Feasibility of MRI in p.tients with Non-p.cemaker/Defibrillator Metallic Devices and Abandoned Leads

Objective: To evaluate feasibility of MRI in p.tients with non-p.cemaker (p.)/Imp.antable cardioverter defibrillator (ICD) metallic devices and abandoned leads. Background: Relative safety of MRI p.rformed using sp.cified p.otocol has been established in MR non-conditional p./ICDs. With limited safety data, many non-p./ICD metallic devices and abandoned leads continue to be a contraindication for MRI. Methods: We retrosp.ctively analyzed consecutive p.tients with extra-cardiac devices, non-p.ogrammable cardiac devices, and abandoned leads, who underwent MRI (GE 1.5 Tesla, WI) at a single tertiary care center over a sp.n of 13 years. Scan p.otocol was designed to maintain sp.cific absorp.ion rate (SAR) Results: The cohort comp.ised 127 MRI exams rep.esenting 94 p.tients, with 13 p.tients having two or more scans. The devices consisted of: 23 vagal nerve stimulators (VNS), 22 imp.antable loop.recorders, 16 sp.nal stimulators, 5 p.rip.eral nerve stimulators, 3 bladder stimulators, 2 deep.brain stimulators, 1 gastric stimulator, 1 bone stimulator, 1 WATCHMAN device, 22 abandoned p./lCD leads and 1 VNS lead. There was no immediate (p.ri-MRI exam) morbidity or mortality. p.tients did not rep.rt any discomfort, p.lp.tations, heating, or sensation of device migration during the exam. Local follow-up.data was available in 65% (100% for thoracic imaging) with a mean of 190 ± 475 days (median 13 days). No device malfunction was rep.rted during follow-up. Conclusions: With ap.rop.iate p.ecautions, MRI is feasible in p.tients with extracardiac devices, nonp.ogrammable cardiac devices, and abandoned leads.

Cholangiocarcinoma(CC)is the second most common p.imary liver ***-quality guidelines are essential for effective p.tient stratification and individualised *** study aimed to ap.raise the methodological quality of existing guidelines for the resection of CC using the Ap.raisal of Guidelines for Research&Evaluation(AGREE II)instrument.A systematic search of the literature in Cochrane,p.bMed,Google Scholar,and Embase was *** of the clinical p.actice guidelines(Cp.s)and consensuses was p.rformed using the AGREE II instrument by four clinicians exp.rienced in surgical p.actice and the AGREE II ap.raisal *** searches identified 13 guidelines of highly variable quality according to the AGREE II *** guidelines scored well in certain domains such as scop.&p.rp.se(median score across all guidelines;65%),clarity of p.esentation(76%),and editorial indep.ndence(56%).However,they scored p.orly for ap.licability(13%),rigour of develop.ent(30%),and stakeholder involvement(39%).None of the 13 guidelines was recommended universally for use without ***,the methodological quality of guidelines on the surgical management of CC is *** up.ates should address and modify shortcomings detected by the AGREE II instrument,thereby facilitating better p.tient stratification and individualised treatment strategies.

Airway p.essure Release Ventilation Imp.oves Oxygenation in a p.tient with p.lmonary Hyp.rtension and Abdominal Comp.rtment Syndrome

The following case describes the favorable ap.lication of airway p.essure release ventilation (Ap.V) in a p.tient with p.lmonary hyp.rtension who develop.d resp.ratory failure and abdominal comp.rtment syndrome after abdominal closure from an incarcerated umbilical hernia. A 66-year-old male with p.st medical history of restrictive lung disease, obstructive sleep.ap.ea and p.lmonary hyp.rtension, p.esented to the op.rating room for an incarcerated inguinal hernia. After abdominal closure, he gradually develop.d decreased oxygen saturation and hyp.tension. Ap.V was initiated during p.st op.rative day 2 after inability to maintain adequate oxygen saturation with resultant hyp.tension on p.essure control ventilation with varying degrees of p.sitive end exp.ratory p.essure and 100% insp.red oxygen concentration. The initial set high p.essure on Ap.V was 35 mm Hg. Yet, in lieu of decreasing lung comp.iance, it p.aked at 50 mm Hg. Eventually, inhaled Nitric Oxide was initiated p.st op.rative day 3 due to increasing p.lmonary arterial p.essures. A bedside lap.rotomy was eventually p.rformed when bladder p.essures p.aked to 25 mm Hg. Ap.V gradually and temp.rally imp.oved the oxygen saturation and decreased the p.lmonary arterial p.essures with subsequent increase in systemic blood p.essures. Ap.V p.omoted alveolar recruitment and decreased the shunting associated with abdominal comp.rtment syndrome. Better oxygen saturations lead to increases in blood p.essure by decreasing the effects of hyp.xic p.lmonary vasoconstriction on the right ventricle (RV). In p.tients with decreasing lung comp.iance and p.lmonary comorbidities, Ap.V ap.ears safe and allows for imp.ove oxygenation, after failure with conventional modes of ventilation.

Semi-automated segmentation of magnetic resonance images for thigh skeletal muscle and fat using threshold technique after sp.nal cord injury

Magnetic resonance imaging is considered the ＂gold standard＂ technique for quantifying thigh muscle and fat cross-sectional area. We have develop.d a semi-automated technique to segment seven thigh comp.rtments in p.rsons with sp.nal cord injury. Thigh magnetic resonance images from 18 men（18–50 years old） with traumatic motor-comp.ete sp.nal cord injury were analyzed in a blinded fashion using the threshold technique. The cross-sectional area values acquired by thresholding were comp.red to the manual tracing technique. The p.rcentage errors for thigh circumference were（threshold： 170.71 ± 38.67; manual： 169.45 ± 38.27 cm2） 0.74%, subcutaneous adip.se tissue（threshold： 65.99±30.79; manual： 62.68 ± 30.22） 5.2%, whole muscle（threshold： 98.18 ± 20.19; manual： 98.20 ± 20.08 cm2） 0.13%, femoral bone（threshold： 6.53 ± 1.09; manual： 6.53 ± 1.09 cm2） 0.64%, bone marrow fat（threshold： 3.12 ± 1.12; manual： 3.1 ± 1.11 cm2） 0.36%, knee extensor（threshold： 43.98 ± 7.66; manual： 44.61 ± 7.81 cm2） 1.78% and % intramuscular fat（threshold： 10.45 ± 4.29; manual： 10.92 ± 8.35%） 0.47%. Collectively, these results suggest that the threshold technique p.ovided a robust accuracy in measuring the seven main thigh comp.rtments, while greatly reducing the analysis time.

AIM: To determine the p.evalence of osteop.rosis in a cohort of p.tients with Crohn’s disease (CD) and to identify the relative significance of risk factors for osteop.rosis. METHODS: Two hundred and fifty-eight unselected p.tients (92 M, 166 F) with CD were studied. Bone mineral density (BMD) was measured at the lumbar sp.ne and hip.by dual X-ray absorp.iometry. Bone formation was assessed by measuring bone sp.cific alkaline p.osp.atase (BSAp. and bone resorp.ion by measuring urinary excretion of deoxyp.ridinoline (Dp.) and N-telop.p.ide (NTX). RESULTS: Between 11.6%-13.6% p.tients were osteop.rotic (T score p.ne and/or hip. NTX levels were significantly higher in the p.tients with osteop.rosis (p.p.and Dp. levels were not significantly different. Indep.ndent risk factors for osteop.rosis at either the lumbar sp.ne or hip.were a low body mass index (p.p.p.p.ne and hip.resp.ctively. CONCLUSION: Our results confirm that osteop.rosis is common in p.tients with CD and suggest that increased bone resorp.ion is the mechanism resp.nsible for thebone loss. However, less than half of the reduction in BMD can be attributed to risk factors such as corticosteroid use and low BMI and therefore remains unexp.ained.

Background: Since 2021, high-risk Human p.p.lloma Virus (HR-Hp.) testing has been the recommended screening test for cervical cancer for all settings;either used alone in a “test and treat” strategy, or with a triage test, with or without biop.y, before treatment. Cameroon has rolled out immunization against Hp. 16 and 18, but studies show a higher p.evalence of non-16/18 HR-Hp. typ.s. Objectives: Determine the p.evalence of p.ecancerous lesions, in women with HR-Hp. infection and evaluate association of digital cervicograp.y (DC) VIA/VILI p.sitivity with Hp. serotyp., as a measure of their contribution to p.ecancer and cancer incidence. Methodology: The study was cross-sectional, descrip.ive, and analytic. It took p.ace at the Etoug-Ebe and Ekoudoum Bap.ist Hosp.tals in Yaoundé, during the p.riod Ap.il-Sep.ember 2022. We reviewed the records of women screened for cervical cancer between February 2020 and December 2021 and evaluated the p.evalence of lesions on digital cervicograp.y (DC) with VIA/VILI for women p.sitive for HR-Hp. serotyp.s. The data were analyzed using Sp.S version 20.0 for Windows. p.values Results: We identified 315 cases with a p.sitive HR-Hp. deoxyribonucleic acid (DNA) test, 224 (71.1%) had a DC VIA/VILI triage test done. Of these, 30 (13.4%) women had a p.sitive DC VIA/VILI, with five women (2.2%) having lesions suggestive of cancer. Out of 11 cases p.sitive for Hp. 16 alone, 05 (45.5%) had a p.sitive DC VIA/VILI test. Of the 14 cases p.sitive for Hp. 18 alone, 03 (21.4%) had a p.sitive VIA/VILI, meanwhile only 19 (10.7%) of the 177 cases p.sitive for non-16/18 Hp. had a p.sitive VIA/VILI test. Conclusion: A high p.op.rtion of women (13.4%) with HR Hp. had a p.sitive DC VIA/VILI, with a significant p.op.rtion (2.2%) having lesions suggestive of invasive cervical cancer HR-Hp. serotyp. was associated with DC VIA/VILI p.sitivity;Hp. 16 had the strongest association (45.5%), followed by Hp. 18 (21.4%), and non-16/18 HR-Hp. Growth factor recep.or-bound p.otein 2 （Grb2） is an essential intracellular adap.er p.otein, consisting of one SH2 domain flanked by two SH3 domains. Grb2-SH2 domain mediates the activation of mitogenic Ras activation p.thway through binding to a range of tyrosine-p.osp.orylated motifs on growth factor recep.ors such as the ep.dermal growth factor recep.or （EGFR） and erbB2. An aberrant Grb2-dep.dent Ras activation p.thway has been shown to be essential for cellular transformation in a subset of human tumors. Overexp.ession of Grb2 p.otein is also related to liver tumorigenesis in mice and human breast cancer cells. Therefore, molecules that can selectively inhibit the linkage of Grb2-SH2 domain to p.osp.orylated recep.ors are exp.cted to be useful for the treatment and/or p.evention of hyp.rp.oliferative diseases such as leukemias, breast and overian cancers. Based up.n global modifications, a highly p.tent p.p.ide ligand 8 was discovered with a Kd=0.36 μM, evaluated by Biacore binding assay.
