Diffuse Cervico-Facial Cellulitis: Epidemiological, Diagnostic and Therapeutic Aspects at the Teaching Hospital CNHU HKM of Cotonou

Purpose of the work: to study the epidemiological, clinical and therapeutic aspects of diffuse cervico-facial cellulitis collected within the ENT-CCF surgery department of the CNHU of Cotonou. Materials and method: the study was a descriptive retrospective carried out from January 1st, 2018 to December 31st, 2020. It concerned new cases of diffuse cervico-facial cellulitis identified during the period. Results: in 3 years, 55 cases of diffuse cervico-facial cellulitis were identified out of 1101 hospitalized patients. The frequency was 4.99%. Per year, 18 cases of cervico-facial cellulitis are recorded. The mean age of the patients was 41.3 years with extremes of 15 years and 85 years. A clear male predominance was noted with a sex ratio of 1.29. Painful cervico-facial swelling was the most common reason for medical consultation (47 patients or 85.5% of cases). Thirty patients (54.5%) showed a deterioration in general condition. Several clinical forms of diffuse cervico-facial cellulitis have been described, in particular the gangrenous form, the suppurative, necrotizing form and LUDWIG’s angina. The portal of entry was dental in 50 patients (91% of cases), favored by poor oral hygiene and self-medication with non-steroidal anti-inflammatory drugs (NSAIDs). The medico-surgical treatment implemented was favorable for 47 patients (85.5%). Conclusion: Diffuse cervico-facial cellulitis remains a frequent medical-surgical emergency in the CCF ENT department at the Teaching Hospital of Cotonou. Mainly of odontogenic origin, it is encountered in different clinical forms.

Impact of the Foucault Current and Its Vibration Noise Reduction on Bracket Features for AMDT

This manuscript explores the influences of the Foucault current on bracket vibration. Noise measurements are obtained in conditions that the coils are enclosed by the bracket, and exposed to the air for confirming the impact of Foucault current on bracket vibration. The outcomes illustrate that the bracket increases the primary noise to 21 d B. Throughout investigational modal computation, ordinary frequencies of the coils stay afar from the exciting frequency of 100 Hz but the ordinary frequency 72.924 Hz of the bracket stands near to 100 Hz, which similarly explicates the upsurge of the noise level. To do the computations on bracket vibrations instigated by the Foucault current, a finite-element method(FEM) has been utilized for evaluating the eddy current density, electromagnetic forces(EMF), coupled with bracket response in frequency domain. The calculations displayed a proper approval by mean of theoretic investigations, and the simulations analyzed effectively the bracket vibration produced by Eddy current, and the electromagnetic force(EMF). Referring to the simulation, the primary vibration characteristics, and noise level, the method by applying weights(stress) on the top bracket of transformer is proposed to lessen the vibration amplitude, and noise level. To indorse the utility, and application of the suggested technique, the experiment results are obtained, analyzed, and compared with those of simulations, primary vibration characteristics, and noise level.

Investigation on Vibration Characteristics of Amorphous Metal Alloy Core Dry-type Distribution Transformer

Large power transformers have severe vibration and noise problems. Thus, as the source of noise, it is vitally important to have accurate core vibration calculations. The calculation of the vibration is incredibly difficult to consider the complex propagation pathway of the vibration in the clamp and the Amorphous Metal Alloy Core Distribution Transformer(AMACDT). To understand the vibration characteristics of AMACDT, this manuscript provides a different approach on investigation of the vibration characteristics of an SCBH15-200/10 type. Based on the vibration characteristics calculation and displacement of an Amorphous Metal Alloy Core Distribution Transformer(AMACDT), they are obtained and solved using Finite Element Analysis software(FEA). Conferring to the outcomes of the vibration analysis, the method by adding bar-reinforcements on the upper clamp of transformer is proposed to lessen the vibration amplitude. To verify the usefulness and application of the suggested approach, the analytical results are obtained. The suggested approach is analyzed with those of experimental.

A gliding electric arc(glidarc)discharge generates a low-temperature plasma at atmospheric *** the discharge occurs in humid air as the feed gas,the chemistry of a glidarc plasma consists of in situ formation of HO°and NO°as the primary chemical *** trioxide(WO_(3))nanoparticles were successfully prepared by exposure of a liquid precursor to glidarc *** WO_(3)samples were calcined at three different temperatures(300℃,500℃and 800℃),resulting in different pure polymorphs:γ-WO_(3)(at 300℃),β-WO_(3)(at 500℃)andα-WO_(3)(at 800℃)according to x-ray diffraction *** identification of WO_(3)compounds was also confirmed by attenuated total reflection Fourier transform infrared spectroscopy *** in the calcination temperature of WO_(3)induced a decrease in its specific surface area according to Brunauer–Emmett–Teller nitrogen physisorption *** UV-visible results showed that the absorption bands of plasma-WO_(3)samples were more intense than those of WO_(3)samples obtained by a precipitation route,a classical method used for ***,this parameter can improve the photocatalytic properties of WO_(3)under visible *** photodegradation(in sunlight conditions)of gentian violet,chosen as a model pollutant,confirmed the photocatalytic properties of plasma-WO_(3)*** novel synthesis method has great potential to improve the efficiency of advanced tungsten trioxide-based functional material preparation,as well as in pollution-reducing and energy-saving tungsten extractive metallurgy.

Role of transcatheter arterial embolization for massive bleeding from gastroduodenal ulcers

Intractable bleeding from gastric and duodenal ulcers is associated with significant morbidity and mortality. Aggressive treatment with early endoscopic hemostasis is essential for a favourable outcome. In as many as 12%-17% of patients,endoscopy is either not available or unsuccessful. Endovascular therapy with selective catheterization of the culprit vessel and injection of embolic material has emerged as an alternative to emergent operative intervention in high-risk patients. There has not been a systematic literature review to assess the role for embolotherapy in the treatment of acute upper gastrointestinal bleeding from gastroduo-denal ulcers after failed endoscopic hemostasis. Here,we present an overview of indications,techniques,and clinical outcomes after endovascular embolization of acute peptic-ulcer bleeding. Topics of particular relevance to technical and clinical success are also discussed. Our review shows that transcatheter arterial embolization is a safe alternative to surgery for massive gastroduodenal bleeding that is refractory to endoscopic treatment,can be performed with high technical and clinical success rates,and should be considered the salvage treatment of choice in patients at high surgical risk.

Arterial embolization is the best treatment for pancreaticojejunal anastomotic bleeding after pancreatoduodenectomy

Massive pancreaticojejunal anastomotic bleeding, mainly from the gastroduodenal stump, is one of the most common complications of pancreato-duodenectomy. Selective angiography should be systematically the first step of investigative procedure in such situations. Pharmacoarteriography may be used if the bleeding point is not spontaneously identified, and allows safe and effective treatment with transcatheter arterial embolization compared to blind open surgical hemostasis. Coil embolization of the common or proper hepatic artery on either side of the bleeding point with "sandwich technique" is then the preferred technique to prevent retrograde filling. Surgery should be performed only as a last resort.
