Risk factors associated with pancreatic fistula after distal pancreatectomy, which technique of pancreatic stump closure is more beneficial?

AIM: To identify risk factors related to pancreatic fistula in patients undergoing distal pancreatectomy (DP) and to determine the effectiveness of using a stapled and a sutured closed of pancreatic stump. METHODS: Sixty-four patients underwent DP during a 10-year period. Information regarding diagnosis, operative details, and perioperative morbidity or mortality was collected. Eight risk factors were examined. RESULTS: Indications for DP included primary pancreatic disease (n = 38, 59%) and non-pancreatic malignancy (n = 26, 41%). Postoperative mortality and morbidity rates were 1.5% and 37% respectively; one patient died due to sepsis and two patients required a reoperation due to postoperative bleeding. Pancreatic fistula was developed in 14 patients (22%); 4 of fistulas were classified as Grade A, 9 as Grade B and only 1 as Grade C. Incidence of pancreatic fistula rate was significantly associated with four risk factors: pathology, use of prophylactic octreotide therapy, concomitant splenectomy, and texture of pancreatic parenchyma. The role that technique (either stapler or suture) of pancreatic stump closure plays in the development of pancreatic leak remains unclear. CONCLUSION: The pancreatic fistula rate after DP is 22%. This is reduced for patients with non-pancreatic malignancy, fibrotic pancreatic tissue, postoperative prophylactic octreotide therapy and concomitant splenectomy.

Miscibility and crystallization have been studied for polypropylene-polyethylene and polyethylene-polyethyleneblends. In the case of the polypropylene blends the composition of interest is 20% polypropylene. At this composition thepolypropylene has been found to be soluble in linear low density polyethylene but insoluble in high, low and very lowdensity polyethylenes. The miscibility has been concluded from the crystallization kinetics and polarised optical microscopywith a hot stage. Polyethylene-polyethylene blends have been formed from polymers with similar average branching contentbut where they have different melting temperatures. Important consequences are to introduce long branches into apolyethylene that only has short branches, and to modify the morphology of a polyethylenes so that haze, gloss and strainhardening are improved. Polyethylene blends must be developed after careful consideration of the branch content anddistribution within each of the constituents. It is not sufficient to simply blend polyethylenes, with the desired range ofproperties, without regard to the miscibility of the blend composition.

Wavelet Cleaning of Solar Dynamic Radio Spectrograms

By applying the state-of-the-art mathematical apparatus, the wavelet transformation, we explore the possibility of a dynamic cleaning of raw data ob- tained with the Chinese solar radio spectrographs over a wide wavelength range (from 0.7 to 7.6 GHz). We consider the problem of eliminating the interference caused by combination rates of data sampling (10-20 ins), and the low-frequency interference (4-30 s) caused by the receiving equipment changing its characteristics with time. It is shown that the best choice to reconstruct a signal suffering from amplitude, frequency and phase instabilities, is by means of wavelet transformation at both high and low frequencies. We analysed observational data which contained interferences of nonsolar origin such as instrumental effects and other man-made signals. A subsequent comparison of the reference data obtained with the acousto- optical receiver of the Siberian Solar Radio Telescope (SSRT) with the 'cleaned' spectra confirms the correctness of this approach.

An Encyclopedic Approach to the Conservation of 20^(th)-Century Architecture

The built heritage of the 20^(th) century concerns society in general: it is the background to everyday life and the stage of ‘past experience’. Therefore, recent past buildings should not be considered merely for their aesthetic features;they also have to do with physical well-being, social rituals and representations, as well as with associated concepts such as values and emotions. But conservation of 20^(th)-century heritage is not straightforward. It requires specific strategies as well as critical and operational tools, which are not part of the cultural background of the actors of the transformation of our cities (architects, engineers, heritage officials, etc.) and do not yet feature in current design teaching curricula in most universities. To fill these gaps, in 2008 four Swiss architectural schools (USI, EPFL, ETHZ, SUPSI) launched a research project titled ‘Critical Encyclopaedia for reuse and restoration of 20^(th)-century architecture’ (2009-2013). The article presents one of the outcomes of this research: the work developed by the section titled ‘Historical and Critical Tools for Conservation’, which will be published shortly. It provides readers with the cultural, theoretical and critical frame of reference required to understand recent architectural heritage in its widest sense: as a historical, symbolic and aesthetic resource, but also as being endowed with social, economic and ecological value. By examining theories and doctrines which have developed over time and thanks to the exemplary nature and variety of the selected cases, it provides a historical appraisal of how intervention in the field of recent heritage has evolved over the past 60 years. On the criteria to create a susceptibility map to debris flow at a regional scale using Flow-R

Studies on susceptibility to debris flows at regional scale （ioo-looo km2） are important for the protection and management of mountain areas. To reach this objective, routing models, mainly based on land topography, can be used to predict susceptible areas rapidly while necessitating few input data. In this research, Flow-R model is implemented to create the susceptibility map for the debris flow of the Vizze Valley （BZ, North-Eastern Italy; 134 km^2）. The analysis considers the model application at local scale for three sub-catchments and then it explores the model upsealing at the regional scale by verifying two methods to generate the source areas of debris-flow initiation. Using data of an extreme event occurred in the Vizze Valley （4 August 2012） and historical information, the modeling verification highlights that the propagation parameters are relatively simple to set in order to obtain correct runout distances. A double DTM filtering - using a threshold for the upslope contributing area （0.1 km^2） and a threshold for the terrain-slope angle （15°） provides a satisfactory prediction of source areas and susceptibility map within the geological conditions of the Vizze Valley.
