Gleason score and tumor laterality in radical prostatectomy and transrectal ultrasound-guided biopsy of the prostate： a comparative study

We aimed to compare Gleason score and tumor laterality between transrectal ultrasound-guided biopsy of the prostate （TRUSBX） and radical prostatectomy （RP）. Some factors that could cause a discrepancy in results between these two procedures were also evaluated. Among the 318 cases reviewed, 191 cases were selected for inclusion in this comparative Study, We divided the patients into two groups using the Gleason score： an intermediate/high-grade group （≥7） and a low-grade group （〈6）. Exploratory analyses were conducted for comparisons between groups. We also performed comparisons between TRUSBX and RP for tumor laterality. TRUSBX overestimated 6% and underestimated 24% cases in comparison with RP for Gleason score, and overestimated 2.6% and underestimated 46% cases compared with RP for tumor laterality. Biopsy specimens were slightly smaller in TRUSBX cases with underestimated tumor laterality （P〈 0.05）, and no relationship between the biopsy specimen size and underestimated Gleason score in TRUSBX was found. Prostatic volume showed no statistical correlation with the likelihood of under or overestimation （P 〉 0.05）. Thus, our study showed that TRUSBX has a high likelihood of underestimating both the Gleason score and tumor laterality in prostate cancer （PCa）. The size of the fragment appears to be an important factor influencing the likelihood of laterality underestimation and Gleason score overestimation via TRUSBX. Due to the high likelihood of underestimation of the Gleason score and tumor laterality by 12-core prostate biopsy, we conclude that this type of biopsy should not be used alone to guide therapy in Pca.

Effect of moisture on physical parameters of timber from Turkey oak (Quercus cerris L.) coppice in Central Italy

This study aims to investigate wood density at different levels of moisture, basic density and shrinkage of timber from Turkey oak （Quercus cerris L.） coppice forests growing in Central Italy. We also studied the variability in density in the trees within and among sites. Density shows no significant statistical differences in the tested population. A higher variability in the shrinkage than in the density was found. Wood moisture is referred to as dry mass and fresh mass, which is related to many performance characteristics of wood, i.e., energy production. Trends in moisture and water content were studied because these physical parameters play an important role in the specific area of firewood which requires an accurate estimation of mass, volume and energy content. This work is a contribution to improve xylo-energy estimates of small and medium forestry issues.

Stumpage value in the short wood system for the conversion into high forest of a oak coppice

This study aims to analyze the factors affecting the costs of different logging systems, in particular considering three different bunching-extraction methods. Moreover the stumpage value of each plot was calculated, and a comparison analysis of the results is presented. The work was carried out in Central Italy, in an aged Quercus cerris L. coppice in hill zones, with a 45% average slope. Short wood system harvesting was applied. Felling and processing were performed by chainsaw, while extraction with three different methods： plot No. 1 with mules, plot No. 2 with a tractor with winch, plot No. 3 with polyethylene chute line. Transport of firewood from the forest track to the landing was made by a tractor loaded as a mule. In the plot No. 1 the costs per unit mass （30.76 ∈·t33% ^-1） were higher despite having the lower cost per hour （39.99∈·th^- 1）. The reason was the lower productivity （1.3 t33%·h^-1）. The plot No. 2 had the highest cost per hour （66.79 ∈·h^-1）. However, productivity was the highest （2.7 t33%·h^- 1）. This aspect was notable from the financial point of view （24.74 ∈·t33%^-1）. Plot No. 3 was the less expensive （23.92 ∈·t33%^-1）. Comparing the three methods, a reduction of the costs through more appropriate extraction systems may increase the stumpage value from 8.3 to 9.65 ∈·t33%^- 1.

The need to standardise stratigraphical subdivisions of continuous marine sedimentary successions that outcrop for hundreds metres, using clearly defined points fixed in the field is strongly felt. A proposal is presented here to formalize regional stages for the Lower Pleistocene （Calabrian） and for the Middle Pleistocene （Ionian） in Italy. The sections are well exposed, carefully investigated using multiple criteria （an integrated stratigraphical approach） and located in the central part of the Mediterranean. This area is recognized worldwide as a classical region for the Neogene and the Pleistocene sequences. The terrestrial record is directly correlated with the deep-sea record, as determined from ODP Sites 653 （Tyrrhenian Sea）, 963 （Sicily Channel） and 964 （Ionian Sea）from the central Mediterranean. The present formal proposal is consistent with the INQUA and ICS decision to define and correlate the Plio/Pleistocene boundary （GSSP at Vrica） within the Olduvai Subchron, at 1.81 My, the Lower/Middle Pleistocene boundary at the Matuyama/Brunhes reversal at 0. 78 My and the Middle/ Upper Pleistocene boundary with the base of MIS 5, approximately 0. 13My. The present proposal also is consistent with the call for stage unit stratotypes that emphasize both the physical content of a stage and its boundaries （Hilgen et al., 2006）.

Desimplification to multi-tablet antiretroviral regimens in human immunodeficiency virus-type 1 infected adults: A cohort study

BACKGROUND Highly active antiretroviral therapy (HAART) is provided free of charge to all human immunodeficiency virus (HIV) positive residents in Italy. As fixed dose coformulations (FDCs) are often more expensive in comparison to the same drugs administered separately in a multi-tablet regimen (MTR), we considered a costeffective strategy involving patients in the switch from their FDCs to corresponding MTRs including generic antiretrovirals. AIM To verify if this would affect the virological and immunological response in comparison to maintaining the FDC regimens. METHODS From January 2012 to December 2013, we assessed the eligibility of all the HIV-1 positive adults on stable HAART being treated at our hospital-based outpatient clinic in Treviso, Italy. Participants who accepted to switch from their FDC regimen to the corresponding MTR joined the MTR group, while those who maintained a FDC regimen joined the FDC group. Clinical data, including changes in HAART regimens, respective reasons why and adverse effects, were recorded at baseline and at follow-up visits occurring at weeks 24, 48 and 96. All participants were assessed for virological and immunological responses at baseline and at weeks 24, 48 and 96. RESULTS Two hundred and forty-three eligible HIV-1 adults on HAART were enrolled: 163 (67%) accepted to switch to a MTR, joining the MTR group, while 80 (33%) maintained their FDCs, joining the FDC group. In a parallel analysis, there were no significant differences in linear trend of distribution of HIV-RNA levels between the two groups and there were no significant odds in favour of a higher level of HIV-RNA in either group at any follow-up and on the overall three strata analysis. In a before-after analysis, both FDC and MTR groups presented no significant differences in distribution of HIV-RNA levels at either weeks 48 vs 24 and weeks 96 vs 24 cross tabulations. Estrogen receptor modulators are compounds of interest because of their estrogenic agonistic/antagonistic effects and tissue specificity. These compounds have many clinical applications, particularly for breast cancer treatment and osteoporosis in postmenopausal women, as well as for the treatment of climacteric symptoms. Similar to estrogens, neuroprotective effects of estrogen receptor modulators have been described in different models. However, the mechanisms of action of these compounds in the central nervous system have not been fully described. We conducted a systematic search to investigate the effects of estrogen receptor modulators in the central nervous system, focusing on the modulation of cytoskeletal proteins. We found that raloxifene, tamoxifen, and tibolone modulate some cytoskeletal proteins such as tau, microtuble-associated protein 1（MAP1）, MAP2, neurofilament 38（NF38） by different mechanisms of action and at different levels： neuronal microfilaments, intermediate filaments, and microtubule-associated proteins. Finally, we emphasize the importance of the study of these compounds in the treatment of neurodegenerative diseases since they present the benefits of estrogens without their side effects.

Streptococcus pyogenes(group A Streptococcus)is frequently involved in a wide range of human *** we evaluated polymorphonuclear neutrophils and mononuclear cells from healthy subjects for their bactericidal function after stimulation with live and inactivated Streptococcus pyogenes(Streptococcus Group A).Mononuclear cells and Neutrophils were isolated from heparinized blood samples(n=18)using a Ficoll-Hypaque gradient and cultured in RPMI 1640 for 18 hours with a suspension of either live or inactivated Streptococcus *** the respiratory burst(flow cytometry)and nitrite,TNF and IL17 production(ELISA)were measured in the cell culture *** increased respiratory burst(expressed as R index)was induced by both live and inactivated ***,increased nitrite,TNF and IL17 concentrations were found in cell culture supernatants in both *** findings may provide some explanation as to the roles played by neutrophils and mononuclear cells in Streptococcus pyogenes immunopathogenicity。

Extraction method of nanoparticles concentration distribution from magnetic particle image and its application in thermal damage of magnetic hyperthermia

Magnetic particle imaging(MPI)technology can generate a real-time magnetic nanoparticle(MNP)distribution image for biological tissues,and its use can overcome the limitations imposed in magnetic hyperthermia treatments by the unpredictable MNP distribution after the intratumoral injection of ***,the MNP concentration distribution is generally difficult to be extracted from MPI *** study proposes an approach to extract the corresponding concentration value of each pixel from an MPI image by a least squares method(LSM),which is then translated as MNP concentration distribution by an interpolation *** resulting MPI-based concentration distribution is used to evaluate the treatment effect and the results are compared with the ones of two baseline cases under the same dose:uniform distribution and MPI-based distribution considering ***,the treatment effect for all these cases is affected by the blood perfusion rate,which is also investigated deeply in this *** results demonstrate that the proposed method can be used to effectively reconstruct the concentration distribution from MPI images,and that the weighted LSM considering a quartic polynomial for interpolation provides the best results with respect to other cases ***,the results show that the uniformity of MNP distribution has a positive correlation with both therapeutic temperature distribution and thermal damage degree for the same dose and a critical power dissipation value in the *** MNPs uniformity inside biological tissue can be improved by the diffusion behavior after the nanofluid injection,which can ultimately reflect as an improvement of treatment *** addition,the blood perfusion rate considering local temperature can have a positive effect on the treatment compared to the case which considers a constant value during magnetic hyperthermia.
