Graft loss among renal-transplant recipients with early reduction of immunosuppression for BK viremia

AIm To review the incidence of graft loss and acute rejection among renal transplant recipients with early reduction of immunosuppression for BK *** We performed a retrospective analysis of consecutive de-novo kidney-only transplants from January 2009 to December 2012 to evaluate the incidence of Polyomavirus associated nephropathy(PyV AN). Recipient plasma was screened for BKV DNA via quantitative polymerase chain reaction(PCR) at months 1,3,6,9 and 12 post-transplant and on worsening graft *** was reduced at ≥ 3-log copies/mL. Those with viremia of ≥ 4-log copies/mL(presumptive PyV AN) underwent renal transplant biopsy. Presumptive Py VAN(PP) and definitive Py VAN(DP; biopsy-proven) were treated by immunosuppression reduction(IR) only. RESULTS Among 319 kidney transplant recipients,the median age was 53 years(range 19-83),65.8% were male,and 58.9% were white. Biopsy-proven acute rejection was found in 18.5% within 0-168 wk. Death-censored graft loss occurred in 5.3%(n = 17) and graft loss attributable to PyV AN was 0.6%(n = 2). Forty-seven patients were diagnosed with PP(14.7%) and 18(5.6%) with DP. Graft loss among participants with PyV AN(8.5%) and those without(4.8%) was not significantly different. Deceased donor kidney transplantation(OR = 2.3,95%CI = 1.1-4.6) and AR(OR = 2.3,95%CI = 1.2-4.7) were associated with Py VAN in the multivariate analysis. BK viremia between 3 and 4-log copies/mL occurred in 27 patients,all of whom underwent IR. Of these,16(59%) never developed PyV AN while 11(41%) developed PyV AN(4 DP,7 PP) within a range of 11-39 wk. CONCLUSION Instituting an early reduction of immunosuppression,in the absence of adjunctive antivirals,is effective at preventing PyV AN and may be associated with a lower incidence of graft-loss without a reciprocal increase in the incidence of acute rejection.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.

Validation of a Risk-Based Biomarker-Enhanced Scoring System for Lower Respiratory Tract Infections (OPTImA I Basel)—An Observational Survey

Background: Despiteits recommendation in management guide lines for community acquired pneumonia (CAP), the CURB65 score is frequently not followed for disposition decisions in clinical routine. We therefore proposed an improved CURB65 A score, supplemented by proadre nome dull in (ProADm) levels for patients with CAP and other lower respiratory tract infections (LRTIs). In this study, we vali dated this risk based biomarker enhanced disposition in patients with LRTIs presenting to the emergency department of the University Hospital of Basel. methods: In this prospective observational cohort study of 85 patients presenting with LRTIs, site of care was decided by the physicians in charge according to their judgement. Retro spectively the CURB65 A score was calculated and a virtual disposition assigned. This was compared with the existing disposition in order to identify efficacy of the novel risk based biomarker enhanced disposition. Results: The novel disposition criteria considered 14 patients suitable for outpatient treatment compared to 11 in the current disposition (p = 0.5). It detected 7 patients to be best treated outside the hospital for nursing reasons, while the current disposition detected only 1 patient requiring geriatric care (p = 0.09). Further, it decreased regular hospitalizations considerably (32 vs. 64, p 0.001). Conclusions: The novel risk based biomarker enhanced disposition is an objective, safe and probably more efficient disposition system to identify outpatient treatment options than the current practice at the University Hospital of Basel.

＜正＞ 1 INTRODUCTION CERN, the European Laboratory for Particle Physics, is currently using the largest accelerator tunnel in the world. The same tunnel will be used and re-equipped for the future Large Hadron Collider （LHC） projects. Safety-related components of the LHC project as the CmS detector with its support, the HF tower and its end cap are analysed for the load case earthquake. The earthquake is defined for the site as a spectra which-after generation of time histories-is deconvoluted to the different levels of foundations and down to the tunnel in 46 m depth. The HF tower consists of 4 risers, each 18 t, a platform with 30 t and the CmS detector including shielding mass with 220 t. Risers and platform are connected by 4 swivel points,

Selection method of Surfactants for Chemical Enhanced Oil Recovery

Alternative energy sources have not yet presented suitable to meet the energy demand of the world;therefore crude oil will play furthermore an essential role in the energy consumption in the future. EOR is a challenging field for several scientific disciplines. The number of patents highlights the importance of this area. most of the publications label that the target of the chemical processes is the reduction of IFT between the displacing liquid and oil phase. Based on the results in the last two decades the surfactants and flow modifier type polymers have shown more potential for a higher efficiency of the EOR than in any other methods. The aim of this work was to develop different surfactant testing methods that are capable to characterize the most important surfactant properties separately and to evaluate their combined or complementary effects. There was no correlation between the specific characteristics. But the weak correlation was detected by using the CE complex evaluation method. Further improvements could contribute significantly to improve the selectivity of the new experimental non-ionic surfactants for EOR applications.

We propose to use the low-coherence property of amplified spontaneous emission(ASE) noise to mitigate optical crosstalk, such as spatial, polarization, and modal crosstalk, which currently limits the density of photonic integration and fibers for dense space-division multiplexing. High optical crosstalk tolerance can be achieved by ASE-based low-coherence matched detection, which avoids dedicated optical lasers and uses spectrally filtered ASE noise as the signal carrier and as a matched local oscillator. We experimentally demonstrate spatial and modal crosstalk reduction in multimode fiber(mmF) and realize mode-and wavelength-multiplexed transmission over1.5-km mmF supporting three spatial modes using a single ASE source. Performance degradation due to model dispersion over mmF is experimentally investigated.
