Background: The trigger finger, also known as stenosing tenosynovitis of the flexor tendon, is a com.on cause of hand disability;This can result in significant lim.tations in certain daily living activities. It is possible to treat this condition with conservative or surgical m.thods. m.thods: This is a retrospective, observational, cross-sectional and descriptive study. The source population consisted of all patients with a diagnosis of trigger finger who had been treated in the departm.nt of plastic and reconstructive surgery of our institution between 2020 and 2023. The prim.ry objective of this study is to determ.ne the prevalence and characterize the clinical and surgical characteristics of the population affected by trigger finger in our hospital. Results: The total population was 237 patients, of which 165 were wom.n (70%) and 72 m.n (30%). The m.st affected hand was the right hand, with a total of 158 patients (67%). In the right hand, the m.ddle finger was the m.st affected (45%). Of the 237 patients, 80 of them.had obesity (34%), 86 had type 2 diabetes m.llitus (36%), 95 had system.c arterial hypertension (40%). Open technique was perform.d in 190 (80%) patients, while percutaneous technique was perform.d in 47 (20%) of them. Conclusion: The fem.le sex was twice as frequent, and the m.ddle finger and thum. were the m.st affected. m.st of the patients received surgical treatm.nt with open approach and percutaneous approach in 20% of them.

Introduction: Post-hospital syndrom. (PHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from.hospital, in which m.ltiple factors com. into play. Hyponatrem.a is the m.st frequent hydroelectrolytic disorder in hospitalized patients and m.y be related to the appearance of PHS. Objective: The objective is to estim.te the prevalence of PHS that is assessed as the rate of readm.ssions in the first 30 days after discharge, in patients with hyponatrem.a. m.terial and m.thods: It is a descriptive observational study of patients with hyponatrem.a who were discharged from.1 Septem.er 2010 to 2 February 2020 at the Internal m.dicine Service of the Hospital University of San Juan (Alicante, Spain). Results: Of the 25 included patients, 5 (20%) were readm.tted within a m.nth of discharge, after a m.an of 11.4 days (standard deviation [SD] 5.1). The overall m.rtality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatrem.a was undeterm.ned. The m.st frequently recorded etiology for the condition was pharm.cological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The m.st widely used corrective m.asure was drug withdrawal, in 16 patients (64%). Water intake restriction was the m.st com.on treatm.nt after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. Conclusion: Hyponatrem.a m.y be the cause of PHS, which could increase the rate of early readm.ssion. Hyponatrem.a is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system.to optim.ze its m.nagem.nt and, in future studies, to assess its im.act on PHS.

目的在肥胖症治疗术前,采用声脉冲辐射力成像(ARFI)通过测定剪切波速度(SWV)技术,评价鉴别非酒精性脂肪肝(NAFLD)与非酒精性脂肪性肝炎(NASH)的能力。方法 32例肥胖症病人术前进行ARFI和常规超声检查。以肝脏活检结果为标准,并与ARFI和超声结果进行比较。根据组织学结果将病人分为3组:A组,单纯脂肪变性;B组,炎性改变;C组,纤维化。结果 SWV值的中位数为(1.57±0.79)m.s。所有病人的组织病理学检查均发现存在肝脏异常改变病变(100%),包括实验室检查正常者。A、B、C3组间的SWV值存在统计学差异,分别为(1.34±0.90)m.s、(1.55±0.79)m.s和(1.86±0.75)m.s(Pm.s,敏感度85%,特异度83.3%)。结论在无症状的肥胖症病人中,ARFI技术是鉴别NAFLD与NASH的有效诊断工具。

Severe Acute Respiratory Syndrom. Coronavirus(SARS-CoV)-2 infection induces an exacerbated inflam.ation driven by innate im.unity *** cells(DCs)play a key role in the defense against viral infections,for instance plasm.cytoid DCs(pDCs),have the capacity to produce vast am.unts of interferon-alpha(IFN-α).In COVID-19 there is a deficit in DC num.ers and IFN-αproduction,which has been associated with disease *** this work,we described that in addition to the DC deficiency,several DC activation and hom.ng m.rkers were altered in acute COVID-19 patients,which were associated with m.ltiple inflam.atory ***,previously hospitalized and nonhospitalized patients rem.ined with decreased num.ers of CD1c+m.eloid DCs and pDCs seven m.nths after SARS-CoV-2 ***,the expression of DC m.rkers such as CD86 and CD4 were only restored in previously nonhospitalized patients,while no restoration of integrinβ7 and indoleam.ne 2,3-dyoxigenase(IDO)levels were *** findings contribute to a better understanding of the im.unological sequelae of COVID-19.
