Management of inflammatory bowel disease with Clostridium difficile infection

To address the management of Clostridium difficile (C. difficile) infection (CDI) in the setting of suspected inflammatory bowel disease (IBD)-flare. METHODSA systematic search of the Ovid MEDLINE and EMBASE databases by independent reviewers identified 70 articles including a total of 932141 IBD patients or IBD-related hospitalizations. RESULTSIn those with IBD, CDI is associated with increased morbidity, including subsequent escalation in IBD medical therapy, urgent colectomy and increased hospitalization, as well as excess mortality. Vancomycin-containing regimens are effective first-line therapies for CDI in IBD inpatients. No prospective data exists with regards to the safety or efficacy of initiating or maintaining corticosteroid, immunomodulator, or biologic therapy to treat IBD in the setting of CDI. Corticosteroid use is a risk factor for the development of CDI, while immunomodulators and biologics are not. CONCLUSIONStrong recommendations regarding when to initiate IBD specific therapy in those with CDI are precluded by a lack of evidence. However, based on expert opinion and observational data, initiation or resumption of immunosuppressive therapy after 48-72 h of targeted antibiotic treatment for CDI may be considered.

Emerging data have highlighted the co-existence of nonalcoholic fatty liver disease(NAFLD) and inflammatory bowel disease; both of which are increasingly prevalent disorders with significant complications and impact on future health burden. Cross-section observational studies have shown widely variable prevalence rates of co-existing disease,largely due to differences in disease definition and diagnostic tools utilised in the studies. Age,obesity,insulin resistance and other metabolic conditions are common risks factors in observational studies. However,other studies have also suggested a more dominant role of inflammatory bowel disease related factors such as disease activity,duration,steroid use and prior surgical intervention,in the development of NAFLD. This suggests a potentially more complex pathogenesis and relationship between the two diseases which may be contributed by factors including altered intestinal permeability,gut dysbiosis and chronic inflammatory response. Commonly used immunomodulation agents pose potential hepatic toxicity,however no definitive evidence exist linking them to the development of hepatic steatosis,nor are there any data on the impact of therapy and prognosis in patient with co-existent diseases. Further studies are required to assess the impact and establish appropriate screening and management strategies in order to allow early identification,intervention and improve patient outcomes.

Prevalence and factors associated with vitamin C deficiency in inflammatory bowel disease

BACKGROUND Patients with inflammatory bowel disease(IBD) are prone to several nutritional deficiencies. However, data are lacking on vitamin C deficiency in Crohn’s disease(CD) and ulcerative colitis(UC) patients, as well as the impact of clinical, biomarker and endoscopic disease severity on the development of vitamin C *** To determine proportions and factors associated with vitamin C deficiency in CD and UC *** In this retrospective study, we obtained clinical, laboratory and endoscopic data from CD and UC patients presenting to the IBD clinic at a single tertiary care center from 2014 to 2019. All patients had an available plasma vitamin C level. Of 353 subjects who met initial search criteria using a cohort discovery tool, 301ultimately met criteria for inclusion in the study. The primary aim described vitamin C deficiency(≤ 11.4 μmol/L) rates in IBD. Secondary analyses compared proportions with deficiency between active and inactive IBD. Multivariate logistic regression analysis evaluated factors associated with *** Of 301 IBD patients, 21.6% had deficiency, including 24.4% of CD patients and 16.0% of UC patients. Patients with elevated C-reactive protein(CRP)(39.1% vs 16.9%, P < 0.001) and fecal calprotectin(50.0% vs 20.0%, P = 0.009) had significantly higher proportions of deficiency compared to those without. Penetrating disease(P = 0.03),obesity(P = 0.02) and current biologic use(P = 0.006) were also associated with deficiency on univariate analysis. On multivariate analysis, the objective inflammatory marker utilized for analysis(elevated CRP) was the only factor associated with deficiency(odds ratio = 3.1, 95%confidence interval: 1.5-6.6, P = 0.003). Introduction:Alzheimer 's disease(AD) is a common neurodegenerative disorder and the primary cause of dementia. Considerable evidence supports the “amyloid hypothesis,” stating that the pathogenesis of AD is primarily caused by the deposition of amyloid-β(Aβ), which drives tau phosphorylation, neuroinflammation, and neurodegeneration in the brain. The amyloid hypothesis is strengthened by the significant and moderate benefit of lecanemab, a humanized antibody through an anti-amyloid mechanism,showing slowed clinical decline(van Dyck et al.,2023). The recent positive results of anti-amyloid trials have brought back focus on the amyloid hypothesis through biochemical, genetic, and pharmacological approaches(Zhang, 2023).

Application of magnetic flux controllers/concentrators to induction heating coils can drastically improve the process efficiency and heat pattern control. Presentation includes: benefits provided by flux controllers, materials available for controllers, application techniques, computer assisted design of induction coils with concentrators, examples of applications. Depending on induction system design, magnetic flux controllers can concentrate heating in a specified area, change heat source distribution and shield a particular part zone or external area preventing unintended eddy current heating. Besides of the coil efficiency improvement and optimal power distribution, magnetic flux controllers reduce the coil current demand from a supplying circuitry thus strongly reducing losses in busswork, cables, transformers and inverter components. Improvement that can be achieved due to magnetic flux controllers is case dependable. 2D and 3D computer simulation allows the designer to predict accurately effect of controllers on the coil parameters and temperature distribution and optimize the whole electromagnetic system. Special attention in presentation is paid to new magnetodielectric materials optimized for induction heating conditions. These materials have high magnetic permeability and saturation flux density, excellent machinability, good chemical and temperature resistance. Concentrators from these materials can work in a wide range of frequencies and specific powers. Examples of magnetic flux controller application include surface hardening of shafts and gears, induction surface hardfacing and brazing.

Sliding-window multi-stream join （SWMJ） is a fundamental operation for correlating information from dif- ferent streams. We provide a solution to the problem of as- sessing significance of the SWMJ result by focusing on the relative frequency of windows satisfying a given equijoin predicate as the most important parameter of the SWMJ re- suit. In particular, we derive a formula for computing the expected relative frequency of windows satisfying a given equijoin predicate that can be. evaluated in quadratic time in the window size given a proposed probabilistic model of the multi-stream. In experiments conducted on a daily rain- fall data set we demonstrate the remarkable accuracy of our method, which confirms our theoretical analysis.
