背景手术后恶心、呕吐（pONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者pONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊... 详细信息

背景手术后恶心、呕吐（pONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者pONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的pONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生pONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（p〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，p〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（p〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，p〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少pONV，主要是通过减少不良反应而获得。

Background The present study aimed to investigate the detailed mode and specific sites for their binding as well as the functional relevance of this binding in the phenotypic proliferation of vascular smooth muscle cells(SMCs). Methods CREG knocked-down SMCs were employed to evaluate the biological activity of wtCREG and *** of SMC differentiation markers SM myosin heavy chain(SM-MHC),SM-actin,heavy caldesmon and myocardin were determined by Western blotting using specific antibodies. Cellular growth of SMCs was assessed by bromide dewuridine (BrdU) incorporation and cell cycle analysis on fluorescence-activated cell sorting(FACS).A solid-phase binding assay was used to study the binding of CREG to extracellular domains of M6p/*** cellular co-localization of the two recombinant CREGs with M6p/IGF2R was detected on SMC surface by immunoprecipitation and immunofluorescence *** The molecular weight of wtCREG was around 30 kD while that of the mCREG was～25 *** of wtCREG with pNGase F reduced its molecular weight from～30 kD to～25 kD,whereas pNGase F treatment had no effect on the molecular weight of *** wtCREG and mCREG proteins enhanced SMC differentiation,inhibited BrdU incorporation,and arrested cell cycle progression when added to the culture *** CREG knocked-down SMCs,the amount of CREG detected by immunoblotting in M6p/IGF2R immunoprecipitates was significantly reduced when compared to normal *** recombinant CREGs co-immunoprecipitated with M6p/IGF2R, although slightly reduced amount of the mutant CREG was detected in M6p/IGF2R *** revealed that His-tagged CREGs co-localized with IGF2R on the cell surface in a glycosylation-independent *** vitro binding assay showed that CREGs bound to M6p/ IGF2R extracellular domains 7-10 and 11-13 in a glycosylation -dependent and -independent manner,*** blocking experiments using soluble M6p/IGF2R fragments and M6p/

背景与目的尽管有一些广泛认可的相关因素，但导致包膜挛缩的多层面的因素在近几十年内仍未解决。本研究将调查首次隆乳术患者导致包膜挛缩的一些潜在危险因素。方法本研究资料包含2560例首次隆乳术患者，假体5109只，由34位医师在5年内施行的SIENTRA临床研究结果。患者每年随访一次，记录包膜挛缩的Baker分级。潜在危险因素包括患者属性、假体属性、手术特征、腔隙冲洗和术后特征，对其进行频率和多变量模型分析。结果在5年中，共有179例患者的265只乳房假体发生包膜挛缩。由假体产生的包膜挛缩的总体Kaplan—Meier概率为7．6％。未调整的分析显示，以下因素会增加包膜挛缩的发生概率：光面假体、环乳晕切口、乳腺下层面置入、抗生素及类固醇腔隙冲洗、被建议按摩及外科胸罩（p值均〈0．05）。将所有危险因素调整为模型中的变量，其多变量分析的结果发现6种因素与包膜挛缩有独立关联（假体置入层次、假体表面、切口位置、血肿或血清肿发展、假体尺寸及外科胸罩；p值均〈0．05）。结论本研究分析证实，排除其他因素的情况下，肌肉下层次置人和选择毛面假体可显著减少包膜挛缩的发生。
