The patient, a 77-year-old male, underwent right middle lobectomy for adenocarcinoma of the lung, pT1aN0M0, in November 2007. In November 2008, chest CT revealed two ground-glass opacities (GGOs) in the right lower lobe. In October 2009, both of these GGOs had increased in size, and three new GGOs were found. In July 2011, all of the five GGOs had increased in size and three new GGOs were found yet again. Right lower lobe S6 segmentectomy was performed on September 6, 2011, and histopathological examination revealed eight pulmonary adenocarcinomas in the right S6;all of them classified as pl0, ly0, v0, pT1aN0M0. Among the eight lesions, the doubling times of five were measured during the follow-up course, and the mean doubling time was 402 days. The mean doubling time of the lesions showing high c-erbB2 expression was significantly lesser than that of the lesions showing low c-erbB2 expression (273 days vs. 488 days, p = 0.047). Despite being localized GGOs that had arisen in the same individual, it should be noted that the growth rate of the GGO lesions may vary according to the expression level of a molecular markers, and some GGO lesions may show rapid increase in size.

A 67-year-old man was referred for further evaluation of an abnormal chest roentgenogram. Computed tomography showed a 40 × 30 mm mass in the left upper lobe. A giant bulla occupying about two-thirds of the right thorax was found compressing the adjacent lung parenchyma, shifting the mediastinum to the left. The mass was a primary lung cancer, clinical T2aN0M0, stage IB. Preoperative respiratory function evaluation showed poor pulmonary function, with a forced expiratory volume in 1 second of 1070 ml (29.2% of predicted). Therefore, we first performed giant bullectomy by video-assisted thoracoscopic surgery. At 1 month after this operation, improvement of the forced expiratory volume in 1 second significantly to 2140 ml (80.1% of predicted) was observed. Therefore, we performed resection for the tumor. He was discharged after an uneventful postoperative course, and has remained in good condition for 6 months after the operation.

Amebic liver abscesses(ALAs)are the most commonly encountered extraintestinal manifestation of human invasive amebiasis,which results from Entamoeba histolytica(***)spreading *** can be complicated by liver abscess in 9%of cases,and ALAs led to almost 50000 fatalities worldwide in *** there have been fewer and fewer cases in the past several years,ALAs remain an important public health problem in endemic *** causes both amebic colitis and liver abscess by breaching the host’s innate defenses and invading the intestinal *** often enter the circulatory system,where they are filtered in the liver and produce abscesses,and develop into severe invasive diseases such as *** clinical presentation can appear to be colitis,including upper-right abdominal pain accompanied by a fever in ALA *** diagnosis requires nonspecific liver imaging as well as detecting *** antibodies;however,these antibodies cannot be used to distinguish between a previous infection and an acute ***,diagnostics primarily aim to use PCR or enzyme-linked immunosorbent assay to detect *** can be treated medically,and percutaneous catheter drainage is only necessary in approximately 15%of *** indicated treatment is to administer an amebicidal drug(such as tinidazole or metronidazole)and paromomycin or other luminal cysticidal agent for clinical *** is good with almost universal *** which diagnostic methods are most efficacious will necessitate further analysis of similar clinical cases.

Are we riding the fourth wave of democracy,as predicted by Toffler(1984)and Huntington(1991),a post-capitalist,post-global wave?Western liberal democracy is currently responding to the dramatic change in the economic system after capitalism,the coronavirus pandemic,and the growing power of nationalist and fundamentalist *** this process indicates a change in Western democracy or is it the end of democracy and what is the role of civic education within it?In Israel,as in other Western countries,the nationalist and fundamentalist ideologies(gained strength,undermining liberalism)threaten the conception of global citizenship and strive to shape a model of an illiberal *** Israel,contrary to other Western countries,they also threaten civic equality and the very existence of the democratic *** the absence of a constitution and consensus,Israel is experiencing culture and religious war over values,identity and the concept of“citizenship”as a legal,political,and social ***,the struggle over civic education reflects the struggle over educational hegemony,as well as over national *** this process,nationalists and fundamentalists initiate anti-democratic legislation,violate human rights,obstruct government supervisory mechanisms,and expand ideological education in order to change Israel‟s character to a Jewish state rather than a Jewish and democratic state.

BACKGROUND Dextromethorphan is a prevalent antitussive agent that can be easily obtained as an over-the-counter *** has been a growing number of reported cases of toxicity in recent ***,there are numerous instances of mild symptoms,with only a limited number of reports of severe cases necessitating intensive *** presented the case of a female who ingested 111 tablets of dextromethorphan,leading to shock and convulsions and requiring intensive care that ultimately saved her *** SUMMARY A 19-year-old female was admitted to our hospital via ambulance,having overdosed on 111 tablets of dextromethorphan(15 mg)obtained through an online importer in a suicide *** patient had a history of drug abuse and multiple self-inflicted *** the time of admission,she exhibited symptoms of shock and altered ***,upon arrival at the hospital,the patient experienced recurrent generalized clonic convulsions and status epilepticus,necessitating tracheal *** convulsions were determined to have been caused by decreased cerebral perfusion pressure secondary to shock,and noradrenaline was administered as a *** lavage and activated charcoal were also administered after *** systemic management in the intensive care unit,the patient’s condition stabilized,and the need for vasopressors *** patient regained consciousness and was *** patient was subsequently transferred to a psychiatric facility,as suicidal ideation *** We report the first case of shock caused by an overdose of dextromethorphan.

Here we report imaging studies of a patient with pulmonary benign metastasizing leiomyoma (BML). A 44-year-old woman who underwent a hysterectomy for uterine cellular leiomyoma presented with abnormal shadows on a chest X-ray. Chest computed tomography (CT) revealed multiple well-defined nodules in both lungs. Chest magnetic resonance imaging (MRI) indicated these nodules as T1-low/T2-high intensity lesions. Contrast-enhanced MRI indicated these nodules as well-enhanced lesions, while 18F-fluorodeoxyglucose positron emission tomography-CT revealed no abnormal accumulation in these nodules. Bilateral lung wedge resections were performed for the largest 2 lesions to confirm the diagnosis, and both nodules were histologically diagnosed as BML.

动力分析过程中输入地震波的选择非常重要,通过扰动自然波生成人工地震波是丰富设计地震波池的方法之一。提出使用复连续小波变换来修改、扰动自然波的方法来拟合人工地震波,以2000年日本鸟取M W6.7地震为研究对象,对比Shannon小波基和Gabor小波基在生成人工波过程中的特点。结果表明:使用改进方法可以在任意指定的时-频域上修改地震波的相位谱从而产生人工波,且Shannon小波对地震波能量的保留优于Gabor小波,故Shannon小波基是更好的选择。
