Post-dural puncture headache continues to be a significant cause of morbidity in parturients. Despite being a common complication faced by many anesthesiologists, there is a lack of consensus regarding its management. Many still use traditionally taught treatments such as strict bed rest and aggressive hydration despite lack of evidence for their usage. Few are using newly tested treatments such as gabapentin and ACtH despite being proven effective in randomized controlled trials. Furthermore, when and how the epidural blood patch should be used is contentious between different practitioners. this review aims at answering what is the best strategy to manage post-dural puncture headache and proposes an evidence-based practice guideline.

the article by Jinghai Li in this issue of Engineering[1] calls for a new approach to how we organize science and technology （S＆t）, given that we are encountering more challenges than solutions. His paper is a plea and a demand for more transdisciplinary ap- proaches to solving real-world, complex challenges, in the light of the reductionist approach that results in disciplines splintering into ever tighter groupings. His own approach of using mesoscale considerations to link the microscale and macroscale has been demonstrably successful over many years.

Different genes are expressed in different tissues, depending on functional objectives and selection pressures. Based on complete knowledge of the structure of the metabolic network and all the reactions taking place in the cell, elementary modes (EMs) and minimal cut sets (MCSs) can relate compounds observed in a tissue, to the genes being expressed by respectively providing the full set of non-decomposable routes of reactions and compounds that lead to the synthesis of external products, and the full set of possible target genes for blocking the synthesis of external products. So, for a particular tissue, only the EMs containing the reactions that are related to the genes being expressed in those tissues, are active for the production of the corresponding compounds. this concept is used to develop an algorithm for determining a matrix of reactions (which can be related to corresponding genes) taking place in a tissue, using experimental observations of compounds in a tissue. the program is applied to the Arabidopsis flower and identified 20 core reactions occurring in all the viable EMs. they originate from the trans-cinnamate compound and lead to the formation of kaempferol and quercetin compounds and their derivatives, as well as anthocyanin compounds. Analyses of the patterns in the matrix identify reaction sets related to certain functions such as the formation of derivatives of the two anthocyanin compounds present, as well as the reactions leading from the network’s external substrate erythrose-4P to L-Phenylalanine, cinnamyl-alc to trans-cinnamate and so on. the program can be used to successfully determine genes taking place in a tissue, and the patterns in the resulting matrix can be analysed to determine gene sets and the state of the tissue.

Purpose:the aim of this study was to review,systematically,evidence concerning the link between the ACtN3 R577X polymorphism and the rates and severity of non-contact injuries and exercise-induced muscle damage in athletes and individuals enrolled in exercise training ***:A computerized literature search was performed in the electronic databases PubMed,Web of Science,and SPORtDiscus,from inception until November *** included studies compared the epidemiological characteristics of non-contact injury between the different genotypes of the ACtN3 R577X ***:Our search identified 492 *** the screening of titles,abstracts,and full texts,13 studies examining the association between the ACtN3 genotypes and the rate and severity of non-contact injury were included in the *** studies were performed in 6 different countries(Spain,Japan,Brazil,China,the Republic of Korea,and Italy)and involved a total participant pool of 1093 *** the studies,2 studies involved only women,5 studies involved only men,and 6 studies involved both men and *** the studies included were classified as highquality studies(≥6 points in the Physiotherapy Evidence Database(PEDro)scale score).Overall,evidence suggests there is an association between the ACtN3 R577X genotype and non-contact injury in 12 *** studies observed a significant association between A CtN3 R577X polymorphism and exercise induced muscle damage:2 with non-contact ankle injury,3 with non-contact muscle injury,and 1 with overall non-contact ***:the present findings support the premise that possessing the ACtN3 XX genotype may predispose athletes to a higher probability of some non-contact injuries,such as muscle injury,ankle sprains,and higher levels of exercise-induced muscle damage.
