Cachexia is a multifactorial syndrome related to unintentional weight loss and to loss of muscle and fat mass. In head and neck cancer (HNC) its incidence is important and not only related to a deficient intake of food due to the impact of the disease in the vital functions. A complex disturbance in the normal metabolism of the patient promotes a persistent inflammatory state and a shifting in the metabolism balance toward a catabolic predominance affecting primarily the skeletal muscle. This leads to severe impairment of the functional, emotional and social status and quality of life of the patients that will compromise response to treatment and the disease prognosis. Understanding this deleterious syndrome and mainly identifying it in early stages of the disease is of a major importance in achieving better outcomes to head and neck cancer patients. This study pretends to identify clinical aspects related to cachexia in HNC in a clinical perspective for application on the routine clinical practice. In our study, 30 HNC patients were enrolled and evaluated in terms of nutritional values (actual and loss of weight in the past 6 months, body mass index (BMI), nutritional risk index (NRI), malnutrition universal screening tool), serum biochemical markers (albumin, total proteins, cholesterol, triglycerides, urea, C-reactive protein (CRP), interleukin-6 (IL-6), tumour necrosis factor-α (TNF-α) and myostatin) and health related quality of life (HRQoL) evaluation (using European Organisation for the Research and Treatment of Cancer (EORTC) quality of life questionnaires (QLQ): EORTC QLQ-C30 and EORTC QLQ-HN43). A minimum follow-up of 48 months was considered for all patients. Our results showed that NRI is a good and sensitive index to identify cachexia. This index uses two parameters, one constitutional (loss of weight) and one biochemical (level of serum albumin). The spatiotemporal variability of the greenhouse gas methane （CH4） in the atmosphere over the Amazon is studied using data from the space-borne measurements of the Atmospheric Infrared Sounder on board NASA＇s AQUA satellite for the period 2003-12. The results show a pronounced variability of this gas over the Amazon Basin lowlands region, where wetland areas occur. CH4 has a well-defined seasonal behavior, with a progressive increase of its concentration during the dry season, followed by a decrease during the wet season. Concerning this variability, the present study indicates the important role of ENSO in modulating the variability of CH4 emissions over the northern Amazon, where this association seems to be mostly linked to changes in flooded areas in response to ENSO-related precipitation changes. In this region, a CH4 decrease （increase） is due to the E1 Nifio-related （La Nifia-related） dryness （wetness）. On the other hand, an increase （decrease） in the biomass burning over the southeastern Amazon during very dry （wet） years explains the increase （decrease） in CH4 emissions in this region. The present analysis identifies the two main areas of the Amazon, its northern and southeastern sectors, with remarkable interannual variations of CH4. This result might be useful for future monitoring of the variations in the concentration of CH4, the second-most important greenhouse gas, in this area.

Background: Spondylodiscitis and spinal epidural abscesses are rare pathologic entities, but increasing in incidence. Group G beta hemolytic Streptococcus has been recently described associated with human infections. They often present clinically in a non-specific fashion, a fact which can lead to diagnostic delay, with serious consequences for the patient. Case Report: An 80-year-old man was admitted to the hospital with complaints of fever for three days, dysuria, hematuria, and back pain. Both septic embolizations and spondylodiscitis due to Group G beta hemolytic Streptococcus were detected. The patient was successfully treated with intravenous penicillin G for eight weeks, followed by oral amoxicillin for five months. Discussion: In all patients with spondylodiscitis, infective endocarditis should be considered, particularly in patients with heart valve disease history, since spondylodiscitis may be the presenting sign of an infective endocarditis. A high level of suspicion is therefore necessary in order correctly diagnose such entities as quickly as possible. The present case illustrates the pathogenic potential of group G streptococci in spondylodiscitis and native valve endocarditis.

Loggerheads are distributed by ten Regional Management Units (RMUs) worldwide. The Atlantic Ocean houses three of these: the Northwest, Southwest and Northeast RMUs. The most studied is, so far, the Northwest RMU, but the other two have focused attention of researchers. In contrast, marine turtles from the African Atlantic region (Southeast) belong to a complex but little-known region. What is their role in the Atlantic? Are these RMUs connected? To understand these questions, research was made of reports and publications concerning the Northeast Atlantic (NEA) RMU. The asymmetry in information and available knowledge about NEA is high when compared to other RMUs. This demonstrates that there is still a lack of conservation programs besides Cape Verde, and that there is difficulty in transforming data (nesting ecology, molecular, telemetry, etc.) into publications. However, this issue is evident for other marine turtle species of East African Atlantic. There is a need for better scientific support, to enable local conservation programs to deliver data in reports, and even scientific publications. There are so many aspects of loggerhead’s life cycle that will only be unravelled by more and better supported studies. This is a paper that, by analysing the available information of the NEA loggerhead RMU, reflects the following steps to address in loggerhead conservation for the African Atlantic coast.

Antithrombotic treatment in chronic heart failure and sinus rhythm: Systematic review

AIM: To assess the efficacy and safety of antithrombotic drugs(antiplatelet or anticoagulant drugs) compared to no antithrombotic treatment or placebo in patients with heart failure(HF) and sinus rhythm. METHODS: We searched Medline and Cochrane Library for randomized controlled trials evaluating antithrombotic treatment and no antithrombotic treatment in patients with HF and sinus rhythm. Risk ratio(RR) and 95%CIs were estimated performing meta-analysis with random effects method. RESULTS: Two studies met the inclusion criteria: Heart failure Long-term Antithrombotic Study and Warfarin/Aspirin Study in Heart failure, with 336 patients and mean follow-up 1.8-2.25 years. Stroke risk was not reduced by acetylsalicylic acid(RR = 1.18, 95%CI: 0.17-8.15), oral anticoagulation(RR = 0.30, 95%CI: 0.03-2.65) or overall antithrombotic drugs(RR = 0.52, 95%CI: 0.10-2.74). Acetylsalicylic acid showed a significant increased risk of worsening HF(RR = 1.78, 95%CI: 1.08-2.92), while oral anticoagulation had no impact in this outcome(RR = 1.03, 95%CI: 0.61-1.75). Overall antithrombotic drugs showed a significant risk increase of major bleeding(RR = 6.99, 95%CI: 0.89-54.64). CONCLUSION: Best available evidence does not support the routine use of antithrombotic drugs in patients with HF and sinus rhythm. These drugs, particularly oral anti-coagulation has the hazard of increase significantly major bleeding risk.
