Long term outcomes of cardiac transplant for immunoglobulin light chain amyloidosis:The Mayo Clinic experience

AIM: To determine the outcome of orthotopic heart transplantation(OHT) in immunoglobulin light chain(AL) ***: The medical records of patients with AL who underwent orthotopic heart transplantation at the Mayo Clinic in Rochester Minnesota from 1992 to 2011 were reviewed. Patients met at least one of the following at: New York Heart Association class Ⅳ heart failure, ventricular thickness > 15 mm, ejection fraction < 40%. Selection guidelines for heart transplant included age < 60 years, absence of multiple myeloma and significant extra-cardiac organ involvement. Baseline characteristics including age, gender, organinvolvement, and New York Heart Association functional class were recorded. Laboratory data, waiting time until heart transplant, and type of treatment of the underlying plasma cell disorder were recorded. Survival from the time of OHT was calculated using KaplanMeier survival curves. Survival of patients undergoing OHT for AL was compared to that of non-amyloid patients undergoing OHT during the same time ***: Twenty-three patients(median age 53 years) with AL received OHT. There were no deaths in the immediate perioperative period. Twenty patients have died post OHT. For the entire cohort, the median overall survival was 3.5 years(95%CI: 1.2, 8.2 years). The 1-year survival post OHT was 77%, the 2-year survival 65%, and the 5-year survival 43%. The 5-year survival for non-amyloid patients undergoing OHT during the same era was 85%. Progressive amyloidosis contributed to death in twelve patients. Of those without evidence of progressive amyloidosis, the cause of death included complications of autologous hematopoietic stem cell transplantation for 3 patients, post-transplant lymphoproliferative disorder for 2 patients; and for the remaining one death was related to each of the following causes: acute rejection; cardiac vasculopathy; metastatic melanoma; myelodysplastic syndrome; and unknown. Evaluation of the 20 L dust explosibility testing chamber and comparison to a modified 38 L vessel for underground coal

The phenomenon of combustible dust explosions is present within many industries. Tests for explosibility of dust clouds per ASTM E1226 use a 20 L explosive chamber that places the combustible dust directly below the dispersion nozzle which generates a thorough mixture for testing purposes. However, in the underground coal mining industry, there are a number of geologic, mining, and regulatory factors that change the deposition scheme of combustible coal dust. This causes the atmosphere of a coal mine to have a variable rock dust-coal dust mixture at the time of ignition. To investigate the impact of this variable atmosphere, a series of lean explosibility tests were conducted on a sample of Pittsburgh Pulverized coal dust. These explosibility tests were conducted in a 38 L chamber with a 5 kJ Sobbe igniter. The 38 L chamber generates a variable air-dust mixture prior to ignition. The test results indicate that the 38 L chamber experiences reduced explosive pressures, and lower explosibility index values when compared to the 20 L chamber.

Solutions to Forced and Unforced Lin–Reissner–Tsien Equations for Transonic Gas Flows on Various Length Scales

The Lin–Reissner–Tsien equation is useful for studying transonic gas flows, and has appeared in both forced and unforced forms in the literature. Defining arbitrary spatial scalings, we are able to obtain a family of exact similarity solutions depending on one free parameter in addition to the model parameter holding the scalings. Numerical solutions compare favorably with the exact solutions in regions where the exact solutions are valid. Mixed wave-similarity solutions, which describe wave propagation in one variable and self-similar scaling of the entire solution, are also given,and we show that such solutions can only exist when the wave propagation is sufficiently slow. We also extend the Lin–Reissner–Tsien equation to have a forcing term, as such equations have entered the physics literature recently. We obtain both wave and self-similar solutions for the forced equations, and we are able to give conditions under which the force function allows for exact solutions. We then demonstrate how to obtain these exact solutions in both the traveling wave and self-similar cases. There results constitute new and potentially physically interesting exact solutions of the Lin–Reissner–Tsien equation and in particular suggest that the forced Lin–Reissner–Tsien equation warrants further study.

Role of ion channels in gastrointestinal cancer

Mechanistic study of substrate-based galvanic replacement reactions

The sacrificial templates used in galvanic replacement reactions dictate the properties of the hollow metal nanostructures formed. Here, we demonstrate that substrate-based Au-Ag nanoshells with radically altered properties are obtained by merely coating silver templates with an ultrathin layer of gold prior to their insertion into the reaction vessel. The so-formed nanoshells exhibit much smoother surfaces, a higher degree of crystallinity and are far more robust. Dealloying the nanoshells results in the first demonstration of substrate-based nanocages. Such cages exhibit a well-defined pattern of geometric openings in directions corresponding to the {111}-facets of the starting template material. The ability to engineer the cage geometry through adjustments to the orientational relationship between the crystal structure of the starting template and that of underlying substrate is demonstrated. Together these discoveries provide the framework to advance our understanding of the mechanisms governing substrate- based galvanic replacement reactions.

Galvanic replacement reactions have been widely used to transform solution dispersed silver template structures into intricate nanoshell geometries. Here, we report on the use of these same reactions to form hollow substrate-supported Au-Ag nanoshells from silver templates having a heteroepitaxial relationship with the underlying single crystal substrate. The structures obtained exhibit a nanohut geometry, show highly tunable plasmonic properties and are formed as periodic arrays using a lithography-free technique. When removed from the substrate the inverted nanohuts appear as nanobowls with a notch in the rim. The study lays the groundwork for wafer-based devices utilizing nanoshells located at site-specific locations.

Deficits in the impulse control system are an important predictor of energy intake and body *** classified as overweight to obese may possess these deficits as a general behavioral trait or they may be food*** present study assessed motor impulsivity(ability to suppress a pre-potent response)when presented with food and neutral(non-food)cues,testing if deficits in motor impulsivity is specific to food cues or a general trait among participants classified as overweight to *** proportion of inhibitory failures to no-go targets following food cues(10.8%)was significantly greater than the proportion of inhibitory failures to no-go targets following neutral cues(1.9%,p<0.001).These differences remained when covering for sex and *** indicates deficits in food-specific impulse control(as opposed to general impulse control)are present in those classified as overweight to *** the specific aspect of impulse control that is present in this population is needed for the development of future impulse control training interventions that seek to change eating behaviors as a means for weight control.

Estimate of the energy value of soybean meal relative to corn based on growth performance of nursery pigs

Terrorist Networks, Network Energy and Node Removal: A New Measure of Centrality Based on Laplacian Energy

In this work we propose a centrality measure for networks, which we refer to as Laplacian centrality, that provides a general framework for the centrality of a vertex based on the idea that the importance (or centrality) of a vertex is related to the ability of the network to respond to the deactivation or removal of that vertex from the network. In particular, the Laplacian centrality of a vertex is defined as the relative drop of Laplacian energy caused by the deactivation of this vertex. The Laplacian energy of network G with?n?vertices is defined as , where ?is the eigenvalue of the Laplacian matrix of G. Other dynamics based measures such as that of Masuda and Kori and PageRank compute the importance of a node by analyzing the way paths pass through a node while our measure captures this information as well as the way these paths are “redistributed” when the node is deleted. The validity and robustness of this new measure are illustrated on two different terrorist social network data sets and 84 networks in James Moody’s Add Health in school friendship nomination data, and is compared with other standard centrality measures.
