Long-term survival of a patient with advanced pancreatic cancer under adjunct treatment with Viscum album extracts: A case report

BACKGROUND Advanced pancreatic cancer(aPC) has a poor prognosis with limited survival benefit from current standard treatment. Viscum album extracts(VAE) are used by many cancer patients, showing immune-stimulating effects, improved quality of life, and a survival benefit in patients with *** SUMMARY A 59-year-old architect developed epigastric pain. A cystic lesion of the pancreas of 45-mm diameter was detected. In a follow-up magnetic resonance imaging,about one year later, multiple lesions were seen in the corpus and the tail of the pancreas; CA-19-9 was elevated to 58.5 U/mL. A distal pancreatectomy with splenectomy was performed, and a tumor of 7 cm × 5 cm × 3.5 cm was *** investigation showed an intraductal papillary mucinous neoplasmassociated invasive adenocarcinoma with invasion of the lymph vessels,perineural invasion, and positive nodes(2/27); surgical margins showed tumor cells, and the tumor was classified as pT3 N1 M0 R1. The patient was treated with radiation of the tumor bed and capecitabine/oxaliplatin followed by gemcitabine and FOLFIRINOX. Seven months after surgery, a liver metastasis was detected and treatment with FOLFIRINOX was started. Four months after detection of the metastasis, the patient opted for additional treatment with VAE. Another month later, the metastasis was treated with radiofrequency ablation(RFA). Eight months later, the hepatic lesion recurred and was again treated with RFA. Thecontinuous VAE treatment was increased in dose, and the patient stayed recurrence-free for the next 39 mo in good health and working full-time(as of the time this case report was written).CONCLUSION We present the case of a patient with aPC with R1-resection with development of liver metastasis during the course of treatment who showed an overall survival of 63 mo and a relapse-free survival of 39 mo under increasing VAE therapy. Impact of COVID-19 on Liver Transplant Activity in the USA: Variation by Etiology and Cirrhosis Complications

Genotype × environment interaction effects on early fresh storage root yield and related traits in cassava

Cassava(Manihot esculenta Crantz) is an important root crop worldwide. It exhibits substantial differential genotypic responses to varying environmental conditions, a phenomenon termed genotype × environment interaction(GEI). A significant GEI presents challenges in the selection of superior genotypes. The objective of this study was to examine the effect of genotype,environment and GEI on early fresh storage root yield(FSRY) and related traits in ***, 12 cassava genotypes were evaluated in a randomised complete block design at three contrasting locations(Jinja, Nakasongola and Namulonge) in Uganda. Trials were harvested nine months after planting and the data collected were analysed using the additive main effects and multiplicative interaction(AMMI) model. The AMMI analysis of variance showed significant variation among genotypes for early FSRY and all other traits *** were significantly different for all traits except for cassava brown streak disease root necrosis. The GEI effect was non-significant for early FSRY, but significant for other traits. For early FSRY, 48.5% of the treatment sum of squares was attributable to genotypes, 27.3% to environments, and 24.1% to GEI, indicating a predominance of genotypic variation for this *** of genotypic variation was also observed for all the other traits. A majority of the genotypes(67%) had low interaction effects with locations for early FSRY, with Akena, CT2, CT4 and NASE14 being the most stable genotypes for the trait. Significant negative correlation was observed between cassava mosaic disease severity and early FSRY and storage root number,indicating significant negative effects of cassava mosaic disease on early FSRY and stability in cassava. The information generated will inform future selection initiatives for superior early-yielding cassava genotypes combining resistance to cassava mosaic and brown streak diseases in Uganda.

Primary parasitoids of red scale （Aonidiella aurantii） in Australia and a review of their introductions from Asia

We used morphological and molecular differences to confirm the identities of red scale （Aonidiella aurantii） and yellow scale （A. citrina）, and their primary parasitoids, in Australia. An extension to the distribution of yellow scale was confirmed. Six primary parasitoids of red scale were identified： Aphytis chrysomphali, A. lingnanensis, A. melinus, Comperiella bifasciata, Encarsia citrina, and E. perniciosi. With the exception of A. lingnanensis, these parasitoids, and a species of Aphelinus, were detected in association with red scale during studies in citrus orchards in coastal New South Wales between 2009 and 2012. Two races ofA. melinus were recorded： one from the Indian Subcontinent, the other previously only recorded in China. The studies, and reviews of historical records, led us to conclude that 4 parasitoids, A. lingnanensis, C. bifasciata, and both species of Encarsia, were present in Australia before successful or unsuccessful formal introductions between 1902 and 1970. The A. melinus race previously recorded in China may also have been present before the Indian Subcontinent race was formally introduced in 1961. We suggest the possibility that the natural distribution of some of the parasitoids may include East and Southeast Asia, and parts of Australasia. We found no reports of native armored scales being recorded on species and hybrids of Citrus introduced to Australia, and no reports of introduced armored scales being recorded on native Rutaceae, including 6 species of Citrus. However, we subsequently recorded yellow scale on Geijera parviflora, a native rutaceous tree.

Distal small bowel motility and lipid absorption in patients following abdominal aortic aneurysm repair surgery

AIM： To investigate distal small bowel motility and lipid absorption in patients following elective abdominal aortic aneurysm （AAA） repair surgery, METHODS： Nine patients （aged 35-78 years; body mass index （BMI） range： 23-36 kg/m2） post-surgery for AAA repair, and seven healthy control subjects （20-50 years; BMI range： 21-29 kg/m^2） were studied, Continuous distal small bowel manometry was performed for up to 72 h, during periods of fasting and enteral feeding （Nutrison）. Recordings were analyzed for the frequency, odgin, length of migration, and direction of small intestinal burst activity. Lipid absorption was assessed on the first day and the third day post surgery in a subset of patients using the ^13C-triolein-breath test, and compared with healthy controls. Subjects received a 20-min intraduodenal infusion of 50 mL liquid feed mixed with 200μL ^13C-triolein. End-expiratory breath samples were collected for 6 h and analyzed for ^13CO2 concentration. RESULTS： The frequency of burst activity in the proximal and distal small intestine was higher in patients than in healthy subjects, under both fasting and fed conditions （P〈 0.005）. In patients there was a higher proportion of abnormally propagated bursts （71% abnormal）, which began to normalize by d 3 （25% abnormal） *** absorption data was available for seven patients on d 1 and four patients on d 3 post surgery. In patients, absorption on d 1 post-surgery was half that of healthy control subjects （AUC ^13CO2 1323±244 vs 2646±365; P〈0.05, respectively）, and was reduced to the one-fitch that of healthy controls by d 3 （AUC ^13CO2 470±832 vs 2646±365; P〈 0.05, respectively）. CONCLUSION： Both proximal and distal small intestinal motor activity are transiently disrupted in critically ill patients immediately after major surgery, with abnormal motility patterns extending as far as the ileum. Specialized protein products in broiler chicken nutrition:A review

In poultry nutrition, most attention is given to protein products, due to the importance of protein as a major constituent of the biologically active compounds in the body. It also assists in the synthesis of body tissue, for that renovation and growth of the body. Furthermore, protein exists in form of enzymes and hormones which play important roles in the physiology of any living organism. Broilers have high dietary protein requirements, so identification of the optimum protein concentration in broiler diets, for either maximizing broiler performance or profit, requires more knowledge about birds' requirements for protein and amino acids and their effects on the birds' growth performance and development. It also requires knowledge about the protein sources available that can be used in poultry diets. The broad aim of this review is to highlight the importance of some of the available high-quality specialized protein products of both animal and plant origins which can be explored for feeding broiler chickens. Minimization of the concentration of anti-nutritional factors(ANFs) and supplementation with immunologically active compounds are the main focus of gut health-promoting broiler diets. These diet characteristics are influenced by feed ingredient composition and feed processing. The general hypothesis is that these protein products are highly digestible and devoid of or contain less ANFs. Feeding these products to broiler chicks, especially at an earlier age, can assist early gut development and digestive physiology, and improve broiler growth performance and immunity.

Multidisciplinary management of Mirizzi syndrome with cholecystobiliary fistula: the value of minimally invasive endoscopic surgery

Mirizzi syndrome, a rare complication of gallstones, is defined by obstruction of the main bile duct. This obstruction may worsen and thus result in cholecystobiliary fistula. Surgical management of Mirizzi syndrome is complicated by the presence of inflamed tissue around the hepatic pedicle, making it impossible to distinguish between the main bile duct and the gallbladder. The surgeon＇s first task is to perform subtotal cholecystotomy（from the fundus of the gallbladder to the neck） without trying to locate the cystic duct. In a second step, the gallstones are extracted and the main bile duct is then repaired. In most cases, a T-tube is used to drain the main bile duct, and abdominal drainage is left in place（in case a bile fistula forms）. This study concluded that preoperative drainage of the main bile duct in the treatment of Mirizzi syndrome types II and III is feasible and might help to decrease the postoperative complication rate.

Genetic variation amongst biotypes of Dactylopius tomentosus

The tomentose cochineal scale insect, Dactylopius tomentosus （Lamarck）（Hemiptera： Dactylopiidae）, is an important biological control agent against invasive species of Cylindropuntia （Caryophyllales： Cactaceae）. Recent studies have demonstrated that this scale is composed of host-affiliated biotypes with differential host specificity and fitness on particular host species. We investigated genetic variation and phylogenetic rela-tionships among D. tomentosus biotypes and provenances to examine the possibility that genetic diversity may be related to their host-use pattern, and whether their phylogenetic relationships would give insights into taxonomic relatedness of their host plants. Nucleotide sequence comparison was accomplished using sequences of the mitochondrial cytochrome c oxidase I （COI） gene. Sequences of individuals from the same host plant within a region were identical and characterized by a unique haplotype. Individuals belonging to the same biotype but from different regions had similar haplotypes. However, haplotypes were not shared between different biotypes. Phylogenetic analysis grouped the monophyletic D. tomentosus into 3 well-resolved clades of biotypes. The phylogenetic relationships and clustering of biotypes corresponded with known taxonomic relatedness of their hosts. Two biotypes, Fulgida and Mamillata, tested positive for Wolbachia （α-Proteobacteria）, a common endosymbiont of insects. The Wolbachia sequences were serendipitously detected by using insect-specific COI DNA barcoding primers and are most similar to Wolbachia Supergroup F strains. This study is the first molecular characterization of cochineal biotypes that, together with Wolbachia sequences, contribute to the better identification of the biotypes of cochineal insects and to the biological control of cacti using host-specific biotypes of the scale.

Granular titanite from the Roter Kamm crater in Namibia:Product of regional metamorphism,not meteorite impact

Accessory minerals with so-called granular texture have risen in importance as geochronological tools for U-Pb dating of meteorite impact events. Grain-scale recrystallization, typically triggered by a combination of high-strain deformation and post-impact heating, can create a polycrystalline microstructure consisting of neoblasts that expel radiogenic Pb, which are thus ideal for isotopic dating. While granular domains in zircon and monazite from shocked rocks have been demonstrated to preserve impact ages,few U-Pb dating studies have been conducted on granular microstructures in titanite(CaTiSiO;). Here we report the occurrence of granular-textured titanite from ~2020 Ma granite basement rock exposed in the rim of the 4–5 Ma Roter Kamm impact structure in Namibia. Orientation mapping reveals two microstructurally distinct titanite populations: one consisting of strained/deformed grains, and the other consisting of grains that comprise aggregates of strain-free neoblasts. In situ U-Pb geochronology on 37 grains shows that most grains from both titanite populations yield indistinguishable U-Pb dates of ca.1025 Ma, consistent with the observed microstructures forming during the Mesoproterozoic Namaqua Orogeny. Only four grains preserved older age domains, recording ca. 1875 Ma Paleoproterozoic metamorphism. Two significant observations emerge:(1) none of the analyzed titanite grains yield the 2020 Ma igneous crystallization age previously established from zircon in the same sample, and(2) no age-resetting was detected that could be attributed to the 4 to 5 Ma Roter Kamm impact *** the similarity of the neoblastic microstructure to minerals from other sites with an established impact provenance, the granular texture and near-complete Pb-loss in titanite from Roter Kamm granite instead records a Paleo-to Mesoproterozoic polymetamorphic history, rather than Miocene age shockrelated processes.
