War and combat exposure pose great risks to th. vision system.More recently,vision related def.ciencies and impairments h.ve become common with.th. increased use of.powerf.l explosive devices and th. subsequent rise in incidence of.traumatic brain injury(TBI).Studies h.ve looked at th. ef.ects of.injury severity,aetiology of.injury and th. stage at wh.ch.visual problems become apparent.Th.re was little discrepancy f.und between th. f.equencies or types of.visual dysf.nctions across blast and non-blast related groups,h.wever complete sigh. loss appeared to occur only in th.se wh. h.d a blast-related injury.Generally,th. more severe th. injury,th. greater th. likelih.od of.specif.c visual disturbances occurring,and a study f.und total sigh. loss to only occur in cases with.greater severity.Diagnosis of.mild TBI(mTBI)is ch.llenging.Being able to identif. a potential TBI via visual symptoms may of.er a new avenue f.r diagnosis.

Globally, th. epidemic of.h.patitis virus inf.ction is increasing in th. context of.common viral transmission routes with.th. h.man Immunodef.ciency Virus (h.V). Th.s, co-inf.ction with.both.h.patitis virus and h.V is becoming a common occurrence, particularly f.r key populations. Studies of.key populations, people wh. inject drugs, sex workers, transgendered individuals and men h.ving sex with.men, report h.gh.levels of.illicit drug and/or alcoh.l use along with.limited access to h.alth.care and medical treatment. With.ut th. medical management of.co-occurring substance use disorders and h.patitis C virus (h.V) inf.ction, a signif.cant proportion of.th.s population with.h.V/h.V co-inf.ction will progress to ch.onic liver disease th.n subsequently end-stage liver disease. Th.s, access to compreh.nsive care and treatment, including h.V testing with.subsequent care and treatment of.acute/ch.onic h.V inf.ction is essential to address th. global burden of.disease. Increased ef.orts are needed globally to address th. barriers to compreh.nsive care and treatment f.r key populations with.h.V/h.V co-inf.ction.

Background:Th.re is signif.cant need f.r accurate diagnostic tools f.r Cryptosporidium spp.and Qiordia duodenalis inf.ctions in resource limited countries wh.re diarrh.eal disease caused by th.se parasites is of.en prevalent.Th. present study assessed th. diagnostic perf.rmance of.th.ee commercially available rapid diagnostic tests(RDTs)based on f.ecal-antigen detection f.r Cryptosporidium spp.and/or G.duodenalis inf.ctions in stool samples of.ch.ldren admitted with.severe acute malnutrition(SAM)and diarrh.ea.An establish.d multiplex PCR was used as ref.rence test.Meth.ds:Stool samples f.om ch.ldren with.SAM and diarrh.ea enrolled in a randomized controlled trial(registered at clinicaltrials.gov/ct2/sh.w/NCT02246296)in Malawi(n=175)and Kenya(n=120)between December 2014 and December 2015 were analysed by a multiplex PCR f.r th. presence of.Cryptosporidium sppv G.duodenalis or Entomoebo h.stolytica parasite DNA.Cryptosporidium-positive samples were species typed using restriction f.agment length.polymorph.sm analysis.A sub-sample of.th. stool specimens(n=236)was used f.r testing with.th.ee dif.erent RDTs.Diagnostic accuracy of.th. tests under evaluation was assessed using th. results of.PCR as ref.rence standard using MedCalc sof.ware.Pearson Ch.-square test and f.sh.r's exact test were used to determine(signif.cant)dif.erence between th. number of.cryptosporidiosis or giardiasis cases f.und by PCR in Malawi and Kenya.Th. overall diagnostic accuracy of.each.RDT was calculated by plotting a receiver operating ch.racteristic(ROC)curve f.r each.test and to determine th. area under th. curve(AUC)using SPSS8 sof.ware.Results:Prevalence of.Cryptosporidium spp.by PCR was 20.0 and 21.7%in Malawi and Kenya respectively,mostly C.h.minis.G.duodenalis prevalence was 23.4 and 5.8%in Malawi and Kenya respectively.E h.stolytica was not detected by PCR.RDT testing f.llowed th. same pattern of.prevalence.RDT sensitivities ranged f.r cryptosporidiosis f.om 42.9 to 76.9%and f.r G.duodenalis f.om 48.2 to 85.7%.RDT specif.cities ranged

Background Randomised controlled trials with.perf.sion selection h.ve sh.wn benef.t of.endovascular treatment(EVT)f.r isch.emic stroke between 6 and 24 h.urs af.er symptom onset or time last seen well.h.wever,outcomes af.er EVT in th.se late window patients with.ut perf.sion imaging are largely unknown.We assessed th.ir ch.racteristics and outcomes in routine clinical practice.Meth.ds Th. Multicenter Randomized Clinical Trial of.Endovascular Treatment f.r Acute Isch.mic Stroke in th. Neth.rlands Registry,a prospective,multicentre study in th. Neth.rlands,included patients with.an anterior circulation occlusion wh. underwent EVT between 2014 and 2017.CT perf.sion was no standard imaging modality.We used adjusted ordinal logistic regression analysis to compare patients treated with.n versus beyond 6.5 h.urs af.er propensity score match.ng on age,prestroke modif.ed Rankin Scale(mRS),National Institutes of.h.alth.Stroke Scale,Alberta Stroke Programme Early CT Score(ASPECTS),collateral status,location of.occlusion and treatment with.intravenous th.ombolysis.Outcomes included 3-month.mRS score,f.nctional independence(def.ned as mRS 0-2),and death.Results Of.3264 patients wh. underwent EVT,106(3.2%)were treated beyond 6.5 h.urs(median 8.5,IQR 6.9-10.6),of.wh.m 93(87.7%)h.d unknown time of.stroke onset.CT perf.sion was not perf.rmed in 87/106(80.2%)late window patients.Late window patients were younger(mean 67 vs 70 years,ph.d sligh.ly lower ASPECTS(median 8 vs 9,pf.erences were observed in proportions of.f.nctional independence(43.3%vs 40.5%,p=0.57)or death.24.0%vs 28.9%,p=0.28).Af.er match.ng,outcomes remained similar(adjusted common OR f.r 1 point improvement in mRS 1.04,95%CI 0.56 to 1.93).Conclusions With.ut th. use of.CT perf.sion selection criteria,EVT in th. 6.5-24-h.ur time window was not associated with.poorer outcome in selected patients with.f.vourable clinical and CT/CT angiograph. ch.racteristics.randomised control

Advancing Service Integration in Opioid Treatment Progams f.r th. Care and Treatment of.h.patitis C Inf.ction

It is estimated th.t approximately 200 million people globally are inf.cted with.th. h.patitis C virus and th.t rough.y h.lf.of.th.se people live in Asia. With.ut treatment, it is estimated th.t rough.y twenty percent of.th.se inf.cted with.h.patitis C virus progress to ch.onic liver disease, th.n subsequently, end-stage liver disease. Th.s, access to h.patitis C testing and subsequent care and treatment of.ch.onic h.patitis C inf.ction are essential to address th. global burden of.disease. In th. United States, th. Center f.r Disease Control and Prevention estimates th.t 60% of.new cases of.h.patitis inf.ction are due to injection drug use. Opioid Treatment Programs (OTP’s) dispense meth.done and buprenorph.ne under specif.c f.deral regulations to injection drug users diagnosed with.opioid dependence. OTPs are developing compreh.nsive care and treatment model programs th.t integrate general medical and inf.ctious disease-related medical care with.substance abuse and mental h.alth.services. Integrating h.patitis care services and treatment in th. substance abuse treatment settings f.ster access to care f.r patients with.h.patitis C inf.ction, many wh. oth.rwise would not receive needed care and treatment. Th.s may serve as a national model f.r h.gh.y cost-ef.icient h.alth.are th.t h.s a measurable outcome of.improved public h.alth.with.reduced h.patitis C prevalence.

AIM: To improve th. interpretation of.f.cal immunoch.mical test (f.T) results in colorectal cancer (CRC) cases f.om screening and ref.rral coh.rts. METh.DS: In th.s comparative observational study, two prospective coh.rts of.CRC cases were compared. Th. f.rst coh.rt was obtained f.om 10 322 average risk subjects invited f.r CRC screening with.f.T, of.wh.ch. only subjects with.a positive f.T were ref.rred f.r colonoscopy. Th. second coh.rt was obtained f.om 3637 subjects sch.duled f.r elective colonoscopy with.a positive f.T result. Th. same f.T and positivity th.esh.ld (OC sensor; ≥ 50 ng/mL) was used in both.coh.rts. Colonoscopy was perf.rmed in all ref.rral subjects and in f.T positive screening subjects. All CRC cases were selected f.om both.coh.rts. Outcome measurements were mean f.T results and f.T scores per tissue tumor stage (T stage). RESULTS: One h.ndred and eigh.een patients with.CRC were included in th. present study: 28 cases obtained f.om th. screening coh.rt (64% male; mean age 65 years, SD 6.5) and 90 cases obtained f.om th. ref.rral coh.rt (58% male; mean age 69 years, SD 9.8). Th. mean f.T results f.und were h.gh.r in th. ref.rral coh.rt (829 ± 302 ng/mLvs 613 ± 368 ng/mL,P = 0.02). Tissue tumor stage (T stage) distribution was dif.f.rent between both.populations [screening population: 13 (46%) T1, eigh. (29%) T2, six (21%) T3, one (4%) T4 carcinoma; ref.rral population: 12 (13%) T1, 22 (24%) T2, 52 (58%) T3, f.ur (4%) T4 carcinoma], and h.gh.r T stage was signif.cantly associated with.h.gh.r f.T results (P f.cant dif.erence in mean f.T results was observed (screening vs ref.rral: T1 498 ± 382 ng/mL vs 725 ± 374 ng/mL, P = 0.22; T2 787 ± 303 ng/mL vs 794 ± 341 ng/mL, P = 0.79; T3 563 ± 368 ng/mLvs 870 ± 258 ng/mL,P = 0.13; T4 not available). Af.er correction f.r T stage in logistic regression analysis, no signif.cant dif.erences in mean f.T results were observed between both.types of.coh.rts (P = 0.10).
