Cachexia is a multifactorial syndrome related to unintentional weight loss and to loss of muscle and fat mass. In head and neck cancer (HNC) its incidence is important and not only related to a deficient intake of food due to the impact of the disease in the vital functions. A complex disturbance in the normal metabolism of the patient promotes a persistent inflammatory state and a shifting in the metabolism balance toward a catabolic predominance affecting primarily the skeletal muscle. This leads to severe impairment of the functional, emotional and social status and quality of life of the patients that will compromise response to treatment and the disease prognosis. Understanding this deleterious syndrome and mainly identifying it in early stages of the disease is of a major importance in achieving better outcomes to head and neck cancer patients. This study pretends to identify clinical aspects related to cachexia in HNC in a clinical perspective for application on the routine clinical practice. In our study, 30 HNC patients were enrolled and evaluated in terms of nutritional values (actual and loss of weight in the past 6 months, body mass index (BMI), nutritional risk index (NRI), malnutrition universal screening tool), serum biochemical markers (albumin, total proteins, cholesterol, triglycerides, urea, C-reactive protein (CRP), interleukin-6 (IL-6), tumour necrosis factor-α (TNF-α) and myostatin) and health related quality of life (HRQoL) evaluation (using European Organisation for the Research and Treatment of Cancer (EORTC) quality of life questionnaires (QLQ): EORTC QLQ-C30 and EORTC QLQ-HN43). A minimum follow-up of 48 months was considered for all patients. Our results showed that NRI is a good and sensitive index to identify cachexia. This index uses two parameters, one constitutional (loss of weight) and one biochemical (level of serum albumin). This paper aims to compare the results of two techniques of Kriging (Ordinary Kriging and Indicator Kriging) that are applied to estimate the Private Motorized (PM) travel mode use (car or motorcycle) in several geographical coordinates of non-sampled values of the concerning variable. The data used was from the Origin/Destination and Public Transportation Opinion Survey, carried out in 2007/2008 at S?o Carlos (SP, Brazil). The techniques were applied in the region with 110 sample points (households). Initially, Decision Tree was applied to estimate the probability of mode choice in surveyed households, thus determining the numeric variable to be used in Ordinary Kriging. For application of Indicator Kriging it was used the variable “main travel mode” in a discrete manner, where “1” represented the use of PM travel mode and “0” characterized others travel modes. The results obtained by the two spatial estimation techniques were similar (Kriging maps and cross-validation procedure). However, the Indicator Kriging (KI) obtained the highest number of hit rates. In addition, with the KI it was possible to use the variable in its original form, avoiding error propagation. Finally, it was concluded that spatial statistics was thriving in travel demand forecasting issues, giving rise, for the both Kriging methods, to a travel mode choice surface on a confirmatory way.

Background: Spondylodiscitis and spinal epidural abscesses are rare pathologic entities, but increasing in incidence. Group G beta hemolytic Streptococcus has been recently described associated with human infections. They often present clinically in a non-specific fashion, a fact which can lead to diagnostic delay, with serious consequences for the patient. Case Report: An 80-year-old man was admitted to the hospital with complaints of fever for three days, dysuria, hematuria, and back pain. Both septic embolizations and spondylodiscitis due to Group G beta hemolytic Streptococcus were detected. The patient was successfully treated with intravenous penicillin G for eight weeks, followed by oral amoxicillin for five months. Discussion: In all patients with spondylodiscitis, infective endocarditis should be considered, particularly in patients with heart valve disease history, since spondylodiscitis may be the presenting sign of an infective endocarditis. A high level of suspicion is therefore necessary in order correctly diagnose such entities as quickly as possible. The present case illustrates the pathogenic potential of group G streptococci in spondylodiscitis and native valve endocarditis.

Re-bleeding events in patients with obscure gastrointestinal bleeding after negative capsule endoscopy

AIM: To investigate long-term re-bleeding events after a negative capsule endoscopy in patients with obscure gastrointestinal bleeding(OGIB) and the risk factors associated with the ***: Patients referred to Hospital Egas Moniz(Lisboa, Portugal) between January 2006 and October 2012 with OGIB and a negative capsule endoscopy were retrospectively analyzed. The following study variables were included: demographic data, comorbidities, bleeding-related drug use, hemoglobin level, indication for capsule endoscopy, post procedure details, work-up and follow-up. Re-bleeding rates and associated factors were assessed using a Cox proportional hazard analysis. The Kaplan-Meier method was used to estimate the cumulative incidence of re-bleeding at 1, 3 and 5 years, and the differences between factors were ***: The study population consisted of 640 patients referred for OGIB investigation. Wireless capsule endoscopy was deemed negative in 113 patients(17.7%). A total of 64.6% of the population was female, and the median age was 69 years. The median follow-up was forty-eight months(interquartile range 24-60). Re-bleeding occurred in 27.4% of the cases. The median time to re-bleeding was fifteen months(interquartile range 2-33). In 22.6%(n = 7) of the population, small-bowel angiodysplasia was identified as the culprit lesion. A univariate analysis showed that age > 65 years old, chronic kidney disease, aortic stenosis, anticoagulant use and overt OGIB were risk factors for re-bleeding; however, on a multivariate analysis, there were no risk factors for re-bleeding. The cumulative risk of re-bleeding at 1, 3 and 5 years of follow-up was 12.9%, 25.6% and 31.5%, *** who presented with overt OGIB tended to rebleed sooner(median time for re-bleeding: 8.5 mo vs 22 mo).CONCLUSION: Patients with OGIB despite a negative capsule endoscopy have a significant re-bleeding risk; therefore, these patients require an extended follow-up strategy.

This research aimed to quantify concentrations of ammonia(NH3),carbon dioxide(CO_(2))and methane(CH_(4)),estimate emissions,and analyze the factors influencing them during warm periods in an open dairy barn equipped with two cooling systems in a Mediterranean climate *** distribution within the barn was observed to vary both vertically and horizontally,with the highest gas concentrations observed in the central area of the ***_(3),CH_(4)and CO_(2)ranged in 1.7–7.4,7–18,560–724μg·g^(–1),*** ventilation through openings and the operation of cooling systems induced changes in indoor microclimate conditions,influencing cow behavior and,consequently,gas *** concentrations were the highest at air velocities below 0.5 m·s^(–1).The highest concentration of NH_(3)was observed when the temperature-humidity index(THI)was>72 and≤78;and CO_(2)and CH_(4)concentrations were the highest with THI≥72 and decreased with THI≤***_(3)concentrations when barn management included three daily milkings were higher than those measured when barn management was based on two daily milkings,and lower for CH_(4)and CO_(2).NH_(3)and CH_(4)emissions were the highest during barn cleaning,while the lowest NH_(3)emissions occurred during activity of the cows(i.e.,feeding,walking).

Discrepancies between the responses to skin prick test to food and respiratory antigens in two subtypes of patients with irritable bowel syndrome

AIM:To compare the response to skin prick tests (SPTs) to food antigens (FAs) and inhalant allergens (IAs) in patients with two subtypes of irritable bowel syndrome (IBS) and healthy controls. METHODS:We compared the results of SPTs for IAs and FAs in 87 volunteers divided into three groups:diarrhea predominant IBS (D-IBS) GroupⅠ(n = 19), constipation predominant IBS (C-IBS) Group Ⅱ (n = 17), and normal controls Group Ⅲ (n = 51). RESULTS:Of the 285 tests (171 for FAs and 114 for IAs) performed in GroupⅠwe obtained 45 (26.3%) positive responses for FA and 23 (20.1%) for IA. Of the 153 tests for FA in Group Ⅱ, we obtained 66 (20.1%) positive responses, and of the 102 tests for IA, we obtained 20 (19.6%) positive responses. Of the 459 tests for FA performed in Group Ⅲ, we obtained 39 (84%) positive responses, and of the 306 for IA, we obtained 52 (16.9%) positive responses. The numbers of positive responses were not significantly different between the three groups, but in the D-IBS group, the number of SPTFA responses differed significantly from those for the other two groups (P < 0.01). CONCLUSION:Despite the small number of cases studied, the higher reactivity to FAs in GroupⅠcompared to Groups Ⅱ and Ⅲ adds new information, and suggests the presence of a possible alteration in intestinal epithelial function.

Development of synthetic bone substitutes has arisen as a major research interest in the need to find an alternative to autologous bone *** an ovine model,the present pre-clinical study presents a synthetic bone graft(BonelikeVR)in combination with a cellular system as an alternative for the regeneration of non-critical *** association of biomaterials and cell-based therapies is a promising strategy for bone tissue *** stem cells(MSCs)from human dental pulp have demonstrated both in vitro and in vivo to interact with diverse biomaterial systems and promote mineral deposition,aiming at the reconstruction of osseous ***,these cells can be found and isolated from many ***-critical bone defects were treated with BonelikeVR with or without MSCs obtained from the human dental *** showed that BonelikeVR and MSCs treated defects showed improved bone regeneration compared with the defects treated with BonelikeVR ***,it was observed that the biomaterial matrix was reabsorbed and gradually replaced by new bone during the healing *** therefore propose this combination as an efficient binomial strategy that promotes bone growth and vascularization in non-critical bone defects.

Mammals can show conspicuous behavioral responses to thermal variation,including changes in movement *** used an integrative approach to understand how environmental temperature can drive the movement behavior of a mammal with low capacity for physiological thermoregulation,the giant anteater(Myrmecophaga tridactyla).We tracked 52 giant anteaters in 7 areas throughout the Brazilian *** estimated the distance moved,area used,use of forest areas,and mean environmental temperature for each monitoring day of each *** modeled these data with Mixed Structural Equations—considering the possible interactions between our variables and controlling for sex and body *** anteaters reduced displacement and increased forest use with decreasing environmental temperature,probably because of their low body heat *** is possible that they reduce distance moved and area used by reducing the duration of *** decreasing temperature,forest habitats become warmer than open ones,besides buffer rain and chilly *** displacement and using forests are important strategies to reduce body heat loss and the energetic costs of ***,decreasing movement can limit food access and,consequently,***,we highlight the importance of forests as thermal *** increasing frequency and intensity of extreme weather events,we showed the need to preserve forest patches to offer suitable conditions for tropical mammals’behavioral *** this context,policies favoring deforestation on Brazilian territory are especially ***,we emphasize the need of integrative approaches to understand the complex interactions between organisms and the environment.
