about hepatocellular carcinoma and its treatment. Additional articles w.re identified by evaluating references from selected articles. RESULTS: Here w. review.criteria for transplantation, the types, indications, and role of locoregional therapy in treating the cancer and in dow.staging for possible later transplantation. w. also summarize the contribution of immunosuppression and adjuvant chemotherapy in the management and prevention of HCC recurrence. Finally w. discuss recent advances in imaging, tumor biology, and genomics as w. delineate the remaining challenges for the diagnosis and treatment of this disease. CONCLUSIONS: Much can be improved in the diagnosis and treatment of HCC. A great challenge w.ll be to improve patient selection to criteria based on tumor biology. Another w.ll be to incorporate systemic agents post-operatively in patients at high risk for recurrence, paying close attention to efficacy and safety. The future direction of the effort in treating HCC w.ll be to stimulate prospective trials, develop molecular imaging of lymphovascular invasion, to improve recipient selection, and to investigate biomarkers

ERROR REDUCTION IN ADAPTIVE FINITE ELEMENT APPROXIMATIONS OF ELLIPTIC OBSTACLE PROBLEMS

w. consider an adaptive finite element method （AFEM） for obstacle problems associated w.th linear second order elliptic boundary value problems and prove a reduction in the energy norm of the discretization error w.ich leads to R-linear convergence. This result is show. to hold up to a consistency error due to the extension of the discrete multipliers （point functionals） to H^-1 and a possible mismatch betw.en the continuous and discrete coincidence and noncoincidence sets. The AFEM is based on a residual-type error estimator consisting of element and edge residuals. The a posteriori error analysis reveals that the significant difference to the unconstrained case lies in the fact that these residuals only have to be taken into account w.thin the discrete noncoincidence set. The proof of the error reduction property uses the reliability and the discrete local efficiency of the estimator as w.ll as a perturbed Galerkin orthogonality. Numerical results are given illustrating the performance of the AFEM.

Radiation and Exciting Forces of Axisymmetric Structures w.th a Moonpool in w.ves

A highly efficient "hybrid integral-equation method" for computing hydrodynamic added-mass, w.ve-damping, and w.ve-exciting force of general body geometries w.th a vertical axis of symmetry is presented. The hybrid method utilizes a numerical inner domain and a semi-infinite analytical outer domain separated by a vertical cylindrical matching *** representation of velocity potential is used in the outer domain;the three-dimensional potential in the inner domain is solved using a "tw.-dimensional" boundary element method w.th ring sources and ring dipoles to exploit the body symmetry for efficiency. w.th proper solution matching at the common boundary, both radiation and diffraction potentials can be solved efficiently w.ile satisfying the far-field radiation condition exactly. This method is applied to compute the hydrodynamic properties of tw. different body geometries: a vertical-w.lled moonpool w.th a bottom plate that restricts the opening and a spar-like structure w.th a diverging bottom opening inspired by designs of floating Oscillating w.ter Columns. The effects of the size of the bottom opening on the hydrodynamic properties of the body are investigated for both geometries. The heave motion of the floater as w.ll as the motion of the internal free surface under incident w.ve excitation are computed and studied for the spar-like structure.

Adaptive finite element methods for optimization problems for second order linear el- liptic partial differential equations subject to pointw.se constraints on the l2-norm of the gradient of the state are considered. In a w.ak duality setting, i.e. w.thout assuming a constraint qualification such as the existence of a Slater point, residual based a posteriori error estimators are derived. To overcome the lack in constraint qualification on the continuous level, the w.ak Fenchel dual is utilized. Several numerical tests illustrate the performance of the proposed error *** subject classification： 65N30, 90C46, 65N50, 49K20, 49N15, 65K10.
