Simple anthropometric measurements to predict dyslipidemias in Mexican school-age children: a cross-sectional study

Objective: The purpose of this study was to identify the best predictors of dyslipidemias in Mexican obese children using different anthropometric and body composition measurements. Methods: In an observational, cross-sectional study, 905 children from 5 schools were measured for weight, height, waist and hip circumference, and triceps and subscapular skinfolds. A fasting blood sample was taken from a random sub-sample of 306 children to determine lipid profile. Abnormal total cholesterol, LDL, HDL, triglycerides, total cholesterol to HDL ratio, and LDL to HDL ratio, were determined. Logistic regressions and ROC analysis were carried out to determine the best anthropometric predictors of these risk factors. Results: Prevalence of elevated total cholesterol, triglycerides and LDL cholesterol was 14%, 56% and 58%, respectively. In logistic regressions, BMI and triceps skinfold had the highest odds ratios to predict elevated total cholesterol (1.05, 95%CI: 0.97 - 1.14;1.07, 1.01 - 1.13, respectively), triglycerides (1.19, 1.11 - 1.27;1.12, 1.08 - 1.17, respectively), LDL cholesterol (1.11, 1.04 - 1.18;1.09, 1.05 - 1.14, respectively), total cholesterol to HDL ratio (1.06, 1.00-1.14;1.07,1.03-1.12, respectively) and LDL to HDL ratio risk (1.08,1.01-1.15;1.07, 1.03-1.12, respectively). After BMI and triceps skinfold, subscapular skinfold also predicted dyslipidemias, except for low HDL;both skinfolds had a narrower odds ratio confidence interval than BMI. In ROC analysis, subscapular skinfold was the best predictor of elevated triglycerides with an AUC ≥ 0.7. Conclusion: Anthropometric measurements are not strongly associated with dyslipidemias in Mexican children. However, since triceps and subscapular skinfolds were better predictors than other anthropometry measures, they may be a simple way to predict dyslipidemias in Mexican children.

Alpha-1 antitrypsin deficiency and the risk of hepatocellular carcinoma in end-stage liver disease

AIM:To evaluate the association between alpha-1 antitrypsin deficiency(A1ATD) and hepatocellular carcinoma(HCC) in patients with end-stage liver disease(ESLD).METHODS:Patients with cirrhosis and ESLD referred to the Cleveland Clinic Foundation for liver transplantation between 2003 and 2014 were included in the study(N = 675). ESLD was defined as having histological features of cirrhosis and/or radiological evidence of cirrhosis in the context of portal hypertension(ascites,variceal bleeding,thrombocytopenia,or hepatic encephalopathy). A1 ATD was diagnosed using phenotype characterization(MZ or ZZ),liver biopsy detection of PAS-positive diastaseresistant(PAS+) globules,or both. Patients with other causes of liver diseases such as hepatitis C virus(HCV),alcoholic liver disease and non-alcoholic steatohepatitis(NASH) or NASH were also included in the study. HCC was diagnosed by using imaging modalities,biopsy findings,or explanted liver inspection. Follow-up time was defined as the number of years from the diagnosis of cirrhosis to the diagnosis of hepatocellular carcinoma,or from the diagnosis of cirrhosis to the last follow up visit. The rate of HCC was assessed using time-tointerval analysis for interval censored ***:This study included 675 patients. 7% of subjects had A1ATD(n = 47). Out of all subjects who did not have A1 ATD,46% had HCV,17% had alcoholic liver disease,19% had NASH and 18% had another primary diagnosis. Of the 47 subjects with A1 ATD,15 had a primary diagnosis of A1ATD(PI*ZZ phenotype and PAS+ globules),8 had a PI*MZ phenotype alone,14 had PAS+ alone,and 10 had both the PI*MZ phenotype and PAS+. Median follow-up time was 3.4(25th,75 th percentiles:1,5.2) years. The overall rate of hepatocellular carcinoma in all subjects was 29%(n = 199). In the A1 ATD group,the incidence rate of HCC was 8.5% compared to 31% in the group of patients with other causes of cirrhosis(P = 0.001). This study compared the potential of assessing sperm DNA fragmentation （SDF） from neat semen and the subsequent swim-up （SU） procedure to predict pregnancy when conducting ICSI of fertile donor oocytes. Infertile females （n=81） were transferred embryos resulting from intracytoplasmic sperm injection （ICSI） of their partner＇s spermatozoa and proven donor oocytes. This model normalized the impact of female factor in putative sperm DNA repair. Semen was blindly assessed for SDF using Halosperm immediately following ejaculation （NS） and after swim-up at the time of ICSI fertilisation. There was a decrease in SDF values of the ejaculated semen sample following the swim-up protocol （P=0.000）. Interestingly, pregnancy could be equally predicted from SDF values derived from either neat or swim-up semen samples. Receiver operator curves and the derived Youden＇s indices determined SDF cutoff values for NS and SU of 24.8% and 17.5%, respectively. Prediction of pregnancy from NS SDF had a sensitivity of 75% and a specificity of 69%, whereas for SU SDF was 78% and 73%, respectively. While increased levels of SDF negatively impact reproductive outcome, we have shown that a reduction in SDF following sperm selection using ICSI with proven donor oocytes is not mandatory for achieving pregnancy. This suggests that a certain level of DNA damage that is not detectable using current technologies could be impacting on the relative success of assisted reproductive technology （ART） procedures. Consequently, we propose a modification of the so called ＇iceberg model＇ as a possible rationale for understanding the role of SDF in reproductive outcome.

Canopy Storage Implications on Interception Loss Modeling

A rainfall interception methodology was implemented in a deciduous Ficus benjamina (L.) tree to evaluate the interception loss, as well as the dynamics on canopy storage capacity (S) and free through fall (ρ). Measurements of gross precipitation (Pg), through fall and meteorological data were recorded every 5 minutes. Nineteen individual storms from summer to autumn 2005, and twenty one in spring to autumn, 2006 were analyzed. For the studied period, 151.59 mm and 203.35 mm of rainfall occurred on 2005 and 2006 respectively. Canopy interception was 59.46% and 70.98% of Pg for the first and second year. Throughfall data recorded during 2005 were ?38.14% (of Pg) and 27.21% (of Pg) for 2006. The throughfall and gross precipitation relationship yielded S = 1.50 mm for the 2005 data. In 2006 storms were analyzed in detail, where ρ and S varied in a range from 0.10 to 0.64 and from 1.00 to 2.03 mm, respectively. Moreover, application of the Rutter and Gash models with two years of rainfall data (2005-2006) from the study area indicated an underestimation and overestimation of 69% and 88%, respectively. The slightly best prediction of the interception loss was obtained with the Gash model. Yet S and ρ change significantly due to wind speed, temperature, rainfall intensity and seasonal vegetation development.

Intensive outpatient comprehensive behavioral intervention for tics: A case series

Recent randomized clinical trials have established the efficacy of Comprehensive Behavioral Intervention for Tics(CBIT) in treating children and adults with Tourette syndrome and persistent tic disorders. However, the standard CBIT protocol uses a weekly outpatient treatment format(i.e., 8 sessions over 10 wk), which may be inconvenient or impractical for some patients, particularly patients, who are required to travel long distances in order to receive care. In contrast, an intensive outpatient program may increase accessibility to evidence-based behavioral treatments for Tourette syndrome and other persistent tic disorders by eliminating the necessity of repeated travel. This case series evaluated the use of an intensive outpatient program CBIT(IOP CBIT) for the treatment of 2 preadolescent males(ages 10 and 14 years) with Tourette syndrome. The IOP CBIT treatment protocol included several hours of daily treatment over a 4-d period. Both children evi-denced notable reductions in their tics and maintained treatment gains at follow-up. Moreover, both patients and their parents expressed treatment satisfaction with the IOP CBIT format. This case series addresses an important research gap in the behavioral treatment of tic disorders literature. The patients' treatment outcomes indicate that IOP CBIT is a promising treatment that warrants more systematic investigation.
