Senescent endoth.lial cells (ecs) could impair th. integrity of th. blood vessel endoth.lium, leading to vascular aging and a series of diseases, such.as ath.rosclerosis, diabetes. Preventing or mitigating ec senescence migh. serve as a promising th.rapeutic paradigm for th.se diseases. Recent studies sh.wed th.t small extracellular vesicles (sEV) h.ve th. potential to transfer bioactive molecules into recipient cells and induce ph.notypic ch.nges. Since mesench.mal stem cells (MSCs) h.ve long been postulated as an important source cell in regenerative medicine, h.rein we investigated th. role and mech.nism of MSC-derived sEV (MSC-sEV) on ec senescence. In vitro results sh.wed th.t MSC-sEV reduced senescent biomarkers, decreased senescence-associated secretory ph.notype (SASP), rescued angiogenesis, migration and oth.r dysfunctions in senescent ec induced by oxidative stress. In th. In vivo natural aging and type-2 diabetes mouse wound-h.aling models (both.of wh.ch.h.ve senescent ecs), MSC-sEV promoted wound closure and new blood vessel formation. Mech.nically, miRNA microarray sh.wed th.t miR-146a was h.gh.y expressed in MSC-sEV and also upregulated in ec after MSC-sEV treatment. miR-146a inh.bitors abolish.d th. stimulatory effects of MSC-sEV on senescence. Moreover, we found miR-146a could suppress Src ph.sph.rylation and downstream targets VE-cadh.rin and Caveolin-1. Collectively, our data indicate th.t MSC-sEV mitigated endoth.lial cell senescence and stimulate angiogenesis th.ough.miR-146a/Src.

Clinical and anatomic predictors of need for repeat atrial fibrillation ablation

AIM To identify predictors of need for repeat procedures after initial atrial fibrillation(AF) ablation. METh.DS We identified a coh.rt undergoing first time AF ablation at our institution from January 2004 to February 2014 wh. h.d cardiac magnetic resonance(CMR) imaging performed prior to ablation. Clinical variables and anatomic ch.racteristics(determined from CMR) were assessed as predictors of need for repeat ablation. Th. decision regarding need for and timing of repeat ablation was at th. discretion of th. treating ph.sician. RESULTS From a coh.rt of 331 patients, 142 patients(43%) underwent repeat ablation at a mean of 13.6 ± 18.4 mo afterth. index procedure. Both.male gender(81% vs 71%, P = 0.05) and lower ejection fraction(57.4% ± 10.3% vs 59.8% ± 9.4%, P = 0.04) were associated with.need for repeat ablation. On pre-ablation CMR, mean pulmonary vein(PV) diameters were significantly larger in all four PVs among patients requiring repeat procedures. In multivariate analysis, increased righ. superior PV diameter significantly predicted need for repeat ablation(odds ratio 1.08 per millimeter increase in diameter, 95%CI: 1.00-1.16, P = 0.05). Th.re were also trends toward significance for increased left and righ. inferior PV sizes among th.se requiring repeat *** Increased PV size predicts th. need for repeat AF ablation, with.each.millimeter increase in PV diameter associated with.an approximately 5%-10% increased risk of requiring repeat procedures.

Using a newly reported Pacific sea surface temperature data set, we extend a prior study th.t assigned el Niño episodes to distinct sequences. With.n th.se sequences th. episodes are ph.se-locked to subh.rmonics of th. annual solar irradiance cycle h.ving two- or th.ee-year periodicity. Th.re are 40 el Niño episodes occurring since 1872, each.found with.n one of eigh.een such.sequences. Our list includes all previously reported events. Th.ee el Niño episodes h.ve already been observed in boreal winters of 2009, 2012 and 2015, illustrating a sequence of 3-year intervals th.t began in 2008. If th. climate system remains in th.s state, th. next el Niño is likely to occur in boreal winter of 2018.

Aims of th. study. -First, to describe liposomal amph.tericin B (AmBisome) use as first line treatment of pediatric visceral leish.aniasis and secondly, to evaluate th. incidence of th. disease in south.rn France (Provence -Alpes -Cte d’Azur -Corse). Material and meth.ds. -Re-trospective ch.rt review of ch.ldren referred for visceral leish.aniasis from 1996 to 2003. Results. -Th.rty-two ch.ldren under 15 years of age and suffering from visceral leish.aniasis were treated with.liposomal amph.tericin B as first line treatment. Clinical and biological features were usual: age h.s population, treatment effectiveness was evaluated to 97%(one relapse). Under treatment, patients quickly improved. Drug regimens varied from 18 to 24 mg/kg (day 1 to 5, and day 10). Four oth.r ch.ldren were not treated with.first-line liposomal amph.tericin B during th. period. Th.s, th. incidence of pediatric visceral leish.aniasis was evaluated to be 0.61/100,000 ch.ldren h. region (2.74 in th. Alpes-Maritimes department, French.Riviera, and 0.6 in th. Bouch.s-du-Rh.ne department, Marseilles area). Conclusion. -Liposomal am-ph.tericin B treatment is usual for ch.ldren referred for visceral leish.aniasis in th.s region. Th.s treatment may be approved regarding th. h.gh.level of effectiveness and th. low number of adverse events. A two days drug regimen with.20 mg/kg sh.uld be evaluated. Moreover, th. incidence of th. pediatric visceral leish.aniasis in south.rn France is decreasing, but local variations may be observed.

BACKGROUND Th. prevalence of cardiovascular diseases,especially h.art failure,continues to rise *** h.art failure,increasing levels of circulating atrial natriuretic peptide(ANP)and brain natriuretic peptide(BNP)are associated with.a worsening of h.art failure and a poor *** To test wh.th.r a h.gh.concentration of BNP would inh.bit relaxation to *** Pulmonary arteries were dissected from disease-free areas of lung resection,as well as pulmonary artery rings of internal diameter 2.5–3.5 mm and 2 mm long,were *** artery rings were mounted in a multiwire myograph.and a basal tension of 1.61gf was *** equilibration for 60 min,rings were pre-constricted with.11.21μmol/L PGF2α(ec80),and concentration response curves were constructed to vasodilators by cumulative addition to th. myograph.*** Alth.ugh.both.ANP and BNP were found to vasodilate th. pulmonary vessels,ANP is more potent th.n ***50 of ANP and BNP were 8.96±0.21 and 7.54±0.18,respectively,and th. maximum efficacy(Emax)for ANP and BNP was-2.03 gf and-0.24 gf,*** addition of BNP,th. Emax of ANP reduced from-0.96gf to-0.675gf(P=0.28).CONCLUSION BNP could be acting as a partial agonist in small h.man pulmonary arteries,and inh.bits relaxation to *** levels of circulating BNP could be responsible for th. worsening of decompensated h.art *** finding could also explain th. disappointing results seen in clinical trials of ANP and BNP analogues for th. treatment of h.art failure.

A Simple Classification System for h.p Fractures

Background: We h.ve developed a h.p Fracture Classification wh.ch.stratifies patients into 4 groups upon admission according to fracture complexity. Th.s classification considers path.-anatomic and ph.siologic parameters and was developed to h.lp identify h.gh.risk patients, with.more surgical demands and consequently h.gh.r incidence of morbidity. Materials and Meth.ds: Data was gath.red prospectively for a coh.rt of 273 consecutive patients admitted over a 12- month.period between 2008 and 2009 and classified according to th. h.p Fracture Complexity Classification (C0 - C3). Th. following outcome measures (mortality and length.of h.spital stay) were reviewed at th.rty days and one year. Result: Th. overall mortality was 4.4% at 30 days and 20% at 1 year.
