Objective:The authors present outcomes of robotic liver resections in comparison with open technique questioning the need to have experience of laparoscopy for such procedures before transition to robotic *** and methods:Retrospective review of liver resections done robotically from February 2015 to June 2017 compared to matched control cohort of open cases from January 2012 to December ***:Twenty-one patients in the study groupwere comparedwithmatched control of 42 open cases(1:2 ratio).The types of procedure were similar in both the *** were 4 left lateral hepatectomy,3 left hepatectomy,and 1 left hepatectomy with hepatico-jejunostomy,3 right hepatectomy,3 right posterior sectorectomy,4 bisegmentectomy and 4 *** were 9 patients with primary liver cancer,2 each with livermetastasis and carcinoma gall bladder and 8 patients had benign liver *** blood losswas 370±311ml in the robotic group compared to 451±330ml in control group(p=0.06).Minor complications developed in 19.0%of robotic cases compared to 40.5%in open surgery,while major complications occurred in 4.7%of robotic cases compared to 7.1%of open *** hospital staywas 5.3±0.8 days for the robotic group and 7.7±4.2 days for open group(p=0.02).Local tumor recurrence occurred in 1 out of 13 resections done for malignancy in the robotic group and 7 out of 26 in the open ***:This study highlights the utility of surgical robots for segmental and complex liver resections with equivalent outcomes and decreased length of hospital stay compared to open surgery without having experience of same with the laparoscopy.

Cytogenetic analysis of chromosomal abnormalities in Sri Lankan children

Background: Cytogenetic analysis is a valuable investigation in the diagnostic work up of children with suspected chromosomal disorders. The objective of this study was to describe the prevalence of various types of chromosomal abnormalities in Sri Lankan children undergoing cytogenetic analysis. Methods: Cytogenetic reports of 1554 consecutive children with suspected chromosomal disorders who underwent karyotyping in two genetic centers in Sri Lanka from January 2006 to December 2011 were reviewed retrospectively. Results: A total of 1548 children were successfully karyotyped. Abnormal karyotypes were found in 783 (50.6%) children. Numerical and structural abnormalities accounted for 90.8% and 9.2%, respectively. Down syndrome was the commonest aneuploidy identifi ed. Other various autosomal and sex chromosomal aneuploidies as well as micro-deletion syndromes were also detected. Conclusions: The prevalence of chromosomal abnormalities in Sri Lankan children undergoing cytogenetic analysis for suspected chromosomal disorders was relatively higher than that in Caucasian and other Asian populations.

Daily heart rate variability of Paralympic gold medallist swimmers： A 17-week investigation

Objectives： Heart rate variability （HRV） can be a simple, non-invasive method of gauging cardiac autonomic nervous system fluctuations across periodised training workloads and taper in elite athlete populations. The purpose of these three case studies was to examine daily cardiac autonomic variations in Paralympic athletes leading in to the Paralympic games. Methods： Three Paralympie gold medallist swimmers were monitored daily for their resting HRV over a 17-week monitoring period leading up to the Paralympic games. Specific time- and frequency-domain measures, along with non-linear indices of HRV were calculated for all analyses. All HRV data were analysed individually using daily values, weekly average values, and average values for rest and training phases. Results： A significant difference in HRV was seen for all variables between athlete 1 and athletes 2 and 3 （amputee disabilities） during the entire monitoring period. Conclusion： Despite minimal long-term changes, both swimming classification and disability type significantly influence HRV during athlete monitoring. An increased understanding of individual responses to training, travel, and other outside influences affecting Paralympic athletes could potentially lead to improved management and monitoring of training workloads for enhanced nerformance.

Design of a CMOS Optical Receiver Front-End Using 0.18 μm Technology

This paper reports design of a CMOS optical receiver front-end using 0.18 μm technology. Design process is current associated with photodiode using trans-impedance amplifier (TIA) for wide bandwidth, high gain, low input referred noise and wide dynamic range. The Automated Gain Control (AGC) voltage is used to provide variable gain for multilevel signals. This design is simulated in 0.18 μm UMC technology for the performance analysis. The best simulation results are reported the maximum TIA gain of 67.26 dB? at 0 V AGC followed by a post amplifier gain of 86.70 dB?. The bandwidth range is 7.03 GHz to 11.5 GHz corresponding to 0 - 3 V AGC respectively. The input referred noise level value is 43.86 pA/√Hz up to 10 GHz frequency. In addition authors have obtained the common mode rejection ratio (CMRR) is 72.42 dB and rectified group delay is 144.48 ps. Verification of the design, reported results are compared with earlier published work and improvements obtained in the present results.

Despite the promise of immunotherapy such as the immune checkpoint inhibitors(ICIs)anti-PD-1 and anti-CTLA-4 for advanced melanoma,only 26%–52%of patients respond,and many experience grade III/IV immune-related adverse *** by the need for an effective therapy for patients non-responsive to clinically approved ICIs,we have developed a novel nanoimmunotherapy that combines locally administered Prussian blue nanoparticle-based photothermal therapy(PBNP-PTT)with systemically administered agonistic anti-CD137 monoclonal antibody therapy(aCD137).PBNP-PTT was administered at various thermal doses to melanoma cells in vitro,and was combined with aCD137 in vivo to test treatment effects on melanoma tumor progression,animal survival,immunological protection against tumor rechallenge,and *** administered at a melanoma-specific thermal dose,PBNP-PTT elicits immunogenic cell death(ICD)in melanoma cells and upregulates markers associated with antigen presentation and immune cell co-stimulation in ***,PBNP-PTT eliminates primary melanoma tumors in vivo,yielding long-term tumor-free ***,the antitumor immune effects generated by PBNP-PTT cannot eliminate secondary tumors,despite significantly slowing their *** addition of aCD137 enables significant abscopal efficacy and improvement of survival,functioning through activated dendritic cells and tumor-infiltrating CD8^(+)T cells,and generates CD4^(+)and CD8^(+)T cell memory that manifests in the rejection of tumor rechallenge,with no long-term *** study describes for the first time a novel and effective nanoimmunotherapy combination of PBNP-PTT with aCD137 mAb therapy for melanoma.

Nutrient Removal Efficiencies of <i>Chlorella vulgaris</i>from Urban Wastewater for Reduced Eutrophication

Urban wastewater contains both organic and inorganic nutrients and discharge of untreated water increases nitrogen and phosphorous content in water bodies leading to eutrophication problem. Physical and chemical treatment of urban waste water produces large quantities of waste sludge associated with secondary pollution. Microalgae can assimilate nutrients especially nitrogen and phosphorous from wastewater for their growth and produce valuable biomass and lipid. This study was performed to determine the growth of Chlorella vulgaris in urban wastewater (UWW) and Bold’s basal medium (BBM) thereby identifying cost effective growth medium for microalga cultivation. In addition, nutrient removal abilities of C. vulgaris from various dilutions of urban waste water were explored at 10 days cultivation period. Specific growth rate, biomass and lipid content were higher in microalgae grown in urban waste water than BBM. The highest lipid productivity of 14.31 mg·L?1·day?1 was achieved in the culture grown in UWW medium which exceeded the BBM at 1.15 fold. The amount of nutrient removal tended to increase with higher dilutions of UWW. Removal rates of upto 87.9% and 98.4% were recorded for total nitrogen and total phosphorous by C. vulgaris. The results emphasized that urban waste water as a cost effective growth medium for higher biomass and lipid production accompanied with the nutrient removal efficiency of microalgae to reduce eutrophication.

Ureteric Stone Containing Gas: First Case Report with Review of Cases of Urolithiasis with Gas within the Stone and Its Clinical Implications

We report the first case of a ureteric stone containing gas. This rare stone with gas within it was found during the management of a diabetic patient with urosepsis as the initial presentation. Literature review of cases of renal stone containing gas, mechanisms of gas within the stone, and clinical implications of stone containing gas are discussed. Also, a new terminology is proposed to describe this phenomenon.

Clinical trials in otology:Examining trends and framework for prioritization

Objective:To characterize otologic clinical trials and examine otologic clinical trial trends from 2008 to 2018 using the *** ***:Data was collected from *** and included all clinical trials that focused on otology from 2008 to *** measures include status of trials,funding sources,details regarding otologic conditions studied,and trends in clinical ***:There were 992 otology clinical trials from 2008 to 2018.457(46.1%)were completed and 94(9.5%)were *** remained the highest(76.5%)contributor to otology clinical *** otologic conditions studied,from most common to least common,include hearing loss(40.6%),vestibulopathy(18.8%),tinnitus(18.8%),and otitis media(15.1%).The number of otology clinical trials increased by an average of 12.0 trials per year from 2008 to 2018(p<0.001).The number of otology clinical trials focusing on hearing loss and vestibulopathy significantly increased over the studied period(p<0.001),while those focusing on tinnitus and otitis media did not(p=0.09 and p=0.20,respectively).The majority of clinical trials on each of these four conditions focused on treatment ***:Our study describes trends in otology clinical trials registered on *** from 2008 through *** total number of clinical trials over this time period increased significantly,driven by trials investigating hearing loss and ***,most clinical trials were industrysponsored and focused on treatment *** study provides an outline of otology clinical trials registered in a US web-based database,which may be of use for the development of future clinical trials.
