Epidemiology of insertional and midportion Achilles tendinopathy in runners:A prospective cohort study

Background:Achilles tendinopathy(AT)is a common problem among runners.There is only limited evidence for risk factors for AT,and most studies have not defined the AT subcategories.No study has compared the incidence and risk factors between insertional AT and midportion AT,though they are considered distinct.This study aimed to assess incidence and risk factors of AT based on data from a large prospective cohort.The secondary aim was to explore differences in risk factors between insertional and midportion AT.methods:Participants were recruited from among registered runners at registration for running events.Questionnaires were completed at baseline,1 month before the event,1 week before the event,and 1 month after the event.Information concerning demographics,training load,registered events,and running-related injuries were collected at baseline.The follow-up questionnaires collected information about new injuries.A pain map was used to diagnose midportion and insertional AT.The primary outcome was the incidence of AT.multivariable logistic regression analysis was applied to identify risk factors for the onset.Results:We included 3379 participants with a mean follow-up of 20.4 weeks.The incidence of AT was 4.2%.The proportion of insertional AT was 27.7%and of midportion AT was 63.8%;the remaining proportion was a combined type of insertional and midportion AT.men had a significantly higher incidence(5%,95%confidence interval(95%CI):4.1%-6.0%)than women(2.8%,95%CI:2.0%-3.8%).AT in the past12 months was the most predominant risk factor for new-onset AT(odds ratio(OR)=6.47,95%CI:4.27-9.81).This was similar for both subcategories of AT(insertional:OR=5.45,95%CI:2.51-11.81;midportion:OR=6.96,95%CI:4.24-11.40).Participants registering for an event with a distance of 10/10.55 km were less likely to develop a new-onset AT(OR=0.59,95%CI:0.36-0.97)or midportion AT(OR=0.47,95%CI:0.23-0.93).Higher age had a significant negative association with insertional AT(OR=0.97,95%CI:0.94-1.00).Conclusion:The incidence of new-onset AT

To the editor:In the USA alone,there were 1.7 million suicide attempts in 2021.1 Although the majority of suicide attempts are not fatal,immediate medical attention is often necessary,especially when violent methods are used.2 These methods include jumping from heights,using a firearm,crashing a motor vehicle,and lying down or jumping in front of a moving object.

Background Commercial diets are frequently formulated to meet or exceed nutrient levels including those of lim-iting essential amino acids(AA)covering potential individual variations within the herd.However,the provision of dietary excess of AA,such as Lys,may lead to reduced appetite and growth in pigs.The mechanisms modulat-ing these responses have not been extensively investigated.This study evaluated the effect of Lys dietary excesses on performance and satiety biomarkers in post weaning pigs.methods Twenty-four pigs aged 21 d and weighing 6.81±0.12kg(mean±SEm)were individually housed and offered 1 of 4 dietary treatments for 3weeks:a diet containing a standardized ileal digestible Lys reaching 100%(T0),120%(T1),150%(T2)or 200%(T3)of the NRC(2012)requirements.At the end of the experiment,blood samples from the cephalic vein of the T0 and T3 groups were obtained for AA analysis.In addition,primary intestinal cultures from T0 pigs were used,following their humane killing,to evaluate the effect of Lys on gut hormone secretion and AA sensors gene expression under ex vivo conditions.Results Feed intake was linearly reduced(Pmetabotropic glutamate receptor 1 and the solute carrier family 7 member 2(Pmal jejunum tissues incubated with Lys at 20mmol/L when compared to the control(Lys 0mmol/L).Plasma Lys and Glu(Pmpared to T0 pigs.In contrast,plasma levels of His,Val,Thr,Leu(Pm that excess dietary Lys inhibits hunger in pigs.moreover,the results provide evidence of pre-and post-absorptive mechanisms modulating these responses.Lys dietary excesses should be nar-rowed,when possible,to avoid negative effects of the AA on appetite in pigs.

A detailed rock density model of the Hong Kong territories

We used the geological map and published rock density measurements to compile the digital rock density model for the Hong Kong territories.We then estimated the average density for the whole territory.According to our result,the rock density values in Hong Kong vary from 2101 to 2681 kg·m^(-3).These density values are typically smaller than the average density of 2670 kg·m^(-3),often adopted to represent the average density of the upper continental crust in physical geodesy and gravimetric geophysics applications.This finding reflects that the geological configuration in Hong Kong is mainly formed by light volcanic formations and lava flows with overlying sedimentary deposits at many locations,while the percentage of heavier metamorphic rocks is very low(less than 1%).This product will improve the accuracy of a detailed geoid model and orthometric heights.

Surgeon preferences in the treatment of thumb carpometacarpal osteoarthritis

BACKGROUND Primary thumb carpometacarpal(CmC)osteoarthritis is one of the most common conditions encountered by hand surgeons.Of the vast number of operations that have been proposed,none have demonstrated results significantly superior to trapeziectomy alone.AIm The purpose of our study was to determine why surgeons opt for their technique in treating CmC arthritis.mETHODS A cross-sectional survey of active members of the American Society for Surgery of the Hand was conducted to evaluate the reasons behind their preferred technique in the treatment of isolated thumb CmC arthritis.Surgeons were contacted by email once and provided with a link to a de-identified survey consisting of 5 treatment questions and 5 demographic questions.RESULTS Of 950 responses were received.40.5%of surgeons preferred trapeziectomy+ligament reconstruction tendon interposition(LRTI),followed by trapeziectomy+suspensionplasty(28.2%),suture button suspension(5.9%),trapeziectomy alone(4.6%),prosthetic arthroplasty(3.2%),arthrodesis(1.1%),and other(6.6%).Proponents of trapeziectomy+LRTI cited familiarity(73.2%),exposure during fellowship(48.8%)and less proximal migration(60%)to be the main reasons affecting their decision.Surgeons who preferred trapeziectomy+suspensionplasty most reported simplicity(74.9%),fewer complications(45.3%),less proximal migration(43.8%),and avoidance of autogenous tissue harvest(42.7%).Advocates of suture button suspension cited avoidance of autogenous tissue harvest(80.4%),shorter immobilization(76.8%),and quicker recovery(73.2%)with their technique.Advocates of trapeziectomy alone cited simplicity(97.7%),fewer complications(86.4%),and avoidance of autogenous tissue harvest(59.1%).In their comments,45%of surgeons choosing trapeziectomy alone cited evidence as an additional rationale.Advocates of prosthetic arthroplasty cited improved pinch strength(83.3%)and improved range of motion(63.3%),while those preferring arthrodesis cited better pinch strength(90%)and frequently in their comments,durability.Of the surgeon

Reacting particle systems play an important role in many industrial applications,for example biomass drying or the manufacturing of pharmaceuticals.The numerical modeling and simulation of such systems is therefore of great importance for an efficient,reliable,and environmentally sustainable operation of the processes.The complex thermodynamical,chemical,and flow processes that take place in the particles are a particular challenge in a simulation.Furthermore,typically a large number of particles is involved,rendering an explicit treatment of individual ones impossible in a reactor-level simulation.One approach for overcoming this challenge is to compute effective,physical parameters from single-particle,high-resolution simulations.This can be combined with model reduction methods if the dynamical behaviour of particles must be captured.Pore network models with their unrivaled resolution have thereby been used successfully as high-resolution models,for instance to obtain the macroscopic diffusion coeffcient of drying.Both parameter identification and model reduction have recently gained new impetus by the dramatic progress made in machine learning in the last decade.We report results on the use of neural networks for parameter identification and model reduction based on three-dimensional pore network models(PNm).We believe that our results provide a powerful complement to existing methodologies for reactor-level simulations with many thermally-thick particles.

Potential of contemporary earth-sheltered buildings to achieve Plus Energy status in various European climates during the heating season

This study investigates the potential of contemporary Earth-Sheltered Buildings to contribute to sustainable societies by achieving Plus Energy Building status during the heating season.The buildings analyzed are intended to house a four-member family(102.5 m^(2)).The house is equipped with a ground source heat pump,geothermal vertical probes,floor heating panels,an energy recovery ventilator,and building-integrated photovoltaic panels installed on the exposed building wall.The numerical analysis(using EnergyPlus software)was carried out for nine European sites covering five different climatic zones(namely:southern,Atlantic,continental,alpine,and northern climate)and three latitude belts spanning Europe from south to north.Buildings above 65°N(northern and alpine climate)could not achieve Plus Energy status with 20% efficient BIPVs.With 15% efficient BIPVs,the building above 59°N(northern climate)was incapable of achieving this as well.Regarding the monthly time step,the buildings in the southern and Atlantic climates with 15% efficient BiPVs were able to produce an energy surplus every month,as was the building in theAlps with 20% efficientBIPVs.

Effects of soil metal(loid)s pollution on microbial activities and environmental risks in an abandoned chemical smelting site

Abandoned chemical smelting sites containing toxic substances can seriously threaten and pose a risk to the surrounding ecological environment.Soil samples were collected from different depths(0 to 13 m)and analyzed for metal(loid)s content and fractionation,as well as microbial activities.The potential ecological risk indices for the different soil depths(ordered from high to low)were:1 m(D-1)>surface(S-0)>5 m(D-5)>13 m(D-13)>9 m(D-9),ranging between 1840.65-13,089.62,and representing extremely high environmental risks,of which Cd(and probably not arsenic)contributed to the highest environmental risk.A modified combined pollution risk index(mCR)combining total content and mobile proportion of metal(loid)s,and relative toxicities,was used to evaluate the degree of contamination and potential environmental risks.For the near-surface samples(S-0 and D-1 layers),the mCR considered that As,Cd,Pb,Sb,and Zn achieved high and alarming degrees of contamination,whereas Fe,mn,and Ti were negligible or low to moderate pollution degrees.Combined microcalorimetry and enzymatic activity measurements of contaminated soil samples were used to assess the microbial metabolic activity characteristics.Correlation analysis elucidated the relationship between metal(loid)s exchangeable fraction or content and microbial activity characteristics(pmicrobial metabolic activity in the D-1 layer was low presumably due to heavy metal stress.Enzyme activity indicators and microcalorimetric growth rate(k)measurements were considered sensitive indicators to reflect the soil microbial activities in abandoned chemical smelting sites.

Paddy fields play an important role in global carbon(C) cycling and are an important source of methane(CH_(4)) emissions. Insights into the processes influencing the dynamics of soil organic C(SOC) in paddy fields are essential for maintaining global soil C stocks and mitigating climate change. Periphytic biofilms composed of microalgae, bacteria, and other microorganisms are ubiquitous in paddy fields, where they directly mediate the transfer of elements at the soil-water interface. However, their contributions to C turnover and exchange have been largely neglected. Periphytic biofilms affect and participate in soil C dynamics by altering both abiotic(e.g., pH and redox potential) and biotic conditions(e.g., microbial community composition and metabolism). This review summarizes the contributions of periphytic biofilms to soil C cycling processes, including carbon dioxide fixation, SOC mineralization, and CH_(4) emissions. Future research should be focused on: i) the mechanisms underlying periphytic biofilm-induced C fixation and turnover and ii) quantifying the contributions of periphytic biofilms to soil C uptake, stabilization, and sequestration in paddy fields.

Integrins and their potential roles in mammalian pregnancy

Integrins are a highly complex family of receptors that, when expressed on the surface of cells, can mediate reciprocal cell-to-cell and cell-to-extracellular matrix(ECm) interactions leading to assembly of integrin adhesion complexes(IACs) that initiate many signaling functions both at the membrane and deeper within the cytoplasm to coordinate processes including cell adhesion, migration, proliferation, survival, differentiation, and metabolism. All metazoan organisms possess integrins, and it is generally agreed that integrins were associated with the evolution of multicellularity, being essential for the association of cells with their neighbors and surroundings, during embryonic development and many aspects of cellular and molecular biology. Integrins have important roles in many aspects of embryonic development, normal physiology, and disease processes with a multitude of functions discovered and elucidated for integrins that directly influence many areas of biology and medicine, including mammalian pregnancy, in particular implantation of the blastocyst to the uterine wall, subsequent placentation and conceptus(embryo/fetus and associated placental membranes) development. This review provides a succinct overview of integrin structure, ligand binding, and signaling followed with a concise overview of embryonic development, implantation, and early placentation in pigs, sheep, humans, and mice as an example for rodents. A brief timeline of the initial localization of integrin subunits to the uterine luminal epithelium(LE) and conceptus trophoblast is then presented, followed by sequential summaries of integrin expression and function during gestation in pigs, sheep, humans, and rodents. As appropriate for this journal, summaries of integrin expression and function during gestation in pigs and sheep are in depth, whereas summaries for humans and rodents are brief.
