Objective:To evaluate the potential effect of human Wharton’s jelly mesenchymal stem cells(hWJMSCs)on acute respiratory distress syndrome in lipopolysaccharide(LPS)-induced ***:The hWJMSCs(5×10^(4)/mL,5×10^(5)/mL,5×10^(6)/mL)were administered to rats on day 1 and day 8 after being induced by LPS(5 mg/kg body weight).TNF-αlevels in the lung and IL-18 and IL-1βlevels in the serum were measured using *** addition,caspase-1 expression in lung tissues was quantified using qRT-PCR,and NF-κB and IL-6 expressions were assessed using ***:The hWJMSCs decreased TNF-αlevels in the lung and plasma IL-18 and IL-1β***,the hWJMSCs downregulated the expressions of caspase-1,IL-6,and NF-κB in lung ***:The hWJMSCs can decrease inflammatory markers of acute respiratory distress syndrome in a rat model and may be further investigated for the treatment of acute respiratory distress syndrome.

Fused deposition modelling (FDM) is one of rapid prototyping (RP) technologies which uses an additive fabrication *** commercially available FDM model has different types of process parameters for different *** of the desired parts require excellent surface finish as well as good *** most common parameters requiring setup are the raster angle,tool path,slice thickness,build orientation,and deposition *** purpose of this paper is to discuss the process parameters of FDM Prodigy Plus (Stratasys,Inc.,Eden Prairie,MN,USA).Various selected parameters were tested and the optimum condition was *** quality of the parts produced was accessed in terms of dimensional accuracy and surface *** optimum parameters obtained were then applied in the fabrication of the master pattern prior to silicone rubber moulding (SRM).These parameters would reduce the post processing *** dimensional accuracy and surface roughness were analyzed using coordinate measuring machine (CMM) and surface roughness tester,*** on this study,the recommended parameters will improve the quality of the FDM parts produced in terms of dimensional accuracy and surface roughness for the application of SRM.

Gamma heating,which is deposited in irradiated targets or samples,is an important issue in research reactors because it affects the safety of samples and the reactor *** heating in the Reaktor Serba Guna ***(RSG GAS)can be measured or calculated using computer *** this paper,we present the results obtained by using gamma calorimeters to measure the gamma heating in the central irradiation position in RSG *** measurement results were verified and then compared with the calculation results obtained using the Gamset ***,the accuracy of the calculation results obtained using Gamset was inadequate,with more than 20%***,Gamset was initially created to calculate gamma heating in 35 MWth reactors,and the RSG GAS reactor has an operating power level of 30 *** address these issues,we developed a new program called NewGamset,built with an informative graphical user interface-based *** can employ new analytical approaches as it utilizes a calculation base that comprises 18 energy groups and can provide updated physical parameters of RSG *** results of gamma heating measurements obtained using gamma calorimeters made of graphite,aluminum,iron,and zirconiumwere 2.20±0.03 W/g,2.25±0.02 W/g,2.58±0.03 W/g,and 2.91±0.10 W/g,*** general,the calculation results were larger than the measurement results,and the average ratio of calculation to measurement result was 1.37 for Gamset and 1.02 for ***,it can be concluded that NewGamset provides more accurate results than *** also report the gamma heating estimation for common elements irradiated in the RSG GAS silicide core,with an operating power level of 15 MWth and 30 *** highly recommend that NewGamset be used for validating the gamma heating data of various target materials in RSG GAS.

Effect of varicocele repair on sperm retrieval rate and testicular histopathological patterns in men with nonobstructive azoospermia

Varicocele adversely affects semen ***,the effect of varicocele repair on the sperm retrieval rate and testicular histopathological patterns in men with nonobstructive azoospermia has not been widely *** retrospectively assessed the sperm retrieval rates and testicular histopathological patterns in men with nonobstructive azoospermia who were referred to the Urology Clinic in *** Mangunkusumo Hospital(Jakarta,Indonesia)and Bunda General Hospital(Jakarta,Indonesia)between January 2009 and December *** compared patients who had undergone a surgical sperm retrieval procedure for assisted reproductive technology no earlier than three months after varicocele repair and those who had not undergone varicocele *** study included 104 patients(age range:26–54 years),42 of whom had undergone varicocele repair before the sperm retrieval procedure and 62 who had *** spermatozoa were found in 29(69.1%)and 17(27.4%)patients who had undergone varicocele repair before the sperm retrieval procedure and those who had not undergone the repair,respectively(relative risk:2.51;95%confidence interval:1.60–3.96;P<0.001).A predicted probabilities graph showed consistently higher sperm retrieval rates for patients with varicocele repair,regardless of their follicle-stimulating hormone *** who underwent varicocele repair showed higher testicular histopathological patterns(P=0.001).In conclusion,men with nonobstructive azoospermia and clinical varicocele who underwent varicocele repair before the sperm retrieval procedure had higher sperm retrieval rates compared to those who did not undergo varicocele repair.

Validation of the trauma mortality prediction scores from a Malaysian population

Background:Well-known trauma mortality prediction scores such as New Injury Severity Score (NISS), Revised Trauma Score (RTS), and Trauma and Injury Severity Score (TRISS) have been externally validated from high-income countries with established trauma databases. However, these scores were never used in Malaysian population. In this current study, we attempted to validate these scoring systems using our regional trauma surgery database. Methods:A retrospective analysis of the regional Malaysian Trauma Surgery Database was performed over a period of 3 years from May 2011 to April 2014. NISS, RTS, Major Trauma Outcome Study (MTOS)-TRISS, and National Trauma Database (NTrD)-TRISS scores were recorded and calculated. Individual scoring system's performance in predicting trauma mortality was compared by calculating the area under the receiver operating characteristic (AUC) curve. Youden index and associated optimal cutoff values for each scoring system was calculated to predict mortality. The corresponding positive predictive value, negative predictive value, and accuracy of the cutoff values were calculated. Results:A total of 2208 trauma patients (2004 blunt and 204 penetrating injuries) with mean age of 36 (SD=16) years were included. There were 239 deaths with a corresponding mortality rate of 10.8%. The AUC calculated for the NISS, RTS, MTOS-TRISS, and NTrD-TRISS were 0.878, 0.802, 0.812, and 0.848, respectively. The NISS score with a cutoff value of 24, sensitivity of 86.6%and specificity of 74.3%, outperformed the rest (p<0.001). Mortality was predicted by NISS with an overall accuracy of 75.6%;its positive predictive value was at 29.02%and negative predictive value at 97.86%. Conclusion:Amongst the four scores, the NISS score is the best trauma mortality prediction model suited for a local Malaysian trauma population. Further validation with multicentre data in the country may require to ascertain the finding.

Malaysia has one of the highest total numbers of COVID-19 infections amongst the Southeast Asian nations,which led to the enforcements of the Malaysian"Movement Control Order"to prohibit disease *** overwhelming increasing amount of infections has led to a major strain on major healthcare *** leads to shortages in hospital beds,ventilators and critical personnel protective *** article focuses on the critical adaptations from a general surgery department in Malaysia which is part of a Malaysian tertiary hospital that treats COVID-19 *** core highlights of these strategies enforced during this pandemic are:(1)surgery ward and clinic decongestions;(2)deferment of elective surgeries;(3)restructuring of medical personnel work force;(4)utilization of online appli cations for tele-communication;(5)operating room(OR)adjustments and patient screening;and(6)continuing medical education and updating practices in context to *** adaptations were important for the continuation of emergency surgery services,preventing transmission of COVID-19 amongst healthcare workers and optimization of medical personnel work force in times of a global *** addition,an early analysis on the impact of COVID-19 pandemic and lockdown measures in Malaysia towards the reduction in total number of elective/emergent/trauma surgeries performed is described in this article.

Purpose: Amongst the ASEAN countries, Malaysia has the highest road fatality risk (>15 fatalities per 100 000 population) with 50% of these fatalities involving motorcyclist. This contributes greatly to ward admissions and poses a significant burden to the general surgery services. From mild rib fractures to severe intra-abdominal exsa nguinations, the spectrum of cases man aged by surgeons resulting from motorcycle accidents is extensive. The objective of this study is to report the clinical characteristics and identify predictors of death in motorcycle traumatic injuries from a Malaysian trauma surgery centre. Methods: This is a prospective cross-sectional study of all injured motorcyclists and pillion riders that were admitted to Hospital Sultanah Aminah and treated by the trauma surgery team from May 2011 to February 2015. Only injured motorcyclists and pillion riders were included in this study. Patient demography and predictors leading to mortality were identified. Significant predictors on univariate analysis were further analysed with multivariate analysis. Results: We included 1653 patients with a mean age of (35 ± 16.17) years that were treated for traumatic injuries due to motorcycle accidents. The mortality rate was 8.6%(142) with equal amount of motorcycle riders (788) and pillion riders (865) that were injured. Amongst the injured were male predominant (1 537) and majority of ethnic groups were the Malays (897) and Chinese (350). Severity of injury was reflected with a mean Revised Trauma Score (RTS) of 7.31 ± 1.29, New Injury Severity Score (NISS) of 19.84 ± 13.84 and Trauma and Injury Severity Score (TRISS) of 0.91 ± 0.15. Univariate and multivariate analysis revealed that age>35, lower GCS, head injuries, chest injuries, liver injuries, and small bowel injuries were significant predictors of motorcycle trauma related deaths with p < 0.05. Higher trauma severity represented by NISS, RTS and TRISS scores was also significant for death with p < 0.05. C

目的:为了调查云南肿瘤医院(昆明医科大学第三附属医院)非小细胞肺癌患者表皮生长因子受体(EGFR)的突变情况。方法:采用突变扩增系统(ARMS)聚合酶链式反应(PCR)检测54例经病理证实的非小细胞肺癌组织和相应血浆样本的EGFR突变,并进一步探讨EGFR突变、临床因素、样本类型(组织和血浆)和组织学类型(腺癌)之间的关系。结果:1、组织样本的突变率为44.4%,血浆样本的突变率为22.2%;女性(P = 0.01),不吸烟(P = 0.005),组织标本(P = 0.001)和肿瘤进展期(P = 0.003)有较高的EGFR突变率;2、腺癌与鳞状细胞癌的突变率无显著性差异;3、最常见的突变是外显子19缺失(37.5%)和L858R点突变(37.5%);血浆的敏感性为45.8%,特异性为96.7%,阳性预测值(PPV)为91.7%,阴性预测值(NPV)为69.04%。结论:1、在女性、非吸烟、肿瘤进展期、腺癌中EGFR突变率高;2、检测血浆样品中cfDNA EGFR突变是一种敏感性中等,特异性强的检测方法。
