BACKGROUND Esophageal submucosal hematoma is a rare *** the exact etiology remains uncertain,vessel fragility with external factors is believed to have led to submucosal bleeding and hematoma formation;the vessel was ruptured by a sudden increase in pressure due to nausea,and the hematoma was enlarged by antiplatelet or anticoagulant *** conditions are rare,with a better *** present the first known case of submucosal esophageal hematoma-subsequent hemorrhagic shock due to Mallory-Weiss *** SUMMARY A 73-year-old female underwent endovascular treatment for an unruptured cerebral *** patient received aspirin and clopidogrel before surgery and heparin during surgery,and was well during the *** hours after returning to the ICU,she complained of chest discomfort,vomited 500 m L of fresh blood,and entered hemorrhagic *** submucosal hematoma with Mallory-Weiss syndrome was diagnosed through an endoscopic examination and computed *** addition to a massive fluid and erythrocyte transfusion,we performed a temporary compression for hemostasis with a Sengstaken-Blakemore(S-B)***,she became hemodynamically *** postoperative day 1,we performed an upper gastrointestinal endoscopy and confirmed no expansion of the hematoma nor any recurring bleeding;therefore,we removed the S-B tube and clipped the gastric mucosal laceration at the esophagogastric *** started oral intake on postoperative day *** patient made steady progress,and was discharged on postoperative day *** We present the first known case of submucosal esophageal hematoma subsequent hemorrhagic shock due to Mallory-Weiss syndrome.

A Carbon Monoxide Sensing Film Based on Hemoglobin Allostery

Carbon monoxide (CO) is a gaseous mediator, which is generated via anenzymatic reaction of heme oxygenase, and it plays physiological roles to regulate cellular respiration and blood flow in the liver. The concentration and distribution of CO molecules in the living body is unknown owing to a lack of a suitable technique for measuring them in vivo. A needle-type CO sensor has been used for bioinstrumentation, but it is inappropriate for implantation in vivo and long-term monitoring. We developed a CO sensor sheet based on hemoglobin (Hb) allostery, as Hb undergoes a conformational change on CO binding. Hb was extracted from mice blood and mixed with agarose gel with a reducer to stabilize deoxy-Hb in the gel. CO-releasing molecules (CORM) were used to mimic CO-generating tissue, and the sensitivity of the Hb gel could be regulated by Hb concentration. We defined the CO–Hb index, an absorbance ratio at 539 and 557 nm, to estimate the accumulation of captured CO in the gel. It correlatively increased with CORM dose, indicating that gel-embedded Hb underwent a conformational change on CO binding, thereby acting as a CO sensor. We subsequently used the Hb-sensor sheet for two-dimensional imaging of CO distribution. CORM-containing gels with different sizes and doses were layered on this sheet. Size- and dose-dependent CO distribution was visualized by scanning the CO-Hb index in the sheet. Our Hb-based CO sensor sheet is composed of biocompatible materials and can be applied to detect low-level CO sources in the living body.

Plasma-Assisted Chemical Vapor Deposition of Titanium Oxide Layer at Room-Temperature

Plasma-assisted chemical vapor deposition (PCVD) at pressure as low as 3 mtorr using titanium-tetra-isopropoxide (TTIP) and oxygen mixed gas plasma generated by 13.56 MHz radio frequency power (RF-power) below 70 W were applied to deposit titanium-oxide layer at temperature under 40°C. Plasma optical emission spectroscopy and FTIR indicated that density of OH group in the amorphous layer was related to the density of OH or H2O in the plasma and the species was formed on electrode to induce the RF-power. Hydrophilicity on the layer was dependent on the density of chemisorbed OH, but was degraded by the excess OH. The PCVD-TiOx coating was demonstrated on polyethylene terephthalate and showed good hydrophilic property with the contact angle of water about 5°.

Endoscopic submucosal dissection of a rectal carcinoid tumor using grasping type scissors forceps

Endoscopic submucosal dissection （ESD） with a knife is a technically demanding procedure a ssociated with ahigh complication rate. The shortcomings of this meth-od are the inability to fix the knife to the target lesion,and compression of the lesion. These can lead to major complications such as perforation and bleeding. To reduce the risk of complications related to ESD, we developed a new grasping type scissors forceps （GSF）,which can grasp and incise the targeted tissue using electrosurgical current. Colonoscopy on a 55-year-old woman revealed a 10-ram rectal submucosal *** histological diagnosis of the specimen obtained by biopsy was carcinoid tumor. Endoscopic ultrasonog-raphy demonstrated a hypoechoic solid tumor limitedto the submucosa without lymph node involvement. Itwas safely and accurately resected without unexpectedincision by ESD using a GSF. No delayed hemorrhage or perforation occurred. Histological examination confirmed the carcinoid tumor was completely excisedwith negative resection margin.

BACKGROUND Diffuse large B-cell lymphoma(DLBCL)is the most common type of malignant lymphoma(ML),accounting for 30%-40%of cases of non-Hodgkin’s lymphoma(NHL)in *** paranasal sinus lymphoma is a rare presentation of extranodal NHL that accounts for only 0.17%of all *** from the maxillary sinus(MS)is a particularly rare presentation,and is thus often difficult to *** have reported the first known case of DLBCL originating from the MS with rapidly occurrent multiple skin *** SUMMARY An 81-year-old Japanese man visited our hospital due to continuous pain for 12 d in the left maxillary nerve *** medical history included splenectomy due to a traffic injury,an old right cerebral infarction from when he was 74-years-old,hypertension,and type 2 diabetes mellitus.A plain head computed tomography(CT)scan revealed a 3 cm×3.1 cm×3 cm sized left *** day 25,left diplopia and ptosis occurred,and a follow-up CT on day 31 revealed the growth of the left MS *** on an MS biopsy on day 50,we established a definitive diagnosis of DLBCL,non-germinal center B-cell-like originating from the left *** patient was admitted on day 62 due to rapid deterioration of his condition,and a plain CT scan revealed the further growth of the left MS mass,as well as multiple systemic metastasis,including of the skin.A skin biopsy on day 70 was found to be the same as that of the left MS *** notified the patient and his family of the disease,and they opted for palliative care,considering on his condition and *** patient died on day *** This case suggests the need for careful,detailed examination,and for careful follow-up,when encountering patients presenting with a mass.

BACKGROUND Dextromethorphan is a prevalent antitussive agent that can be easily obtained as an over-the-counter *** has been a growing number of reported cases of toxicity in recent ***,there are numerous instances of mild symptoms,with only a limited number of reports of severe cases necessitating intensive *** presented the case of a female who ingested 111 tablets of dextromethorphan,leading to shock and convulsions and requiring intensive care that ultimately saved her *** SUMMARY A 19-year-old female was admitted to our hospital via ambulance,having overdosed on 111 tablets of dextromethorphan(15 mg)obtained through an online importer in a suicide *** patient had a history of drug abuse and multiple self-inflicted *** the time of admission,she exhibited symptoms of shock and altered ***,upon arrival at the hospital,the patient experienced recurrent generalized clonic convulsions and status epilepticus,necessitating tracheal *** convulsions were determined to have been caused by decreased cerebral perfusion pressure secondary to shock,and noradrenaline was administered as a *** lavage and activated charcoal were also administered after *** systemic management in the intensive care unit,the patient’s condition stabilized,and the need for vasopressors *** patient regained consciousness and was *** patient was subsequently transferred to a psychiatric facility,as suicidal ideation *** We report the first case of shock caused by an overdose of dextromethorphan.

A Conservative Approach to Treatment of Dental Erosion: Case Report

Tooth surface loss is a common condition with nearly three-quarter of the population in Trinidad & Tobago exhibiting signs and about one-fifth of those persons have moderate or severe wear. These patients become difficult to treat due mainly to loss of clinical crown height and also loss of occlusal vertical dimension. This case depicts the use of a conservative approach to treating a patient with moderate tooth surface loss, as assessed via the tooth wear index, with aetiology of acid erosion. The poor appearance of the dentition was the patient’s chief presenting complaint. With the use of 3M Filtek? Z 250 Universal Restorative Composite (St Paul, MN, USA), temporary posterior build-ups were placed to increase the patient’s Occlusal Vertical Dimension (OVD) and were subsequently replaced with metal onlays. Anterior composite veneers were placed to achieve an aesthetically pleasing result at the increased OVD. The combined use of composite and metal onlays allowed for an economical and more conservative treatment modality for dental erosion rather than full coverage posterior and anterior crowns.

Counter-Rotating Type Pump-Turbine Unit Stabilizing Momentarily Fluctuating Power from Renewable Energy Resources

It is difficult for renewable energy resources to provide constant power with excellent quality for the grid system. This serial research proposes a power stabilization system with a pumped storage to guarantee power quality and capacity, while the outputs from the energy resources are at unstable and/or fluctuating conditions. The power stabilization system with a counter-rotating type pump-turbine unit was prepared and operated at the pumping and the turbine modes. The unit composed of the tandem impellers/runners connected to the inner and the outer armatures of the unique motor/generator. The experiments have verified that this type pump-turbine unit is reasonably effective to stabilize momentarily/instantaneously the fluctuating power from the renewable energy resources.
