Mother-to-Child Transmission of Human Papillomavirus in Burkina Faso

Introduction: Human papillomavirus (HPV) infection is the most widespread sexually transmitted infection in the world. Today, there is growing evidence that HPV can be transmitted early in life, and one potential route is mother-to-child transmission. Data on this route of HPV transmission are scarce in Africa and particularly in Burkina Faso, where no data on the subject are yet available. The aim of our study was to estimate the rate of mother-to-child transmission of HPV infection and to identify circulating genotypes. Methodology: cervico-uterine samples were collected from 100 full-term pregnant women and, buccal samples were obtained from their newborns at Hopital Saint Camille de Ouagadougou (HOSCO) by the specialist physician. HPV DNA amplification and genotyping were performed by PCR followed by hybridization using the HPV Direct Flow Chips kit, detecting 36 genotypes including 18 high-risk and 18 low-risk. Results: The prevalence of HPV in newborns was 8% (8/100). Six (6) HPV-positive neonates had HPV-positive mothers, while 2 HPV-positive neonates had HPV-negative mothers. The vertical transmission rate was 26.09% (6/23). Mother-newborn genotypes were concordant. However, the genotype profile of the newborns was more restricted than that of the mothers. Conclusion: HPV DNA was found in 8% of newborns in our study. The genotype profile of the mother-newborn pair was concordant. Asymptomatic HPV infection in a pregnant woman could constitute a risk factor for vertical transmission.

Introduction: cervical cancer is a public health concern and is mainly caused by Human papillomaviruses (HPV). In many parts of the world, studies are being carried out to understand the different genotypes to better tackle this issue. We conducted a study to determine the prevalence of HPV genotypes in women with chronic hepatitis B or C infection, co-infected or not with HIV, treated at the Hôpital Saint Camille in Ouagadougou (Burkina Faso). Methods: This study was conducted from April to July 2023, including 100 women in gastroenterology at Hôpital Saint Camille. A questionnaire on their socio-demographic and life style was administrated;and endocervical samples were collected using sterile swabs and then sent to centre of Biomolecular Research Pietro Annigoni (ceRBA). HPV molecular detection and genotyping were performed by PCR and hybridization using the HPV Direct Flow Chips kit. Data were analysis using chi square test or Fischer’s exact test with a significance threshold for p Results: The prevalence of HPV infection was 28% (28/100) on the sample of women tested. The most frequent genotypes were HPV 52 (8.33%), followed by HPV 18 and 68 (6.25% each) for high-risk HPVs, and HPV 6, 44/55 and 62/81 (8.33% each) for low-risk HPVs. Conclusion: This study, the first of its kind in Burkina Faso on this group of the population, reveals that the most frequent genotypes found in this study are not included in the vaccine available in Burkina Faso (Gardasil®4).

Objectives: Atlantoaxial dislocation remains a rare and serious condition with a high preoperative and postoperative morbidity and mortality. Its successful surgical management is still challenging and gratifying for neurosurgeons. Several technics have been described such as wiring, trans articular screwing, C1C2 screwing with plate and screw introduced by Goel et al., and modified by insertion of polyaxially screw and rod many years later by Harms. Unavailability and expensiveness of upper cervical spine instrumentation device led us to C1C2 Wiring resulting in a good outcome. Finally, a quadriplegic patient with a more comfortable financial condition had ordered devices from abroad and benefit for Goel and Harms screwing technique and improved dramatically from ASIA A to ASIA E. Material and methods: This is a retrospective study of patients managed in our department by a same neurosurgeon from January 2019 to April 2024. Results: We defined 6 men and 1 woman with an average age of 33 years. Unrestrained driver in a rollover motor vehicle accident was most common. Only one patient was neurologically intact on admission. Neurovegetative disorders were noticed in one patient. Dislocation was associated to a fracture of the dens in two patients. Three patients have been successfully operated with remarkable outcome, mostly from ASIA A to E. Conclusion: C1C2 dislocation is a serious condition and C1C2 Wiring represents an effective and cheaper technic. Therefore, this technic should deserve consideration above all in low incomes countries when screwing devices are not available. Seatbelt should be demanded for motor vehicle drivers and passengers.

The aim of the study was to determine the best food absorbents between wheat, rice and maize bran and palm kernel cake, from a technical and economic point of view, in order to make a better recommandation for their use in the production process of food based on mango by-products (peels, peels + pulp). To this end, series of 18 kg of fresh feed were prepared and spread out in a stall for sun-drying using a randomized Fisher system. Preparations were made using either mango peels (75%) + absorbent (25%) or pulp + mango peels (67%) + sorbent (33%). The results show that mango Peel feed loses more water (WL) with lover production yields (PY), higher mango incorporation rates into dry feed (MRI), longer drying times (UDT) and lower production costs (CPkgPD) than mango peel + pulp. The average DM, MAT, ADF and NDF fiber contents were almost equal to those of mango peels + pulp feed. The average Crude Fiber (CF) (25.13%) and DEp (2839 kcal/kgDM) contents were higher for mango skin feed than for mango skin + pulp feed, at 8.59% and 2536 for MAT and DEp respectively. Mango peels + wheat bran (PSB25) and whole mango (MESB33) feeds recorded the highest and almost equal levels of TCP, NDF and MM. Production costs per kg of feed dry mater (CPkgDM) for feed produced at 25% were on average 33% higher than for whole mango (WM) feed. Excluding mango raw material, palm kernel meal (PK), rice bran (RB), maize bran (MB) and wheat bran (WB) can be ranked 1st, 2nd, 3rd and 4th in terms of cumulative performance of production parameters. Producers can then choose the type of sorbent they wish to use according to this ranking and the local availability of the sorbent. These feeds can be used for both ruminants and monogastrics, but are better suited to ruminant feeding due to their high fiber content.

Background: Neoadjuvant chemotherapy (NAC) is one of the treatment options for breast cancer. Its aim is to significantly reduce the size of the tumour in preparation for surgery. The aim of this work is to analyze the conditions of clinical and radiological evaluation of NAC at the Yalgado Ouédraogo University Hospital (CHUYO). Patients and Methods: This was a descriptive cross-sectional study based on the medical records of patients followed up in the cancer department of the CHUYO from 1 January 2013 to 31 December 2021. all patients followed for histologically proven, non-metastatic breast cancer and having received at least one course of NAC were included in this study. The variables were related to the socio-demographic characteristics of the patients, the indications, the protocols of NAC and the sequences of evaluation of the tumour response (clinical, radiological and anatomopathological). Results: We collected 105 cases. The average age of the patients concerned was 44 years. The most frequent histological type was non-specific invasive carcinoma in 97.1% of cases. Immunohistochemically, triple-negative patients accounted for 51.4%. At the initial stage, all patients underwent clinical exploration. Clinical measurement of the tumour was performed in 70.5% of cases. The radiological size of the tumour was determined by ultrasound in 59.1% of cases. One patient had a breast MRI. Thirty-one patients were lost to follow-up after the initial evaluation. At mid-term and at the end of treatment, clinical tumour size was performed in 38.6% and 45.6% of cases respectively. There was no breast imaging performed at mid- and end-of-treatment. CT scans were performed in all cases at baseline, mid-term and end of treatment for extension assessment but did not mention the breast tumour. The tumour response rate was not recorded. Lannea velutina is a traditional herbal remedy used to treat various diseases, including hypertension (HBP). Herbal medicines are affordable, and some have minimal side effects;they are rich in bioactive components that encourage prevention and treatment. This work uses appropriate experimental paradigms to investigate the phytochemical composition, antioxidant activity, and acute oral toxicity of L. velutina trunk bark extracted in water, methanol, ethyl acetate, dichloromethane, and hexane. According to a high-performance thin-layer chromatography profile, this shrub’s bark contains sterols, saponosides, flavonoids, and tannins. Compared to ethyl acetate and aqueous extracts, the methanol extract had the highest total phenolic (607.06 ± 0.6 mg GAE/g DW), flavonoid (20.97 ± 0.23 mg QE/g DW), and condensed tannins (194.50 ± 0.75 ce/g DW) content. The methanol extract displayed the highest antioxidant activity using the 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging capability assay (IC50 = 8.59 g/mL;AAI = 4.66) compared to the other extracts. It exhibited antiradical action comparable to that of Trolox (IC50 = 15.16 g/mL), ascorbic acid (IC50 = 11.94 g/mL), and catechin (IC50 = 11.64 g/mL). The connection between flavonoid concentration and hydrophilic antioxidant activity was robust (r = 0.997). Mice were used to evaluate the acute oral toxicity of ethyl acetate, methanol, and aqueous extracts using guidelines 425 of the Organization for Economic Cooperation and Development. all tested extracts have an estimated LD50 greater than 2000 mg/kg body weight. The truncal bark of L. velutina could be an alternative source for HTA management.

Introduction: Genetic polymorphisms of some Glutathione S-Transferase (GST) which encode the enzyme responsible for the biotransformation of drugs and xenobiotics, have been associated with the risk of several pathologies that can progress to cancer such as Hepatitis B. This study aims to characterize the impact of the rs1695 polymorphism of GSTP1 gene among people with chronic Hepatitis B infection in Burkina Faso. Methods: rs1695 polymorphisms of GSTP1 gene genotyping was performed for 50 people infected with chronic Hepatitis B virus and 124 healthy people with the PCR-RFLP method. Conventional PCR was used for DNA amplification and alw26I enzyme was used for enzymatic digestion. Results: The results show that the frequencies of AA, AG and GG genotypes are respectively 31.00%, 36.80% and 32.20% in general the study population with a mutation rate of 50.57%. However, the incidence of the AA, AG and GG genotypes are respectively 30.64%, 38.71% and 30.64% among people with chronic Hepatitis B virus infection and 32.00%, 32.00% and 36.00% among healthy people. In cases, the frequencies of the A and G alleles are 48.00% and 52.00% respectively, and in controls 50.00% each. No statistical difference was found by comparing genotypic and allelic frequencies between cases and controls (p > 0.05). Conclusion: Our study allowed us to determine the rate of GSTP1 rs1695 genotypes in the study population, cases and controls. From our analyses, GSTP1 rs1695 is not associated to chronic Hepatitis B virus infection in Ouagadougou.

Introduction: cervical cancer, caused by persistent high-risk human papillomavirus (HPV) infection, remains a global public health problem. The cellular transformation and maintenance of the malignant phenotype of these HPVs are attributed to the viral oncoproteins E6 and E7. Objective: This study aims to detect the presence of human papillomavirus DNA and E6/E7 oncoprotein mRNA of HPV genotypes 16, 18, 31 and 33 in cases of cervical cancer and precancerous lesions, histologically confirmed in Burkina Faso. Methods: This descriptive cross-sectional study focused on cases of cervical cancer and high-grade intraepithelial neoplasia (CIN) and was conducted from June to December 2022. One hundred (100) samples of fixed and paraffin-embedded tissues were collected from the pathological anatomy and cytology laboratories of hospitals in the capital of Burkina Faso. High-risk human papillomavirus (HR-HPV) DNA was detected using multiplex real-time PCR, while the presence of E6 and E7 mRNA in cervical cancer and high-grade CIN samples was determined using real-time Reverse Transcriptase-PCR (RT-PCR) with TaqMan probes. Results: The mean age of women diagnosed with cervical cancer and high-grade CIN was 50.81 ± 13.65 years, ranging from 22 to 82 years. cervical cancer and high-grade CIN were positive for at least one high-risk human papillomavirus (HR-HPV) in 80% of cases. The most prevalent genotypes observed were HPV16, 18, 31, and 33, collectively accounting for 70.08% of cases. Of the 89 samples that tested positive for HR-HPV genotypes 16, 18, 31, and 33, 88 (98.88%;95% CI: [94.58 - 99.94]) were also positive for the presence of mRNA encoding the E6 and E7 oncoproteins of HPV16, 18, 31, and 33. Conclusion: In the presence of HPV DNA, testing for E6 and E7 oncoprotein mRNA could serve as a promising biomarker and valuable tool for improved assessment of the progression to cervical cancer.
