Kidney transplantation is the best replacement treatment for the end-stage renal ***,the imbalance between the number of patients on a transplant list and the number of organs available constitutes the crucial limitation of this *** expand the pool of organs amenable for transplantation,kidneys coming from older patients have been employed;however,the combination of these organs in conjunction with the chronic use of immunosuppressive therapy increases the risk of incidence of graft small renal *** narrative review aims to provide the state of the art on the clinical impact and management of incidentally diagnosed small renal tumors in either donors or *** to the most updated evidence,the use of grafts with a small renal mass,after bench table tumor excision,may be considered a safe option for high-risk patients in *** the other hand,an early small renal mass finding on periodic ultrasound-evaluation in the graft should allow to perform a conservative treatment in order to preserve renal ***,in case of a renal tumor in native kidney,a radical nephrectomy is usually recommended.

Supporting flexible regulation of crisis management by means of situated artificial institution

This paper highlights the use of situated artificial institution(SAI) within a hybrid, interactive,normative multi-agent system to regulate human collaboration in crisis management. Norms regulate the actions of human actors based on the dynamics of the environment in which they are situated. This dynamics results from both environment evolution and actors' actions. Our objective is to situate norms in the environment in order to provide a context-aware crisis regulation. However, this coupling must be a loose one to keep both levels independent and easyto-change in order to face the complex and changing crisis situations. To that aim, we introduce a constitutive level between environmental and normative states providing a loose coupling of normative regulation with environment evolution. Norms are thus no more referring to environmental facts but to status functions, i.e., the institutional interpretation of environmental facts through constitutive rules. We present how this declarative and distinct SAI modelling succeeds in managing the crisis with a context-aware crisis regulation.

Silicon(Si)photonics has recently emerged as a key enabling technology in many application fields thanks to the mature Si process technology,the large silicon wafer size,and promising Si optical *** monolithic integration by direct epitaxy of III-V lasers and Si photonic devices on the same Si substrate has been considered for decades as the main obstacle to the realization of dense photonics *** considerable progress in the last decade,only discrete III-V lasers grown on bare Si wafers have been reported,whatever the wavelength and laser *** we demonstrate the first semiconductor laser grown on a patterned Si photonics platform with light coupled into a waveguide.A mid-IR GaSb-based diode laser was directly grown on a pre-patterned Si photonics wafer equipped with SiN waveguides clad by SiO_(2).Growth and device fabrication challenges,arising from the template architecture,were overcome to demonstrate more than 10 mw outpower of emitted light in continuous wave operation at room *** addition,around 10%of the light was coupled into the SiN waveguides,in good agreement with theoretical calculations for this butt-coupling *** work lft an important building block and it paves the way for future low-cost,large-scale,fully integrated photonic chips.

Cardiogenic shock(CS)is a life-threatening condition characterized by acute end-organ hypoperfusion due to inadequate cardiac output that can result in multiorgan failure,which may lead to *** diminished cardiac output in CS leads to systemic hypoperfusion and maladaptive cycles of ischemia,inflammation,vasoconstriction,and volume ***,the optimal management of CS needs to be readjusted in view of the predominant dysfunction,which may be guided by hemodynamic *** monitoring enables(1)characterization of the type of cardiac dysfunction and the degree of its severity,(2)very early detection of associated vasoplegia,(3)detection and monitoring of organ dysfunction and tissue oxygenation,and(4)guidance of the introduction and optimization of inotropes and vasopressors as well as the timing of mechanical *** is now well documented that early recognition,classification,and precise phenotyping via early hemodynamic monitoring(e.g.,echocardiography,invasive arterial pressure,and the evaluation of organ dysfunction and parameters derived from central venous catheterization)improve patient *** more severe disease,advanced hemodynamic monitoring with pulmonary artery catheterization and the use of transpulmonary thermodilution devices is useful to facilitate the right timing of the indication,weaning from mechanical cardiac support,and guidance on inotropic treatments,thus helping to reduce *** this review,we detail the different parameters relevant to each monitoring approach and the way they can be used to support optimal management of these patients.

Background Mali is known to be a schistosomiasis-endemic country with a limited supply of clean *** has forced many communities to rely on open freshwater bodies for many human-water contact(HWC)***,the relationship between contact with these water systems and the level of schistosome infection is cur-rently receiving limited *** study assessed human-water interactions including cercarial emergence pat-tern and their influences on urinary schistosomiasis transmission in two communities in the Kayes district of *** We carried out a parasitological study frst in children in September 2021,then a cross-sectional study of quantitative observations of human-water contact activities in the population,and finally a study of snail infectiv-ity at contact points in September *** study took place in two communities,Fangoune Bamanan and Diakalel in the Kayes region of western *** chronobiological study focused on cercarial release from naturally infected *** cercariae were molecularly genotyped by targeting the cox1 region,and the ITS and 18S ribosmal DNA gene(18S rDNA)regions of the *** between sociodemographic parameters,human water-contact points and hematuria were established using multivariate statistical analysis or the logistic regression *** The main factor predisposing the 97 participants to water contact was domestic activity(62.9%).Of the 378 snails collected at 14 sampling sites,27(7.1%)excreted schistosome cercariae,with 15.0%(19/126)at Fangoune Bamanan and 3.3%(8/252)at *** release of Schistosoma cercariae shows three different patterns in Fangoune Bamanan:(i)an early release peak(6:00-8:00 AM),(i)a mid-day release peak(10:00 AM-12:00 PM)and(ili)a double peak:(6:00-8:00 AM)and(6:00-8:00 PM)cercariae release;and two release patterns in Diakalel:early release(6:00-8:00 AM)and(il)mid-day release(12:00-2:00 PM).All cercariae released during early diurnal(6:00-8:00 AM)or nocturnal
