Outcome ofJudet＇s quadricepsplasty for knee contractures and the effect of local infiltration of epinephrine on reducing blood loss

OBJECTIVE:To evaluate the effectiveness of Judet's quadricepsplasty for treatment of knee contractures and to identify the effect of local infiltration of epinephrine on blood loss associated with this procedure.;METHODS:A retrospective cohort study was conducted in which all cases of knee contractures managed with Judet's quadricepsplasty from 1st January 2009 to 31st December 2013 were included and were divided into two groups. The epinephrine group included patients who were infiltrated with diluted epinephrine (1:400,000) along with xylocaine, around the operative field 15 min prior to the incision time, while the control group did not receive any infiltration. Judet's outcome, blood loss, drop in hemoglobin and required blood transfusion were noted for all patients and compared between both groups.;RESULTS:Most common preceding pathology identified for the development of knee contractures was periarticular fracture while ilizarov application was the most common etiology. Both groups were found similar in all preoperative characteristics except preoperative flexion contracture (p = 0.02). All functional outcome measures including Judet's outcome were similar in both groups. In contrast, duration of surgery (p = 0.01), blood loss (p = 0.02), drop in hemoglobin (p = 0.01) and number of transfusions (p = 0.03) were significantly reduced in epinephrine group.;CONCLUSIONS:Judet's quadricepsplasty is a useful procedure to increase the range of motion of rigid knees and local infiltration of epinephrine is effective in decreasing the amount of subsequent blood loss and transfusion requirements.

常规热喷涂工艺制备的金属涂层内的粒子界面弱结合导致其不能为基材提供长效的腐蚀防护,因此,采用大气等离子喷涂,实现粒子间的冶金结合,制备高致密Al-15%WC(体积分数)复合涂层。结果表明,由于WC颗粒中C元素的去氯效应,以及超高温熔滴(>1800℃)间的自冶金结合,在最优等离子喷涂条件下制备出无氧化物杂质的涂层。涂层的致密结构使其表现出优异耐腐蚀性能,其腐蚀电流密度比镁合金基体降低4个数量级,比纯铝块材降低2个数量级。WC硬质颗粒的添加使Al-WC复合涂层的耐磨损性能相较纯Al块材提高1个数量级。

BACKGROUND Surgical site infections are a major cause of morbidity and mortality following orthopedic *** efforts to identify sources of contamination in the operating rooms have implicated mobile *** To investigate microbial colonization on the mobile phones of health care professionals in the orthopedic operating *** We conducted a cross-sectional study involving culture and sensitivity analysis of swabs taken from the mobile phones of orthopedic and anesthesia attendings,residents,technicians and nurses working in the orthopedic operating rooms over a period of two *** and cell phone related factors were recorded using a questionnaire and the factors associated with contamination were *** Ninety-three of 100 mobile phones were *** isolated were Coagulase-negative Staphylococcus(62%),Micrococcus(41%)and Bacillus(26%).The risk of contamination was increased with mobile covers and cracked screens and decreased by cell phone *** Mobile phones belonging to health care workers are frequently contaminated with pathogenic bacteria with the potential of transferring drug resistance to nosocomial *** investigating the relationship to surgical site infections need to be *** concept of"mobile hygiene"involving the change of mobile covers,replacement of cracked screens or even wiping the phone with an alcohol swab could yield the cost-effective balance that contaminated cell phones deserve until they are established as a direct cause of surgical site infections.

Computationally efficient locality-aware interconnection topology for multi-processor system-on-chip(MP-SoC)

This paper evaluates the Triplet Based Architecture,TriBA-a new idea in chip multiprocessor architectures and a class of Direct Interconnection Network(DIN).TriBA consists of a 2D grid of small,programmable processing units,each physically connected to its three neighbors so that advantageous features of group locality can be fully and efficiently *** communication model can be well characterized by locality properties and,any topology has its intrinsic,structural,locality *** propose a new criterion in performance evaluation that is based on the concept of locality in an interconnection network,the "lower layer complete connect".Our proposed criterion depicts how completely a processing node is connected to all its *** is compared with 2D Mesh and Binary Tree as static interconnection *** comparison/evaluation is enumerated from three orthogonal view points,viz.,computational speed,physical layout and *** analysis concludes that TriBA is computationally efficient interconnection strategy that exploits group locality in processing nodes.

Locality Aware Optimal Task Scheduling Algorithm for TriBA —— A Novel Scalable Architecture

An optimal algorithmic approach to task scheduling for, triplet based architecture（TriBA）, is proposed in this paper. TriBA is considered to be a high performance, distributed parallel computing architecture. TriBA consists of a 2D grid of small, programmable processing units, each physically connected to its three neighbors. In parallel or distributed environment an efficient assignment of tasks to the processing elements is imperative to achieve fast job turnaround time. Moreover, the sojourn time experienced by each individual job should be minimized. The arriving jobs are comprised of parallel applications, each consisting of multiple-independent tasks that must be instantaneously assigned to processor queues, as they arrive. The processors independently and concurrently service these tasks. The key scheduling issues is, when some queue backlogs are small, an incoming job should first spread its tasks to those lightly loaded queues in order to take advantage of the parallel processing gain. Our algorithmic approach achieves optimality in task scheduling by assigning consecutive tasks to a triplet of processors exploiting locality in tasks. The experimental results show that tasks allocation to triplets of processing elements is efficient and optimal. Comparison to well accepted interconnection strategy, 2D mesh, is shown to prove the effectiveness of our algorithmic approach for TriBA. Finally we conclude that TriBA can be an efficient interconnection strategy for computations intensive applications, if tasks assignment is carried out optimally using algorithmic approach.

Oxidative desulfurization of dibenzothiophene over Fe promoted Co–Mo/Al_2O_3 and Ni–Mo/Al_2O_3 catalysts using hydrogen peroxide and formic acid as oxidants

This work reports the enhancing effect of a highly cost effective and efficient metal, Fe, incorporation to Co or Ni based Mo/Al_2O_3 catalysts in the oxidative desulfurization(ODS) of dibenzothiophene(DBT) using H_2O_2 and formic acid as oxidants. The influence of operating parameters i.e. reaction time, catalyst dose, reaction temperature and oxidant amount on oxidation process was investigated. Results revealed that 99% DBT conversion was achieved at 60 °C and 150 min reaction time over Fe–Ni–Mo/Al_2O_3. Fe tremendously enhanced the ODS activity of Co or Ni based Mo/Al_2O_3 catalysts following the activity order: Fe–Ni–Mo/Al_2O_3 NFe–Co–Mo/Al_2O_3 NNi–Mo/Al_2O_3 NCo–Mo/Al_2O_3, while H_2O_2 exhibited higher oxidation activity than formic acid over all catalyst systems. Insight about the surface morphology and textural properties of fresh and spent catalysts were achieved using scanning electron microscopy(SEM), X-ray diffraction(XRD), energy dispersive X-ray(EDX)analysis, Atomic Absorption Spectroscopy(AAS) and BET surface area analysis, which helped in the interpretation of experimental data. The present study can be deemed as an effective approach on industrial level for ODS of fuel oils crediting to its high efficiency, low process/catalyst cost, safety and mild operating condition.

Fast Track Surgery and Its Outcome in Colorectal Surgery in a Tertiary Care Hospital

Background: Fast track surgery is an evidence-based multidisciplinary approach. The underlying principle is to enable patients to recover from surgery and leave the hospital sooner by minimizing the stress responses on the body during surgery. Our aim was to compare the outcome of fast-track protocol and conventional methods in colorectal surgery. Method: It was an observational cross-sectional study carried out at the Department of Surgery in different tertiary level hospitals, Dhaka Bangladesh during the period January 2014 to December 2017. Among this population, 50 patients were placed in the fast-track program (Group A) from January 2014 to December 2015 and 50 patients were in the conventional method (Group B), from January 2016 to December 2017. The fast-track patients were selected after receiving ethical approval from the Bangladesh College of Physician & Surgeons. Data analysis was done using the statistical package for social science (SPSS) for windows version 20. Results: The average age of the patients was 45.24 ± 16.65 years (range: 11 - 70 years) in the fast track group (Group A) and 43.24 ± 17.76 years in the conventional method (Group B). The majority were female between two groups, with 56% in group A and 52% in group B. General and surgical complications occurred in 9 (18%) patients and 11 (22%) patients respectively in group A. On the other hand in group B general and surgical complications occurred in 10 (20%) patients and 13 (26%) patients respectively. The average hospital stay was 9.24 ± 5.99 days in group A and the average hospital stay was 10.10 ± 6.04 days in group B. Conclusion: Making the decision to adopt fast-track surgery will challenge current traditional practice for all members of the multidisciplinary team across the whole local health community.
