AIM: To assess the impact of transjugular intrahepatic portosystemic shunt (TIPS) on pulmonary gas exchange and to evaluate the use of TIPS for the treatment of hepatopulmonary syndrome (HPS).METHODS: Seven patients, three of them with advanced HPS, in whom detailed pulmonary function tests were performed before and after TIPS placementat the University of Alabama Hospital and at the Hospital Clinic, Barcelona, were ***: TIPS patency was confirmed by hemodynamic evaluation. No changes in arterial blood gases were observed in the overall subset of patients. Transient arterial oxygenation improvement was observed in only one HPS patient, early after TIPS, but this was not sustained 4 mo ***: TIPS neither improved nor worsened pulmonary gas exchange in patients with portal hypertension. This data does not support the use of TIPS as a specific treatment for HPS. However, it does reinforce the view that TIPS can be safely performed for the treatment of other complications of portal hypertension in patients with HPS.

Hepatitis E virus infection is usually a self-limited ***,during the last years there has been growing evidence for prolonged and chronic infection occurring in patients with *** patients with malig-nant and rheumatic diseases have been identified to be at risk for chronic hepatitis ***,their course and prognosis are not well characterized and there have been no reports of hepatitis E virus infection in patients with gynecological ***,we report three Caucasian females with breast and ovarian cancers presenting with elevation of amino-transferase levels during anticancer *** only few or no clinical hints suggested hepatitis E virus infection,the diagnosis of hepatitis E virus infection was confirmed by seroconversion,which might occur with some delay,and/or by polymerase chain *** two patients had a self-limited course,the third patient with a high-risk oncological constellation required ribavirin in order to resume *** cases highlight the need for hepatitis E virus testing in patients with gynecological cancer and elevated aminotransferase ***,these cases show that in selected high-risk patients,ribavirin treatment may be nec-essary based on the decision of a multidisciplinary team.

The objective of this study was to immunohistochemically elucidate the major extracellular matrix constituents of rabbit tracheal cartilage. The impetus for this project is the need for crucial design and validation criteria for tissue engineering juxtaposed with the conspicuous lack of trachea extracellular matrix data in the literature. Tracheal tissue specimens were harvested from New Zealand White rabbits, and were immunostained for collagen I, collagen II, aggrecan and decorin;and a Verhoeff-Van Gieson stain was performed to visualize elastin. The most striking result was the highly organized relationship between distinct fibrous (containing collagen I, decorin and elastin) and hyaline-like (containing collagen II and aggrecan) regions of the tracheal wall. The tracheal cartilage stained strongly with collagen II throughout, with periodic bands of aggrecan in the tracheal arches, meaning that there were areas void of aggrecan immunostaining alternating with areas with strong aggrecan immunostaining. In contrast, the periphery of the cartilage and the perichondrium itself exhibited strong collagen I staining and no collagen II staining. Elastin fibers and decorin were also detected along the periphery of the cartilage in the perichondrium and corresponded highly with the distribution of collagen I staining. The body of the rabbit trachea is therefore composed of a hyaline-cartilage structure primarily made of collagen II and bands of aggrecan, surrounded by a fibrous region composed of elastin and collagen I, indicative of a flexible tissue with distinct regions of compressive integrity. This information will be a valuable reference to future tissue engineering efforts in the creation of a biosynthetic substitute for laryngotracheal reconstruction.

Background:The American Cancer Society estimates that about 25%of all US cancer deaths will be due to lung cancer-more than from cancers of the colon,breast,and prostate ***:We ascertained county-level age-adjusted and age-specific death rates and 95%confidence intervals from the Centers for Disease Control and Prevention Compressed Mortality *** regression analyses were used to estimate the strength and direction of relationships between county poverty,smoking,fine particulate matter(PM2.5)air pollution,and US Census divisions and race-and sex-specific lung cancer ***:Poverty,smoking,and particulate matter air pollution were positively and signifi-cantly related to lung cancer deaths among white men,but of these,only poverty and smoking were significantly associated with lung cancer deaths among white *** in the South Atlantic,East South Central,and West South Central US Census divisions at the time of death was significantly associated with lung cancer deaths for both white men and white *** with white men,poverty and smoking were associated with lung cancer deaths among black men,but of these,only adult smoking had a statistically significant association among black ***:The results support the need for further research,particularly in high-risk areas,to better differentiate factors specific to race and sex and to understand the impact of local risk factors.

Between the illicit use of opioids and attendant overdoses, and accidental overdoses with prescribed drugs, overuse of opioids has become a serious problem. At the same time, finding that fine balance between minimizing the risk of opioid misuse and abuse while at the same time providing access to treatment for patients who need pain control presents an ongoing challenge. Efforts to discover and develop better agents have led to what we term “new-look” opioids. We summarize here one such approach—known as biased ligands. By targeting a subset of GPCR signal transduction, this approach attempts to increase the separation between therapeutic and adverse effects.

background and purpose Radial artery catheterisation is an alternate route of access that has recently started to gain more widespread use for neuroendovascular procedures,including acute stroke *** this small case series,we present our institution’s outcomes in patients undergoing acute stroke interventions via transradial *** and methods We present a retrospective study of 15 patients who underwent acute stroke intervention via radial artery *** analyse these patients’periprocedural and clinical outcomes after undergoing mechanical *** A total of 15 consecutive patients were included in the study(9 males and 6 females),and all patients were able to successfully undergo mechanical thrombectomy via radial artery *** mean time of arterial puncture to reperfusion was 50±28 min(range:15-104).A TICI 2b/3 revascularisation was achieved in 13/15 patients(87%);a TICI 1 and TICI 2a outcome was achieved on the other two *** patient incurred an iatrogenic vessel dissection during the *** of out 15 patients(53%)had favourable mRS(0-3)at the time of discharge from the *** Radial artery catheterisation is technically feasible for performing acute stroke interventions with favourable time to revascularisation and good overall clinical outcomes.
