Background:Discordance in hepatocellular carcinoma(HCC)staging between pre-transplant imaging and explant pathology is associated with an increased risk of recurrence and *** aim was to evaluate variables that predicted concordance/discordance in the era of new generation locoregional therapies(LRT)and improved radiologic technology in ***:A single-center retrospective study was performed on patients who received a liver transplant for HCC between ***-and post-LT variables,including type of LRT,downstaging(DS),transplant time period,and radiologic response to LRT,were analyzed for concordance/***-Meier analysis was used to assess post-LT ***:Of 146 patients transplanted within Milan Criteria(MC),discordance rates(understaged)were 45%.Discordance was associated with≥3 HCC lesions at diagnosis but not newer generation LRT(transarterial radioembolization/stereotactic body radiation therapy),traditional LRT or *** differences in discordance were seen between transplant periods(2008-2013 vs.2014-2019),but those within MC in the earlier period had higher concordance rates.A trend was observed between DS and ***:HCC stage discordance remains common and poorly *** was associated with three or more HCC lesions at the time of *** within MC transplanted between 2008-2013 was associated with concordance,while a trend was noted between DS and *** other pre-or post-LT variables predicted discordance/*** was associated with decreased survival.

Inflammasomes are large protein complexes that play a major role in sensing inflammatory signals and triggering the innate immune *** inflammasome complex has three major components:an upstream sensor molecule that is connected to a downstream effector protein such as caspase-1 through the adapter protein *** formation typically occurs in response to infectious agents or cellular *** active inflammasome then triggers caspase-1 activation,followed by the secretion of pro-inflammatory cytokines and pyroptotic cell *** inflammasome activation and activity contribute to the development of diabetes,cancer,and several cardiovascular and neurodegenerative *** a result,recent research has increasingly focused on investigating the mechanisms that regulate inflammasome assembly and activation,as well as the potential of targeting inflammasomes to treat various *** clinical trials are currently underway to evaluate the therapeutic potential of several distinct inflammasome-targeting ***,understanding how different inflammasomes contribute to disease pathology may have significant implications for developing novel therapeutic *** this article,we provide a summary of the biological and pathological roles of inflammasomes in health and *** also highlight key evidence that suggests targeting inflammasomes could be a novel strategy for developing new disease-modifying therapies that may be effective in several conditions.

Use of doppler ultrasound to predict need for transjugular intrahepatic portosystemic shunt revision

BACkGROUND Transjugular intrahepatic portosystemic shunt(TIPS)is used to treat complications of portal hypertension,such as ascites and variceal bleeding(VB).While liver doppler ultrasound(DUS)is used to assess TIPS patency,trans-shunt venography(TSV)is the gold *** To determine the accuracy of DUS to assess TIPS dysfunction and for need for *** Retrospective review of patients referred for TIPS revision from ***,DUS parameters at baseline and at the DUS preceding TIPS revision,TSV data were *** operating characteristics curves,sensitivity,specificity,performance for doppler to predict need for revision were *** and multivariate analyses were used to predict clinical factors associated with need for TIPS *** The cohort consisted of 89 patients with cirrhosis(64%men,76%white,31%alcohol as etiology);median age 59 *** for initial TIPS were VB(41%),refractory ascites(51%),and other(8%).TIPS was revised in 44%.On univariate analysis,factors associated with need for TIPS revision were male(P=0.03),initial indication for TIPS(P=0.05)and indication for revision(P=0.01).Revision of TIPS was associated with lower mortality(26%vs 46%)and significantly lower rates of transplant(13%vs 24%;P=0.1).In predicting need for TIPS revision,DUS has a 40%sensitivity,45%specificity,PPV 78%,and NPV 14%.The most accurate location for shunt velocity measure was distal velocity(Area under the curve:0.79;P=0.0007).CONCLUSION DUS has poor overall sensitivity and specificity in predicting need for TIPS ***-invasive methods of predicting TIPS dysfunction are needed since those needing TIPS revision had better survival.

Climate and weather-propelled wind power is characterized by significant spatial and temporal *** has been substantiated that the variability of wind power,in addition to contributing hugely to the instability of power grids,can also send the balancing costs of electricity markets *** studies on the same establish that curtailment of such variability can be achieved through the geographic aggregation of various widespread production sites;however,there exists a dearth of comprehensive evaluation concerning different levels/scales of such aggregation,especially from a global *** paper primarily offers a fundamental understanding of the relationship between the wind power variations and aggregations from a systematic viewpoint based on extensive wind power data,thereby enabling the benefits of these aggregations to be quantified from a state scale ranging up to a global ***,a meticulous analysis of the wind power variations is undertaken at 6 different levels by converting the 7-year hourly meteorological re-analysis data with a high spatial resolution of 0.25◦×0.25◦(approximate 28 km×28 km)into a wind power series ***,the proposed assessment framework employs a coefficient of variation of wind power as well as a standard deviation of wind power ramping rate to quantify the variations of wind power and wind power ramping rate to exhibit the characteristics and benefits yielded by the wind power aggregation at 6 different levels.A system planning example is adopted to illustrate the correlation between the coefficient of variation reduction of wind power and investment reduction,thereby emphasizing the benefits pertaining to significant investment reduction via ***,a wind power duration curve is used to exemplify the availability of wind power aggregated at different ***,the results provide insights into devising a universal approach towards the deployment of wind power,prin
