Objective:Depression a.d a.xiety a.e frequently observed in hea.t fa.lure(HF)pa.ients;however,the effect of such fa.tors on hospita.iza.ion costs of HF pa.ients,a.d whether such costs va.y by ra.e a.d gender rema.n poorly *** a.a.ysis delinea.ed the preva.lence of depression/a.xiety a.ong HF pa.ients a.d estima.ed the effect of ra.e a.d gender on hospita.iza.ion ***:We exa.ined the 2008 files of the Tennessee Hospita. Discha.ge Da.a.System(Hdds)on pa.ients(≥20 yea.s of a.e)with a.prima.y dia.nosis of HF(ICD-9 codes 402,404,a.d 428)a.ong with demogra.hic da.a.depression/a.xiety dia.noses,hospita. costs,a.d *** the HF sa.ple(n=16,889)53%were fema.e a.d 23%were *** a.d gender differences in hospita. costs were eva.ua.ed for the following three groups:(1)HF pa.ients with depression/a.xiety(HF+D);(2)HF-only pa.ients without depression/a.xiety(HFO);a.d(3)HF pa.ients with other menta. dia.noses(HF+m).Results:HF wa. significa.tly(pa.ong bla.ks compa.ed to whites,a.d higher a.ong ma.es tha. *** 25%of HF pa.ients ha. depression/a.xiety(more whites a.d fema.es were depressed).HF pa.ients a.era.ed more tha. 3 comorbidities(bla.ks ha. a.grea.er number of comorbidities a.d hospita.iza.ion cost for the yea.).Costs were higher a.ong HF+D pa.ients compa.ed to HFO *** HF+D pa.ients,costs were higher for bla.k ma.es compa.ed with white *** cost pa.terns preva.led la.gely beca.se of higher comorbidities tha. required more re-a.missions a.d longer hospita. ***:Ra.e a.d depression/a.xiety a.e a.socia.ed with increa.ed hospita.iza.ion costs of HF *** higher costs a.ong bla.ks reflect the higher burden of comorbidities,such a. hypertension a.d dia.etes,which ca.ls for widesprea. dissemina.ion,a.option,a.d implementa.ion of proven interventions for the control of these comorbidities.

Introduction: This study reviewed pa.ients’ demogra.hic, clinica. a.d trea.ment cha.a.teristics to identify prognostic fa.tors a.socia.ed with surviva. of prosta.e ca.cer a.ter developing bone meta.ta.es. We explored the ra.ia. dispa.ities in these fa.tors a.d how they rela.e with surviva.. methods: We conducted a.retrospective study on 79 men dia.nosed with bone meta.ta.is seconda.y to prosta.e ca.cer who underwent surgery a. a.single institution?from November 1977 to June 2011. Descriptive sta.istics were used to summa.ize pa.ients’ cha.a.teristics. The Ka.la.-meier method wa. used to estima.e cha.a.teristics of the surviva. distribution using two origina.ion points—dia.nosis a.d surgery. Cox ha.a.d regression explored the rela.ionship between prognostic fa.tors a.d overa.l surviva.. Results: The ma.ority of men were White (n = 63;80%) followed by Bla.k (n = 7;9%), Hispa.ic (n = 7;9%), a.d a.ia. (n = 2;2%). multiva.ia.e fa.tors a.socia.ed with poorer surviva. a.ter bone meta.ta.is surgery included ra.e (Bla.k), Glea.on score > 8, a.d ra.ia.ion trea.ment. Pa.ients not receiving ra.ia.ion ha. a.longer surviva. experience rela.ive to pa.ients who received ra.ia.ion before or a.ter surgery (10.3 vs 6.5 months;P = 0.030). There wa. a. a.socia.ion of PSa.level a. the time of bone meta.ta.is dia.nosis with surviva. following dia.nosis but prior to surgery. The media. time interva. (Tm in months) between prosta.e ca.cer dia.nosis a.d bone meta.ta.is dia.nosis wa. 39.1 (White), 31.2 (Hispa.ic), 15 (Bla.ks) a.d 43 (a.ia.s). Pa.ients with Tm m > 35 months (HR = 3.22;P Conclusion: The media. surviva. a.d time interva. from prosta.e ca.cer dia.nosis to bone meta.ta.is dia.nosis wa. shorter in Bla.ks with respect to other ra.es. The more a.gressive na.ure of the disea.e in Bla.ks is likely due to the biology of the disea.e ra.her tha. a.cess to trea.ment.

背景：评估肺动脉导管（pulmona.y a.tery ca.heter，Pa.）的随机临床试验（ra.domized clinica. tria.s，RCTs）一直受到研究样本量较小的限制。一些非随机研究表明，Pa.的应用会增加患者的发病率和死亡率。目的：评估Pa.对重症患者的影响。数据来源：在mEDLINE（1985～2005年）数据库、Cochra.e对照试验登记站（1988～2005年）、国立卫生研究院***网站数据库以及美国食品与药物管理局网站上对将患者随机分配至Pa.组和非Pa.组的RCTs进行检索。在重症心力衰竭患者中进行的ESCa.E试验结果也被纳入。检索词包括肺动脉导管、右心导管、导管和Swa.-Ga.z。研究选择：符合条件的研究应包含如下患者：接受手术治疗的患者，进入重症监护病房（intensive ca.e unit，ICU）的患者，因重度心力衰竭入院或者诊断为急性呼吸窘迫综合征和／或败血症的患者。作为观察指标，入选的研究报告了患者死亡、住院天数或ICU住院天数。数据提取：各研究的入选标准、基线特征、干预措施、预后以及方法学质量的信息由2位作者提取。不同意见通过协商达成共识解决。数据综合：13项RCTs共包含5051例随机患者。各试验的血流动力学目标和治疗策略各不相同。使用随机效应模型估计患者死亡、住院天数以及使用变力和静脉血管扩张药物的优势比（odds ra.ios，ORs）。死亡率的合并OR为1．04（95％可信区间[confidence interva.，CI]，0．90～1．20；P=0．59）。Pa.组平均住院天数与非Pa.组平均住院天数的差为0．11天（95％CI，-0．51～0．74；P=0．73）。Pa.与变力药物（OR，1．58；95％CI，1．19～2．12；P=0．002）和静脉内血管扩张药物的使用率较高相关（OR，2．35；95％CI，1．75～3．15；P〈0．001）。结论：对于重症患者而言，Pa.的使用既不增加总体死亡率或住院天数，也没有给患者带来益处。尽管相关RCTs已进行了近20年，但是仍未得出可以提高患者存活率的明确的Pa.使用策略。Pa.对临床预后的中性作用可能为对其在各种重症患者的使用缺乏有效的循证治疗所致。
