Premature Termination of Treatment among Adults Attending Treatment for Eating Disorders: A Critical Review

The article presents a critical review regarding the premature termination of eating disorder’s treatment among inpatients and outpatients, with the aim of identifying—emotional, psychodynamic and family-related aspects involved. The search strategy used the following MeSH terms combined by Boolean operators: “eating disorders” AND “treatment” OR “patient dropouts” OR “drop-out/dropouts” OR “attrition” OR “premature termination” AND “empirical study” OR “qualitative research”, for on Medline/PUBMED, PsycINFO and EMBASE databases. This article follows the PRISMA Guidelines. A total of 26 studies composed this review, of which 24 were original research articles, 1 was a review and 1 a theoretical article. Only two articles applied qualitative methods analyzing categories of content obtained by in-depth interviews, three combine quantitative and qualitative methods and other three present qualitative analyses while discussing quantitative studies. Further qualitative studies should be carried out to clarify meanings of dropout, premature termination of treatment, and attrition. Different expectations held by patients and by therapeutic teams, and the interpersonal difficulties of these types of patients, stand out as difficulties in constructing “therapeutic alliances”, with impacts on dropout, premature termination and attrition rates.

Comparative Study of the Complications of Pedicular Screw Fixation Techniques in the Thoracolumbar Spine: A Systematic Review

Introduction: The use of pedicle screws increases postoperative stability and consolidation of arthrodesis. Pedicle arthrodesis is currently the standard treatment for the thoracolumbar spine, presenting the best fusion and stiffness rates, and among its main indications is the possibility of better correction of spine deformities in the thoracic and lumbar region. However, due to different definitions and the lack of a control group, many of these studies have limited comparative analysis, resulting in the scarcity of comparative studies with standardized methodology. Objective: It was to analyze, through a systematic review, the safety and efficacy of instrumentation with pedicle screws in the spine which have been questioned, despite its wide use for stabilization of the spine, comparing the complications present in the insertion techniques of pedicle screws. Methods: The rules of the Systematic Review-PRISMA were followed. The literary search process was carried out from January to March 2023. A bibliographic search was carried out in MEDLINE, PubMed, and Scielo for articles produced between 2001 and 2023. The quality of the studies was based on the GRADE instrument, and the risk of bias was analyzed according to the Cochrane instrument. The Cohen test (Funnel Plot) and The Heterogeneity Test (Chi-Square Test – X2) were performed, with p Results and Conclusion: A total of 134 articles were found. A total of 67 articles were evaluated in full and 12 were selected to compose the results of this systematic review. According to the GRADE instrument, most studies (X2 = 90.2% > 50%) followed a controlled clinical study model and had a good methodological design, with p < 0.05. It was shown that poor positioning of pedicle screws is the most common cause of complications. Total segmental aplasia of uterus body in bitch

Congenital anomalies of the female tubular genital tract are rare and known as segmental aplasia, resulting from incomplete development of paramesonephric ducts during embryonic stage, which originate from the cranial portion of vagina, cervix, uterus and oviducts. In the literature consulted, several reports of uterine horn aplasia are described in bitches, however there is only one report described in the world literature on total aplasia of the body of the uterus. This study aimed to describe the case of a two years old bitch, no breed defined, with 12 kg of weight, with segmental aplasia of uterus body diagnosed during ovariohysterectomy. During surgery it was noted that the bitch had the ovaries and uterine horns in their anatomical position, however an aplasia was noted in the body of the uterus. It was also found that the uterine horns had liquid inside, suggesting hydrometra. It was concluded in this case that although segmental aplasia of the body of the uterus is rare and difficult to diagnose by maintaining normal cyclicity of the female, it should be considered in dogs that do not have bloody vaginal discharge in proestrus and can not become pregnant after natural coverage or artificial insemination.

Crithidia deanei infection in normal and dexamethasone–immunosuppressed Balb/c mice

Monoxenous trypanossomatids protozoa are not believed to cause in vivo infection in verte- brate hosts throughout their life cycle. However, there are reports mentioning some cases of HIV- positive patients who have presented opportunistic infections caused by these protozoa. Recently, we have demonstrated the in vitro infection of mouse dermal fibroblasts by these protozoa. The aim of the present work is to investigate the possibility of Crithidia deanei, a endosymbiont-bearing monoxenous trypanossomatid, infect BALB/c mice under or not Dexamethasone treatment. To attend it, distinct gro- ups of adult BALB/c mice were immunosuppressed with 50 mg/kg of Dexamethasone. This immunosuppressor was administered 24 hours before infection and daily, for 15 days after C. deanei inoculation. Control groups: C. deanei–inoculated animals but non-immuno- suppressed and non-inoculated animals but immunosuppressed were also used. Light Microscopy analysis revealed an infection process characterized by the presence of the trypanossomatid inside dermal cells in the groups studied. The experimental inoculation resulted in a non-lethal infection characterized by the presence of the trypanossomatid inside dermal cells in the normal BALB/c mice, but notably, in the C. deanei–inoculated immunosuppressed group. These preliminary results lead to the following conclusions: 1) C. deanei is able to infect normal BALB/c mice;2) the immunosupressed mice seemed to be more susceptible to the C. deanei infection compared to the control group. Besides C. deanei in dexamethasone-immunosuppressed mice provides a useful model for studies of monoxenous trypanosomatids ‘in vivo’ infection, resembling that one presumably occurring in imunodeficient individuals with AIDS.
