Outcomes of early versus late irrigation and debridement of pediatric open long bone fractures

BACKGROUND Open long bone fractures are a major concern for pediatric patients due to the risk of surgical site infection(SSI).Early studies have recommended irrigation and debridement of open fractures within 6 hours-8 hours for the prevention of *** to the American College of Surgeons(ACS)Best Practice Guidelines,in 2015,irrigation and debridement should be done within 24 *** To identify whether early irrigation and debridement,within 8 hours,vs late,between 8 hours and 24 hours,for pediatric open long bone fractures impacts rate of *** Using retrospective data review from the National Trauma Data Bank,Trauma Quality Improvement Project(TQIP)of *** database is own by the ACS and it is the largest database for trauma quality program in the *** matching analysis was performed for the *** There were 390 pediatric patients with open long bone fractures who were incl-uded in the *** completing propensity score matching,we had 176 patients in each category,irrigation and debridement within 8 hours and irrigation and debridement between 8 hours and 24 *** found no significant differences between each group for the rate of deep SSI which was 0.6%for patients who received surgical irrigation and debridement within 8 hours and 1.1%for those who received it after 8 hours[adjusted odd ratio(AOR):0.5,95%CI:0.268-30.909,P>0.99].For the secondary outcomes studied,in terms of length of hospital stay,patients who received irrigation and debridement within 8 hours stayed for an average of 3.5 days,and those who received it after 8 hours stayed for an average of 3 days,with no significant difference found,and there were also no sig-nificant differences found between the discharge dispositions of the *** Our findings support the recommendation for managing open long bone fractures from the ACS:Complete surgical irrigation and debridement within 24 hours.

GRADE: Deep learning and garlic routing-based secure data sharing framework for IIoT beyond 5G

The rise of automation with Machine-Type Communication(MTC)holds great potential in developing Industrial Internet of Things(IIoT)-based applications such as smart cities,Intelligent Transportation Systems(ITS),supply chains,and smart industries without any human ***,MTC has to cope with significant security challenges due to heterogeneous data,public network connectivity,and inadequate security *** overcome the aforementioned issues,we have proposed a blockchain and garlic-routing-based secure data exchange framework,i.e.,GRADE,which alleviates the security constraints and maintains the stable connection in ***,the Long-Short-Term Memory(LSTM)-based Nadam optimizer efficiently predicts the class label,i.e.,malicious and non-malicious,and forwards the non-malicious data requests of MTC to the Garlic Routing(GR)*** GR network assigns a unique ElGamal encrypted session tag to each machine partaking in ***,an Advanced Encryption Standard(AES)is applied to encrypt the MTC data ***,the InterPlanetary File System(IPFS)-based blockchain is employed to store the machine's session tags,which increases the scalability of the proposed GRADE ***,the proposed framework has utilized the indispensable benefits of the 6G network to enhance the network performance of ***,the proposed GRADE framework is evaluated against different performance metrics such as scalability,packet loss,accuracy,and compromised rate of the MTC data *** results show that the GRADE framework outperforms the baseline methods in terms of accuracy,i.e.,98.9%,compromised rate,i.e.,18.5%,scalability,i.e.,47.2%,and packet loss ratio,i.e.,24.3%.

Application of machine learning technique for predicting and evaluating chloride ingress in concrete

The degradation of concrete structure in the marine environment is often related to chloride-induced corrosion of reinforcement ***,the chloride concentration in concrete is a vital parameter for estimating the corrosion level of reinforcement *** research aims at predicting the chloride content in concrete using three hybrid models of gradient boosting(GB),artificial neural network(ANN),and random forest(RF)in combination with particle swarm optimization(PSO).The input variables for modeling include exposure condition,water/binder ratio(W/B),cement content,silica fume,time exposure,and depth of *** results indicate that three models performed well with high accuracy of prediction(R2⩾0.90).Among three hybrid models,the model using GB_PSO achieved the highest prediction accuracy(R2=0.9551,RMSE=0.0327,and MAE=0.0181).Based on the results of sensitivity analysis using SHapley Additive exPlanation(SHAP)and partial dependence plots 1D(PDP-1D),it was found that the exposure condition and depth of measurement were the two most vital variables affecting the prediction of chloride *** the number of different exposure conditions is larger than two,the exposure significantly impacted the chloride content of concrete because the chloride ion ingress is affected by both chemical and physical *** study provides an insight into the evaluation and prediction of the chloride content of concrete in the marine environment.

目的探讨膳食模式、肠道菌群在胆囊息肉样病变(Polypoid Lesion of Gallbladder,PLG)发生发展中的作用,为PLG的积极防治提供基础依据。方法通过连续便利抽样方式收集2022年1月至2024年1月就诊于锡林郭勒盟中心医院的PLG患者150例及健康对照者150例,分PLG患者150例为病例组,健康对照者150例为对照组。结果病例组患者的专性厌氧菌中奇异菌、双歧杆菌均少于对照组(P<0.05),艰难梭菌多于对照组(P<0.05),兼性厌氧菌中乳酸菌少于对照组(P<0.05),链球菌、肠球菌、葡萄球菌均多于对照组(P<0.05),高密度脂蛋白胆固醇(HDL-C)水平低于对照组(P<0.05),糖化血红蛋白(HbA1c)、空腹血糖(FBG)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、超敏C反应蛋白(hs-CRP)水平均高于对照组(P<0.05)。奇异菌、罗伊氏乳杆菌均与HbA1c呈负相关(P<0.05),艰难梭菌与HbA1c呈正相关(P<0.05),双歧杆菌与TG、LDL-C均呈负相关(P<0.05),葡萄球菌与hs-CRP呈正相关(P<0.05),发酵乳杆菌与LDL-C呈负相关(P<0.05)。结论胆囊息肉患者肠道菌群结构变化与代谢标志物和膳食模式的关系密切。

Focal mechanism and seismogenic structure of the Shiqu MS4.4 earthquake

The focal mechanism solution of the Shiqu MS 4.4 earthquake occurred on May 16th,2017 in Sichuan Province is studied by the gCAP method using the waveform data from the regional seismic networks in Sichuan,Qinghai,Tibet and Gansu *** strike/dip/dipping angle of the first nodal plane are 214/80/167and those of the second nodal plane are 306/77/10,the optimal centroid depth is 7.30.6 km and the moment magnitude is MW ***,the study investigates the robustness of the results against the error of crustal velocity structure,location,data quality and difference of seismic parameters,subsequently obtaining a stable resolved focal *** to the geological structure in the seismogenic area,spatial distribution of aftershock sequenceof the regional tectonic stress field,and the focal mechanism of the main shock,we suggest that the Shiqu earthquake is induced by a left-lateral strike-slip mechanism and the second nodal plane is inferred to be the seismogenic fault,consistent with the geometry of the Changshagongma fault which is the secondary fault of the northwest part of the Xianshuihe fault zone.
