BACKGROUND Chronic cocaine use is associated with stroke, coronary artery disease and myocardial infarction, resulting in severe impairments or sudden mortality. In the absence of clear cardiovascular symptoms, individuals with cocaine use disorder (iCUD) seeking addiction treatment receive mostly psychotherapy and psychiatric pharmacotherapy, with no attention to vascular disease (i.e., atherosclerosis). Little is known about the pre-clinical signs of cardiovascular risk in iCUD and early signs of vascular disease are undetected in this underserved population. AIM To assess inflammation, plaque burden and plaque composition in iCUD aiming to detect markers of atherosclerosis and vascular disease. METHODS The bilateral carotid arteries were imaged with positron emission tomography/magnetic resonance imaging (PET/MRI) in iCUD asymptomatic for cardiovascular disease, healthy controls, and individuals with cardiovascular risk. PET with 18F-fluorodeoxyglucose (18F-FDG) evaluated vascular inflammation and 3-D dark-blood MRI assessed plaque burden including wall area and thickness. Drug use and severity of addiction were assessed with standardized instruments. RESULTS The majority of iCUD and controls had carotid FDG-PET signal greater than 1.6 but lower than 3, indicating the presence of mild to moderate inflammation. However, the MRI measure of wall structure was thicker in iCUD as compared to the controls and cardiovascular risk group, indicating greater carotid plaque burden. iCUD had larger wall area as compared to the healthy controls but not as compared to the cardiovascular risk group, indicating structural wall similarities between the non-control study groups. In iCUD, wall area correlated with greater cocaine withdrawal and craving. CONCLUSION These preliminary results show markers of carotid artery disease burden in cardiovascular disease-asymptomatic iCUD. Cutaneous lesions and disorders in healthy neonates and their relationships with maternal-neonatal factors: a cross-sectional study

Background:Cutaneous lesions are very common in *** a number of studies have reported on their incidence,very little is known about the factors that influence *** set out to investigate a large population of neonates with the aims of achieving an overall picture of neonatal skin manifestations,and examining their relationships with various maternal,neonatal and perinatal ***:This study was conducted on neonates born at the Department of Obstetrics and Gynaecology at the University of Szeged between June 2013 and July 2015.A total of 4658 consecutive infants underwent a whole-body skin examination within the first 72 hours of extrauterine *** official neonatal medical charts were used to collect data on the history of the participating neonates and on maternal ***:74.35% of the neonates exhibited at least one skin *** major diagnosis groups were transient,benign cutaneous lesions;vascular lesions;traumatic,iatrogenic,congenital or acquired disorders with skin injuries;pigmented lesions;and developmental abnormalities or benign skin *** relationships between the skin findings and six neonatal or maternal factors were examined:gender,gestational age and birth weight of the neonates;maternal age and the number of previous pregnancies of the mothers,and mode and circumstances of the ***:We found several significant correlations between the examined maternal/neonatal factors and the occurrence of birthmarks and neonatal skin *** course,further studies are required to confirm and better understand these associations.

Nimodipine(NMD)has gained increasing attention in the latest scientific studies as it can act alone or in combination with other drugs to relieve or prevent symptoms of traumatic brain injury,ischemic stroke,and migraine or even to improve pregnancy *** an acute stroke,an altered cerebral metabolic rate results in a p H change in the aff ected brain ***,the eff ect of p H changes on NMD penetration into the brain tissue has not been investigated *** a few papers reported the determination of NMD concentration in the brain *** usually required a relatively large volume of brain homogenate,and only one of them used mass spectrometric ***,we aimed to develop,validate,and apply our new targeted method for studying the pharmacokinetics of NMD at diff erent p H conditions in acute rat brain tissue as a model of cerebral ***,we present a fully validated targeted ultra-high performance liquid chromatography-tandem mass spectrometry(UHPLC-MS/MS)method with a five-minute total run time for the quantitation of NMD in rat brain tissue using a low quantity of brain homogenate.A matrix match external calibration procedure was applied for quantitative measurement using a stable isotope-labeled NMD internal *** new sample preparation procedure resulted in high recoveries(96.9–110.6%)in brain homogenate with negligible matrix eff ect(-9–12.9%).The lower limit of detection of NMD was 15 ng/g brain(547 fg of NMD injected into the column).In acute rat brain tissue,the maximum median concentrations of NMD(c_(max):31.93μg/g(pH 7.38),46.16μg/g(pH 6.50),30.89μg/g(pH 7.57)were reached at 50 min(t_(max))).

Ultrasound-assisted extraction of bound phenolic compounds from the residue of Apocynum venetum tea and their antioxidant activities

Bound phenolic compounds from the residue of Apocynum venetum(***)tea were released by alkaline digestion with sonication,followed by extraction using ethyl acetate(EtOAc).The fragment ions of compounds in free and bound forms were identified by liquid chromatography electrospray ionization tandem mass spectrometry(LC-ESI-MS/MS)in both positive and negative *** investigation of chemical constituents in the EtOAc extract led to the isolation of eleven individual compounds,and their chemical structures were identified as(−)-epicatechin(1),(−)-loliolide(2),(−)-syringaresinol-4-O-β-D-glucopyranoside(3),alloside of benzyl alcohol(4),3-[(6-O-hexopyranosylhexopyranosyl)oxy]-2-(palmitoyloxy)propyl(9z,12z,15z)-9,12,15-octadecatrienoate(5),apocynoside I(6),quercetin-3-O-β-D-glucopyranoside(7),kaempferol-3-O-β-D-glucopyranoside(8),[1-acetyloxy-3-[3,4,5-trihydroxy-6-[[3,4,5-trihydroxy-6-(hydroxymethyl)oxan-2-yl]oxymethyl]oxan-2-yl]oxypropan-2-yl]hexadecanoate(9),1-stearoyl-2-palmitoyl-sn-glycerol(10),and 3-hexene-l-O-β-D-glucoside(11)using a series of spectroscopic *** 2–5 and 9–11 were isolated for the first time from ***,while antioxidant experiments showed that compounds 1,7,and 8 displayed strong DPPH and ABTS+radical scavenging *** study represents the first investigation of bound phenolic compounds with potential health benefits in the residue of *** tea.

Correlation between the Changes in Lung Function and Lung Density Changes in Patients Following Radio- (Chemo-) Therapy for Thoracic Carcinomas

Purpose: In this analysis we focused on the correlation of patients’ lung function (PFT) data and lung density changes (ΔHU) detected in follow-up CTs. Material and Methods: PFT and lung function data were available for 58 patients 12 weeks and 47 patients 6 months after radio- (chemo-) therapy for thoracic carcinomas (NSCLC, SCLC and esophageal carcinoma). The follow-up CT scans were matched with the planning CT scans of each patient and then subtracted to calculate ΔHU for each voxel using customized research software. PFT data regarding e.g. vital capacity (VC), total lung capacity (TLC) and diffusion capacity for carbon monoxide (DLCO) were collected before and at several follow-up appointments after treatment. Results: 12 weeks after therapy there was a statistically significant correlation between difference in DLCO and the maximum ΔHU as well as the difference in TLC and the minimum ΔHU. 6 months after treatment there was a significant correlation between the difference in VC and DLCO with numerous lung density parameters, e.g. the mean and median lung density changes and the 75th percentile of ΔHU. There was no significant correlation between the PFT parameters FEV1, pCO2 and pO2 and any lung density parameter at any follow-up appointment. Conclusion: There is a significant correlation between DLCO and ΔHU 6 months after treatment that most likely reflects the underlying pathological mechanisms in terms of the development of fibrotic lung tissue after RT. The relevance of the significant correlations 12 weeks after RT is questionable.

A Pilot Experience in the training of healthcare professionals to face the childhood obesity epidemic through family therapeutic education

Background: Obesity is a high-cost social disease, the management of which, usually assigned to general practice, is less supported by evidence-based medicine. Most general practitioners feel unprepared to face the problem. Objectives: In 2000, at the Pediatric Department of Ferrara, we started a Family Group Therapeutic Education Program for the treatment of childhood obesity. More than 300 families have attended the program. Following our positive results, we created a professional training course on therapeutic education, addressed to healthcare personnel involved in the treatment of childhood obesity. The purpose of the present study is to discover if it is possible to promote the development of a shared therapeutic education strategy for obesity, involving primary care physiccians/pediatricians, specialists and dietitians. Methods: The integrated professional training course consists of a four-day seminar along with an on-line course for distance learning. The assessment of the project takes into account the following criteria: a multidimensional questionnaire (a pre- and post-test which explores: knowledge, motivation, self-efficacy, methodology, communication, modeling, etc.), an appreciation questionnaire with responses collected over the course of 2 years. Moreover, the nine participants of our 2008 training course began to use the new therapeutic tools in their practice and six of them sent us the anthropometric measurements of patients whom they have treated over the past two years using this approach allows us to present outcomes in terms of implementation of this therapeutic education program in everyday professional practice. Results and Conclusions: The course was attended by 15 healthcare professionals: ten in 2008 and five in 2009, from different Italian centers. Results seem to indicate a great interest and appreciation by the participants. Results based on BMI z-score reduction of the children cared for by our trainees seem to support the efficacy
