An Unusual Case of Herpes Simplex Meningitis after Inadvertent Puncture of the Subarachnoid Space

The study seeks to report the clinical evolution of a patient who suffered an inadvertent subarachnoid puncture and evolved with a herpetic viral meningitis, confirmed by cerebrospinal fluid (CSF) analysis. Female, 48 years old, submitted to epidural anesthesia, for surgery, with median puncture, between L2-L3 and inadvertently accessed the subarachnoid space, with clear cerebrospinal fluid. The same puncture was used and the solution with 15 mg hyperbaric bupivacaine and 80 mcg morphine was administered. There was no complication during the entire surgical procedure. On the first postoperative day, she started complaining of headache and nausea, being treated with dipyrone, non-steroidal anti-inflammatory drugs (NSAIDs) and ondansetron. On the third day, she presented two episodes of seizure that ceased with the use of diazepam 10 mg. On the fifth day, the patient presented fever and seizures, which did not stop with medication, requiring sedation, orotracheal intubation and transfer to the intensive care unit. Lumbar puncture was performed for CSF analysis, which was positive for Herpes simplex type I. The patient was extubated, on the second day of hospitalization, maintained with a maintenance dose of phenytoin, sumatriptan, dipyrone, metoclopramide and acyclovir 500 mg, remaining intravenous, without presenting new seizures for 45 days, when she was discharged. After 30 days, he returned for a review consultation without making a complaint.

A demand side management approach to increase self-consumption in buildings

There is a growing interest in increasing the presence of renewable energy in the electric *** production from grid-connected systems is leading this growth in terms of *** this development,concern about network security has emerged,because excesses of intermittent renewable energy on the grid could exceed voltage ***-consumption,understood as the capacity of the producer to consume his or her own production,can partially solve these *** controllable loads,such as heating and cooling,represent 50%of the total amount of energy consumed by buildings;the proper allocation of these loads could be a driving force for *** this study,a demand side management strategy is proposed based on a building energy model equipped with an inverter heat pump coupled with a photovoltaic *** goal is to maximize the use of local energy from the photovoltaic plant(self-consumption),reducing the export and import of energy to and from the *** goal is achieved by optimizing the set-points in each *** array of optimal set-points over six years is *** results show the capacity of the methodology to match similar values of self-consumption(70%in winter and 50%in summer)obtained by strategies based on chemical *** findings are shown in an energy matching chart at different levels of detail(yearly and monthly).Color bubbles are added to the matching chart to help visualize the unmatched energy of the system *** comparison with actual model predictive control technologies,this study’s strategy offers great simplicity and a large saving in computational time.

Background: Tropical forests play a fundamental role in the provision of diverse ecosystem services, such as biodiversity,climate and air quality regulation, freshwater provision, carbon cycling, agricultural support and culture. To understand the role of forests in the carbon balance, aboveground biomass(AGB) estimates are needed. Given the importance of Brazilian tropical forests, there is an urgent need to improve AGB estimates to support the Brazilian commitments under the United Nations Framework Convention on Climate Change(UNFCCC). Many AGB maps and datasets exist, varying in availability, scale and coverage. Thus, stakeholders, policy makers and scientists must decide which AGB product, dataset or combination of data to use for their particular goals. In this study, we assessed the gaps in the spatial AGB data across the Brazilian Amazon forests not only to orient the decision makers about the data that are currently available but also to provide a guide for future ***: We obtained a map of the gaps in the forest AGB spatial data for the Brazilian Amazon using statistics and differences between AGB maps and a spatial multicriteria evaluation that considered the current AGB datasets. The AGB spatial data gap map represents areas with good coverage of AGB data and, consequently, the main gaps or priority areas where further biomass assessments should focus, including the northeast of Amazon State, Amapá and northeast of Pará. Gut microbiota-derived metabolites are novel targets for improving insulin resistance

The gut microbiota plays a key role in metabolic ***-microbiota-derived metabolites are found in different dietary sources,including:Carbohydrate(acetate,propionate,butyrate,also known as short-chain fatty acids,as well as succinate);protein(hydrogen sulfide,indole,and phenylacetic acid);and lipids(resveratrol-,ferulic acid-,linoleic acid-,catechin-and berry-derived metabolites).Insulin resistance,which is a global pandemic metabolic disease that progresses to type 2 diabetes mellitus,can be directly targeted by these ***-derived metabolites have broad effects locally and in distinct organs,in particular skeletal muscle,adipose tissue,and *** metabolites can modulate glucose metabolism,including the increase in glucose uptake and lipid oxidation in skeletal muscle,and decrease in lipogenesis and gluconeogenesis associated with lipid oxidation in the liver through activation of phosphatidylinositol 3-kinase-serine/threonine-protein kinase B and AMP-activated protein *** adipose tissue,gut-microbiota-derived metabolites stimulate adipogenesis and thermogenesis,inhibit lipolysis,and attenuate ***,an increase in energy expenditure and fat oxidation occurs in the whole ***,the therapeutic potential of current pharmacological and non-pharmacological approaches used to treat diabetes mellitus can be tested to target specific metabolites derived from intestinal bacteria,which may ultimately ameliorate the hyperglycemic burden.

A rovibrational model,including anharmonic,centrifugal,and Coriolis corrections,is used to calculate π,K,N,and ∑ orbital and radial *** four orbital excitations of the n meson correspond to the/?(1235),ti2(1670),63(2030),and π4(2250)*** first four radial excitations correspond to the π(1300),π(1800),π(2070),and 7t(2360)*** orbital excitations of the K meson are interpreted as the K_(1)(1270),K_(2)(1770),K_(3)(2320),and K_(4)(2500)resonances;its radial excitations correspond to the K(1460)and K(1830)*** N orbital excitations are identified with the N(1520),N(1680),N(2190),N(2220),and N(2600)*** first four radial excitations of the N family correspond to the N(1440),N(1880),N(2100),and N(2300)*** orbital excitations of the ∑ baryon are associated with the ∑(1670),∑(1915),∑(2100),and ∑(2250)resonances,whereas its radial excitations are identified with the ∑(1660),∑(1770),and ∑(1880)*** proposed rovibrational model calculations show a good agreement with the corresponding experimental values and allow for the prediction of hadron resonances,thereby proving to be useful for the interpretation of excited hadron spectra.

Risk factors, surgical complications and graft survival in liver transplant recipients with early allograft dysfunction

Background: Early allograft dysfunction (EAD) is a severe complication after liver transplantation. The associated risk factors and complications have re-gained recent interest. This study investigated risk factors, survival and complications associated with EAD in a large liver transplant center in Latin America. Methods: Retrospective, unicenter, cohort, based on data from adult patients undergoing first deceaseddonor liver transplant from January 2009 to December 2013. EAD was defined by one or more of the following:(i) bilirubin ≥10 mg/dL on postoperative day 7;(ii) international normalized ratio ≥1.6 on postoperative day 7, and (iii) alanine aminotransferase or aspartate aminotransferase > 2000 IU/L within the first seven days after transplant. Results: A total of 602 patients were included;of these 34.2% developed EAD. Donor risk factors were male ( P = 0.007), age between 50 and 59 years ( P = 0.034), overweight ( P = 0.028) or grade I obesity ( P = 0.012), sodium > 157 mmol/L ( P = 0.002) and grade IV ischemia/reperfusion injury ( P = 0.002). Cold ischemia time ≥10 h ( P = 0.008) and warm ischemia time ≥40 min ( P = 0.013) were the surgical factors. Male ( P 157 mmol/L and grade Ⅳ ischemia/reperfusion injury. Cold ischemia time ≥10 h and warm ischemia time ≥40 min were the surgical risk factors. Male was the only recipient protective factor. Patients with EAD had higher reoperations and graft loss rates.

This paper discusses a mathematical model for a liquid phase reacting flow occurring at the cathode of a patent pending novel fuel cell geometry, where a non homogeneous catalysis carried by gold and Prussian Blue, with the first reducing air O2 and the second the resulting H2O2. The breathing zone is porous walls microtubes, with three different types of pores in its walls. Inside the microtubes there is water solution of sulfuric acid. The microtubes possess an external layer of extremely porous polymer hydrophobic agent. A Prussian Blue thin porous layer is over the selective membrane. Appropriate porous and tubular connecting elements close the fluid loop. The asymmetry induces proper current and electric potential profiles, which leads to a mainly electrocapillary electrokinetic flow, which enhances the oxygen transport and assures the H2O2 flow to its reduction layer.

Stem cell therapies in cardiac diseases: Current status and future possibilities

Cardiovascular diseases represent the world’s leading cause of death. In thisheterogeneous group of diseases, ischemic cardiomyopathies are the mostdevastating and prevalent, estimated to cause 17.9 million deaths per *** all biomedical efforts, there are no effective treatments that can replacethe myocytes lost during an ischemic event or progression of the disease to heartfailure. In this context, cell therapy is an emerging therapeutic alternative to treatcardiovascular diseases by cell administration, aimed at cardiac regeneration andrepair. In this review, we will cover more than 30 years of cell therapy in cardiology,presenting the main milestones and drawbacks in the field and signalingfuture challenges and perspectives. The outcomes of cardiac cell therapies arediscussed in three distinct aspects: The search for remuscularization byreplacement of lost cells by exogenous adult cells, the endogenous stem cell era,which pursued the isolation of a progenitor with the ability to induce heart repair,and the utilization of pluripotent stem cells as a rich and reliable source ofcardiomyocytes. Acellular therapies using cell derivatives, such as microvesiclesand exosomes, are presented as a promising cell-free therapeutic alternative.

Nutrient content affecting Spodoptera frugiperda and Dalbulus maidis occurrence in corn

This work aims to study the effect of the nutrient contents in the leaves of corn on the incidence of Spodopterafrugiperda （Smith）（Lepidoptera： Noctuidae） and Dalbulus maidis （Delong ＆ Wolcott）（Homoptera： Cicadellidae）. The treatments were represented by unfertilized corn plants and corn plants fertilized with 250 kg of the formulation 4-14-8 （N- P-K）＋ 100 kg of ammonium sulfate （AS）/ha and 500 kg of the formulation 4-14-8 （N-P-K）＋200 kg of AS/ha, arranged in randomized blocks with four replicates. Attacks by S. frugiperda and D. maidis were higher as the nitrogen, sulfur, calcium and copper content in the leaves of corn increased. The effects of calcium and copper found in this study were not consistent with that of lower susceptibility associated with higher levels of these nutrients mentioned in the literature. We believe that there was a cumulative effect of the nutrients altogether, building up a nutritiously better vegetable which in turn had less reserve for defense, or that the content of both nutrients in the leaves were not high enough to induce such effects.
