Introduction The efficacy and safety of local intra-arterial(IA)thrombolytics during endovascular thrombectomy(eVT)for large-vessel occlusions is *** analysed how often IA thrombolytics were administered in the Multicenter Randomized Clinical Trial of endovascular Treatment for Acute Ischemic Stroke in the Netherlands(MR CLeAN)Registry,whether it was associated with improved functional outcome and assessed technical and safety outcomes compared with eVT without IA *** In this observational study,we included patients undergoing eVT for an acute ischaemic stroke in the anterior circulation from the MR CLeAN Registry(March 2014-November 2017).The primary endpoint was favourable functional outcome,defined as an modified Rankin Scale score≤2 at 90 *** endpoints were reperfusion status,early neurological recovery and symptomatic intracranial haemorrhage(sICH).Subgroup analyses for IA thrombolytics as primary versus adjuvant revascularisation attempt were *** Of the 2263 included patients,95(4.2%)received IA thrombolytics during *** IA thrombolytics administered were urokinase(median dose,250000 IU(IQR,193750-250000))or alteplase(median dose,20 mg(IQR,12-20)).No association was found between IA thrombolytics and favourable functional outcome(adjusted OR(aOR),1.16;95%CI 0.71 to 1.90).Successful reperfusion was less often observed in those patients treated with IA thrombolytics(aOR,0.57;95%CI 0.36 to 0.90).The odds of sICH(aOR,0.82;95%CI 0.32 to 2.10)and early neurological recovery were comparable between patients treated with and without IA *** primary and adjuvant revascularisation attempts,IA thrombolytics were more often administered for proximal than for distal *** outcomes were comparable for patients receiving IA thrombolytics as a primary versus adjuvant revascularisation *** Local IA thrombolytics were rarely used in the MR CLeAN *** the relatively small st

Abundances of important and imperiled fishes of the Snake River Basin continue to *** assessed the rationale for breaching the four lower Snake River Basin dams to prevent complete loss of these fishes,and to maximize their likelihood of *** summarize the science surrounding Sockeye Salmon(Oncorhynchus nerka),Chinook Salmon(***),steelhead(***),Bull Trout(Salvelinus confluentus),White Sturgeon(Acipenser transmontanus),and Pacific Lamprey(entosphenus tridentatus).From this,we drew ten conclusions:(1)development of the Columbia River System(including the Snake River Basin)has converted mainstem rivers into reservoirs,altering fish behavior and survival;(2)most populations currently record their lowest abundance;(3)the Columbia River System dams reduce productivity of diadromous fishes in the highest-quality spawning grounds that could buffer against future climate dynamics;(4)past actions have done little to reduce impacts or precipitate recovery;(5)the Columbia River System constrains survival and productivity of salmon,steelhead and Bull Trout;(6)Snake River Basin salmon and steelhead remain at high extinction risk;(7)eliminating migration impediments and improving mainstem habitats are essential for maintaining genetic diversity and improving Bull Trout persistence;(8)the lower Snake River Basin dams preclude passage of adult White Sturgeon,constraining gene flow and recruitment;(9)the lower Snake River Basin dams impede dramatically passage of adult and juvenile Pacific Lamprey,and(10)Snake River Basin Pacific Lamprey is at high risk of *** the four lower Snake River Basin dams is an action likely to prevent extirpation and extinction of these *** from the Columbia River System can inform conservation in other impounded rivers.

Hepatitis B and C infection and liver disease trends among human immunodeficiency virus-infected individuals

AIM:To examine trends in and correlates of liver disease and viral hepatitis in an human immunodeficiency virus (HIV)-infected cohort. MeTHODS:The multi-site adult/adolescent spectrum of HIV-related diseases (ASD) followed 29 490 HIVinfected individuals receiving medical care in 11 U.S. metropolitan areas for an average of 2.4 years,and a total of 69 487 person-years,between 1998 and 2004. ASD collected data on the presentation,treatment,and outcomes of HIV,including liver disease,hepatitis screening,and hepatitis diagnoses. ReSULTS:Incident liver disease,chronic hepatitis B virus (HBV),and hepatitis C virus (HCV) were diagnosed in 0.9,1.8,and 4.7 per 100 person-years. HBV and HCV screening increased from fewer than 20% to over 60% during this period of observation (P eaths occurred in 57% of those diagnosed with liver disease relative to 15% overall (P erall 10% of deaths occurred among individuals with a diagnosis of liver disease. Despite care guidelines promoting screening and vaccination for HBV and screening for HCV,screening and vaccination were not universally conducted or,if conducted,not documented. CONCLUSION:Due to high rates of incident liver disease,viral hepatitis screening,vaccination,and treatment among HIV-infected individuals should be a priority.

Characteristics of Anthropogenic Sulfate and Carbonaceous Aerosols over east Asia: Regional Modeling and Observation

The authors present spatial and temporal characteristics of anthropogenic sulfate and carbonaceous aerosols over east Asia using a 3-D coupled regional climate-chemistry-aerosol model, and compare the simulation with the limited aerosol observations over the region. The aerosol module consists of SO2, SO4^2-, hydrophobic and hydrophilic black carbon （BC） and organic carbon compounds （OC）, including emission, advections, dry and wet deposition, and chemical production and conversion. The simulated patterns of SO2 are closely tied to its emission rate, with sharp gradients between the highly polluted regions and more rural areas. Chemical conversion （especially in the aqueous phase） and dry deposition remove 60% and 30% of the total SO2 emission, respectively. The SO4^2- shows less horizontal gradient and seasonality than SO2, with wet deposition （60%） and export （27%） being two major sinks. Carbonaceous aerosols are spatially smoother than sulfur species. The aging process transforms more than 80% of hydrophobic BC and OC to hydrophilic components, which are removed by wet deposition （60%） and export （30%）. The simulated spatial and seasonal SO4^2-, BC and OC aerosol concentrations and total aerosol optical depth are generally consistent with the observations in rural areas over east Asia, with lower bias in simulated OC aerosols, likely due to the underestimation of anthropogenic OC emissions and missing treatment of secondary organic carbon. The results suggest that our model is a useful tool for characterizing the anthropogenic aerosol cycle and for assessing its potential climatic and environmental effects in future studies.

The current pandemic due to the severe acute respiratory syndrome coronavirus 2 has caused an extreme burden for health care systems globally,and the number of cases is expected to continue to increase,at least in the immediate *** virus is estimated to have infected more than 1.5 million *** available reports suggest that gastrointestinal(GI)involvement in coronavirus disease 2019(COVID-19)is common and in some cases the GI symptoms may precede the respiratory *** addition to direct effects of severe acute respiratory syndrome coronavirus 2,the infected patients remain at risk for the complications commonly managed by gastroenterology and hepatology *** most commonly reported GI manifestation of COVID-19 is diarrhea,which is reported in a third to up to more than half of the *** to moderate elevation of the liver enzymes are also common,although no case of acute liver failure has been reported so *** of the medications used for treatment of COVID-19 can also be associated with GI symptoms or liver injury and can be included in the differential diagnosis in these *** the diagnosis of the infection is currently based on RNA analysis in respiratory samples,the available literature on fecal shedding of this virus suggests that fecal RNA testing might prove to be a useful diagnostic *** is reasonable to delay all non-urgent endoscopic procedures during the peak of the pandemic and use additional protective equipment such as N95 respirators during endoscopy while most patients can be considered high risk for having been exposed to the virus.

Reconciling Local and Global Agendas in Sustainable Development: Participatory Research with Indigenous Andean Communities

This paper discusses participatory research in the Andes and presents a case study in Cotacachi, ecuador, where sustainability scientists and indigenous people seek common ground in their respective but drastically different research and social agendas. Participatory research based on Andean experiences pre-dated and inspired much of the later international movement in agriculture, health, and conservation. Andean communities have a long history in demanding that outsiders address the needs of the community as a condition for carrying out scientific or applied activities. What an Andean community, however, sees as relevant may or may not be within the rubric of ‘participatory research’ as it is practiced throughout much of the world. In fact, overzealous participatory researchers are just as bothersome as their predecessors bearing long questionnaires. More important to Andean people is an equitable relationship with researchers and developers in which exchanges of value are made. A distinction between ‘enriching’ and ‘extractive’ research is drawn. In the case of the SANReM project in Cotacachi, ecuador, scientists carried out enriching research activities of interest to local people as a wayto generate social capital for conducting basic research which does not have an obvious, immediate local benefit. The requested research did not have a conventional participatory methodology but provided valuable products (educational opportunity, germplasm, community visualization tools, and information) to the indigenous community in exchange for time and resources to conduct research on more basic natural resource questions. We argue that in the Andean context the key to reconciling the needs of scientists and of local needs is seeking new forms of equitable collaboration which reach beyond the present and now somewhat tired discourse of ‘participation’.

Rapid on-site identification of volatile organic compounds （VOCs） in ambient air is an important first step in remediation efforts. This study describes modification of a commercially available, portable GC/MS system and development of an analysis protocol for rapid （〈 3 min） sampling and identification of VOCs typically found at contaminated sites at the low ppbv level.
