Emerging and re-emerging human infectious diseases:A systematic review of the role of wild animals with a focus on public health impact

Infectious diseases continue to impose unpredictable burdens on global health and economies, a subject that requires constant research and updates. In this sense, the objective of the present article was to review studies on the role of wild animals as reservoirs and/or dispersers of etiological agents of human infectious diseases in order to compile data on the main wild animals and etiological agents involved in zoonotic outbreaks. A systematic review was carried out using PRISMA guidelines, using the PubMed, Scopus and SciELO platforms as data banks. The descriptors used were "zoonosis", "human infectious diseases" and "wild animals". The results show that wild animals(mainly bats, birds and primates) play an important role in the dissemination of etiological agents(mainly viruses, as a new coronavirus called 2019 Novel Coronavirus) in extensive geographic regions. Moreover, these wild animal organisms can act as the site for essential biotic synergy among several pathogenic microorganisms, promoting a higher rate of adaptation, mutation and even genetic recombination, with consequent stimulation of new strains and subtypes, inducing new infectious agents with unknown virulent potential. In conclusion, the monitoring of these diseases and adequate preparation for possible epidemics and pandemics are fundamental conditions for the mitigation of their future impact. The zoonotic threat of these etiological agents and the impact on public health can be enormous as shown by the ongoing epidemic of 2019 novel coronavirus(2019-nCoV) infections.

Meningococcal disease is the acute infection caused by Neisseria meningitidis, which has humans as the only natural host. The disease is widespread around the globe and is known for its epidemical potential and high rates of lethality and morbidity. The highest number of cases of the disease is registered in the semi-arid regions of sub-Saharan Africa. In Brazil, it is endemic with occasional outbreaks, epidemics and sporadic cases occurring throughout the year, especially in the winter. The major epidemics of the disease occurred in Brazil in the 70's caused by serogroups A and C. Serogroups B, C and Y represent the majority of cases in Europe, the Americas and Australia. However, there has been a growing increase in serogroup W in some areas. The pathogen transmission happens for respiratory route(droplets) and clinically can lead to meningitis and sepsis(meningococcemia). The treatment is made with antimicrobial and supportive care. For successful prevention, we have some measures like vaccination, chemoprophylaxis and droplets' precautions. In this review, we have described and clarify clinical features of the disease caused by N. meningitidis regarding its relevance for healthcare professionals.

Yellow fever is an acute viral disease endemic to tropical countries, like Brazil, where, since the 1940 s, has no significant documented outbreaks similar to that observed between 2016/2018(2 045 confirmed cases and 677 deaths; caused by the sylvatic form).The principal manipulating factors inciting this change were absence of appropriate vaccination campaigns and increased urbanization & population growth in forest areas, with prevalence of the virus in the species inhabiting of these *** 2016/2018 outbreaks exhibited incidence in areas with historically low or no yellow fever virus activity, triggering a surge in recorded deaths-mainly in the Southeastern states of *** Brazilian government aggressively responded, reforming the countries' prophylactic measures, including vaccine implementation-as of March, 2018, switching from the former double dose regimen of the vaccine, to a single dose protocol, deemed as ***, some states appropriated the fractionated dosage(1/5 of the standard dose), in foresight of potential vaccine *** prevent the uprising of new sylvatic yellow fever cases in Brazil, it's obligatory the development of effective combative plans, including adaptation of prophylactic measures individually(use of repellents, protective clothing etc.), applicable vaccination campaigns in every endemic region, to raise awareness to locals and visitors *** these preventative strategies, the persistence of cases and the recent outbreaks in Brazil, highlight the possible ineffectiveness of combative *** on these considerations, the objective of this review was to raise more awareness of the epidemiological impact of the disease in Brazil.

AIM To investigate the acute toxicity,phytochemical profile,antidiarrheal activity and mechanisms of action of Maytenus erythroxylon(***)ethanol *** A castor oil-induced diarrhea model was used to evaluate antidiarrheal *** transit and gastric emptying protocols were used to evaluate a possible antimotility *** channels,nitric oxide,presynapticα2-adrenergic and tissue adrenergic receptors were investigated to uncover antimotility mechanisms of action and castor oil-induced enteropooling to elucidate antisecretory *** All tested doses of the extract(62.5,125,250 and 500 mg/kg)possessed antidiarrheal activity,with a significant decrease of the evacuation *** activity is possibly related to a reduced gastric emptying(125,250 and 500 mg/kg)and to a decreased percentage of intestinal transit for all tested *** last effect seems to be modulated by nitric oxide,KATP channels and tissue adrenergic ***,the extract also presented antisecretory effect due to a decrease of intestinal fluid *** The antidiarrheal effect of *** found in this study involves antimotility and antisecretory mechanisms that may be attributed to the chemical compounds found in this species:saponins,flavonoids,tannins,triterpenes and steroids.

Arterial embolization of myomas (AEM) is an established option for the conservative treatment of uterine leio-myomas;it treats all present uterine nodules at once, is less invasive than other procedures and effective in controlling symptoms, and does not require long term hospitalizations. Nevertheless, the potential impact on endometrial morphological and functional outcomes after the procedure is still controversial based on reports of endometritis or eventual transient ischemia. This study evaluated endometrial reorganization in uterine leiomyoma patients, before and after AEM, through gene expression analyses of extracellular matrix and cytokines genes in theendometrial tissue. Eight patients with leiomyomas were evaluated before AEM and 6 months after. The examinations included transvaginal pelvic ultrasonography, dosing of the follicle-stimulating hormone, and endometrial biopsy during the second phase of the menstrual cycle. RNA was extracted from endometrial samples, cDNA was synthesized, and applied on PCR arrayTM plates to evaluate the expression of extracellular matrix (ECM) genes and cytokines and their receptors’ genes (CYT). The ECM overexpressed genes were MMP (1, 3, 10, 11, and 14), CTGF1, ICAM1, TBHS1, ITGA2, ITGA3, ITGB3, COL7A1, COL12A, SPP1, and TNC;ADAMTS8 was underexpressed. The CYT overexpressed genes were SPP1, BCL6, CXCL12, IL-8, and CEBPB;CXCL13 and CCL21 were underexpressed. The ECM results showed overexpression of proteases that are responsible for dysfunctions in the ECM, and of genes responsible for adhesion and membrane components. The CYT results showed overexpression of chemokines responsible for endometrial repair, and underexpression of cytokines involved in inflammatory processes in the endometrial tissue. AEM treatment did not negatively affect the endometrial function at 6 months after embolization. GLP-1 receptor agonists are approved for the treatment of type 2 diabetes, and more recently for obesity treatment. The glucagon-like-peptide-1 (GLP-1) is a glucose dependent hormone produced by intestinal cells, which is involved in insulin secretion and glucagon suppression. This hormone controls glucose plasma levels and reduces food intake. Additional effects were reported in slowing gastric emptying and in inducing satiety. In clinical practice, theGLP-1 receptor agonists are associated with significant reductions in glycosylated hemoglobin (HbA1c) and weight loss, despite showing a low risk of hypoglycemia. Beneficial effects have also been observed on blood pressure and lipid profile. The most common side effects associated with GLP-1 receptor agonists are gastro-intestinal motility disorders, such as nausea, vomiting and diarrhea, which are not associated with long-term health risks. Therefore, GLP-1 receptor agonists represent a relevant medication for type 2 diabetes, whose benefits may go far beyond glycemic control.

Spindle cell lipoma is an histological type of lipoma which are rarely found in the oral cavity. We describe two cases of intraoral spindle cell lipomas. The pa- tients were men and presented painless slow growing masses in the left cheek and hard palate, measuring 50 × 30 mm and 23 × 20 mm respectively. Microsco- pically, both lesions presented a solid proliferation of mature fat cells intermixed with bundles of connec- tive tissue. Cells were immunopositive for S100 pro- tein and CD34 (one case), with low mitotic activity (Ki-67). The final diagnosis was spindle cell lipomas. The lesions were excised and no recurrence was no- ticed after six months. Oral spindle cell lipomas are unexpected to occur in the oral mucosa, and the main differential diagnosis is well-differentiated liposar- coma/atypical lipoma. Lesions are treated with sur-gical excision and recurrences are rare.
