BACKGROUND Primary hyperparathyroidism is typically caused by a single parathyroid *** parathyroid adenomas occur as well,with cases involving various sites,including the mediastinum,presenting in varying *** hyperparathyroidism develops in the context of chronic kidney disease,primarily due to vitamin D deficiency,hypocalcemia,and *** is frequently diagnosed in patients undergoing *** article presents a rare case of hyperparathyroidism involving multiple hyperplastic parathyroid glands with pulmonary seeding in a 50-year-old female patient undergoing hemodialysis(HD).CASE SUMMARY The patient had a history of parathyroidectomy 10 years prior but developed recurrent hyperparathyroidism with symptoms of pruritus and cough with sputum during a period of routine *** imaging revealed multiple nodules in both lungs,with the largest measuring approximately 1.35 *** histopathology confirmed the presence of hyperplastic parathyroid glands within the pulmonary *** tumor resection surgery via videoassisted thoracic surgery with wedge resection,the patient was discharged in stable condition and in follow-up her symptoms showed *** This article describes hyperparathyroidism presenting as pulmonary nodules in a patient undergoing postparathyroidectomy HD,highlighting diagnostic challenges and a positive outcome from tumor resection surgery.

Our previous work has introduced the newly generated program using the code transformation model GPT-2,verifying the generated programming codes through simhash(SH)and longest common subsequence(LCS)***,the entire code transformation process has encountered a time-consuming ***,the objective of this study is to speed up the code transformation process signi􀀀*** paper has proposed deep learning approaches for modifying SH using a variational simhash(VSH)algorithm and replacing LCS with a piecewise longest common subsequence(PLCS)algorithm to faster the veri􀀀cation process in the test *** the code transformation model GPT-2,this study has also introduced MicrosoMASS and Facebook BART for a comparative analysis of their ***,the explainable AI technique using local interpretable model-agnostic explanations(LIME)can also interpret the decision-making *** experimental results show that VSH can reduce the number of quali􀀀ed programs by 22.11%,and PLCS can reduce the execution time of selected pocket programs by 32.39%.As a result,the proposed approaches can signi􀀀cantly speed up the entire code transformation process by 1.38 times on average compared with our previous work.

We propose two variants of tailored finite point(TFP)methods for discretizing two dimensional singular perturbed eigenvalue(SPE)problems.A continuation method and an iterative method are exploited for solving discretized systems of equations to obtain the eigen-pairs of the *** study the analytical solutions of two special cases of the SPE,and provide an asymptotic analysis for the *** theoretical results are verified in the numerical *** numerical results demonstrate that the proposed schemes effectively resolve the delta function like of the eigenfunctions on relatively coarse grid.

目的通过临床访谈、声学评估及尝试性刺激治疗手法,从量化和质性角度探讨1位青春期假声个案的临床嗓音治疗介入成效。方法嗓音治疗介入包含嗓音评估、嗓音治疗及追踪各1次,为期3周。首次评估后给予个案尝试性刺激治疗手法降低音高,辅以视觉及听觉反馈方式,强化个案对喉部的控制能力。治疗前后分别对患者进行嗓音听知觉评估、声学评估及问卷评估。结果个案接受评估治疗后,嗓音快速恢复至正常音高,因嗓音异常导致的心理压力也明显缓解。结论嗓音治疗能快速、有效地矫治青春期假声,帮助个案摆脱嗓音异常带来的心理阴影,重拾正常人际关系互动。

Transcriptome Profiling of the Abdominal Skin of Larimichthys crocea in Light Stress

Large yellow croaker(Larimichthys crocea), one of the most important marine fish species in China, can change its abdominal skin color when it is shifted from light to dark or from dark to light, providing us an opportunity of investigating the molecular responding mechanism of teleost in light stress. The gene expression profile of fish under light stress is rarely documented. In this research, the transcriptome profiles of the abdominal skin of L. crocea exposed to light or dark for 0 h, 0.5 h and 2 h were produced by next-generation sequencing(NGS). The cluster results demonstrated that stress period, rather than light intensity(e.g., light or dark), is the major influencing factor. Differently expressed genes(DEGs) were identified between 0 h and 0.5 h groups, between 0 h and 2 h groups, between 0.5 h light and 0.5 h dark, and between 2 h light and 2 h dark, respectively. The gene ontology(GO) and Kyoto Encyclopedia of Genes and Genomes(KEGG) annotation revealed that the genes relating to immunity, energy metabolism, and cytoskeletal protein binding were significantly enriched. The detailed analysis of transcriptome profiles also revealed regular gene expression trends, indicating that the elaborate gene regulation networks underlined the molecular responses of the fish to light stress. This transcriptome analysis suggested that systematic and complicated regulatory cascades were functionally activated in response to external stress, and coloration change caused by light stress was mainly attributed to the change in the density of chromatophores for L. crocea. This study also provided valuable information for skin coloration or light stress research on other marine fish species.

Immunotherapy is an ascendant approach in cancer treatment. It shows more pronounced effects on killing cancer cells in a specific manner in particular against metastasis more than traditional techniques, such as chemotherapy or surgery. However, tumor immunosuppression limits the response of the immune system to cancer development. In this study, we developed a lipid-based nanocarrier doubly loaded with imiquimod(R837), a toll-like receptor 7 agonist, and caffeine, an adenosine receptor antagonist. This R837/caffeine loaded nanocarrier served as a nano-immunomodulator(RC-n IM) for combination treatment with radiotherapy(RT)against orthotopic breast cancer. RT-induced immunogenic cell death facilitated the production of tumor antigen and elicited the immune response in corporation with R837-medaited activation of antigen-presenting cells(APCs) while RC-n IMs being adopted. Additionally, caffeine, an adenosine analog, can successfully compete with adenosine in the tumor. The tumor-bearing mice that received RT together with RC-n IMs experienced the best antitumor effects and exhibited higher levels of T cells and APCs within the tumor; the growth of secondary tumors was also limited. This work serves as a proof-of-concept study for the development of a new immunotherapy strategy against cancer.

Objective： Curative gastric cancer surgery entails removal of the primary tumor with adequate margins including regional lymph nodes. European randomized controlled trials with recruitment in the 1990＇s reported increased morbidity and mortality for D2 compared to D 1. Here, we examined the extent of lymphadenectomy during gastric cancer surgery and the associated risk for postoperative complications and mortality using the strengths of a population-based study. Methods： A prospective nationwide study conducted within the National Register of Esophageal and Gastric Cancer. All patients in Sweden from 2006 to 2013 who underwent gastric cancer resections with curative intent were included. Patients were categorized into DO, D I, or D I ＋/D2, and analyzed regarding postoperative morbidity and mortality using multivariable logistic regression. Results： In total, 349 （31.7%） patients had a DO, 494 （44.9%） DI, and 258 （23.4%） DI＋/D2 lymphadenectomy. The 30-d postoperative complication rates were 25.5%, 25.1% and 32.2 % （DO, D I and D1 ＋/D2, respectively）, and 90-d mortality rates were 8.3%, 4.3 % and 5.8%. After adjustment for confounders, in mukivariable analysis, there were no significant differences in risk for postoperative complications between the lymphadenectomy groups. For 90-d mortality, there was a lower risk for D1 vs. DO. Conclusions： The majority of gastric cancer resections in Sweden have included only a limited lymphadenectomy （DO and DI）. More extensive lymphadenectomy （DI＋/D2） seemed to have no impact on postoperative morbidity or mortality.

AIM： To enhance the differentiation of insulin producing cell （IPC） ability from embryonic stem （ES） cells in vitro. METHODS： Four-day embryoid body （EB）-formatted ES cells were dissociated as single cells for the followed plasmid DNA delivery. The use of NucleofectorTM electroporator （Amaxa biosystems, Germany） in combination with medium-contained G418 provided a high efficiency of gene delivery for advanced selection. Neucleofected cells were plated on the top of fibronectincoated Petri dishes. Addition of Ly294002 and raised the glucose in medium at 24 h before *** differentiation status of these cells was monitored by semi-quantitative PCR （SQ-PCR） detection of the expression of relative genes, such as oct-4, sox-17, foxa2, mixll, pdx-1, insulin 1, glucagons and somatostatin. The percentage of IPC population on d 18 of the experiment was investigated by immunohistochemistry （IHC）, and the content/secretion of insulin was estimated by ELISA assay. The mice with severe combined immunodeficiency disease （SCID） pretreated with streptozotocin （STZ） were used to eliminate plasma glucose restoration after pax4^＋ ES implantation. RESULTS： A high efficiency of gene delivery was demonstrated when neucleofection was used in the present study; approximately 70% cells showed DsRed expression 2 d after neucleofection. By selection of medium-contained G418, the percentage of DsRed expressing cells kept high till the end of study. The pancreatic differentiation seemed to be accelerated by pax4 nucleofection. When compared to the group of cells with mock control, foxa2, mixll, pdxl, higher insulin and somatostatin levels were detected by SQ-PCR 4 d after nucleofection in the group of pax4 expressing plasmid delivery. Approximately 55% of neucleofected cells showed insulin expression 18 d after neucleofection, and only 18% of cells showed insulin expression in mock control. Three full scale two-story steel concentrically braced frames(CBFs)were tested at the National Center for Research on Earthquake Engineering(NCREE)in *** specimen is a single bay with the braces arranged in a two-story X-brace *** main differences among the three tests are the brace types(hollow structural or wide-flange section)and the design criteria adopted for the gusset plate *** of these three tests confirm that the two-story X-shape steel CBFs all have rather good energy dissipation characteristics up to a story drift of about 0.03 radians under the cyclically increasing lateral *** brace local buckling and out-of-plane displacements were observed during each *** confirm that both the 2tlinear and 8t-elliptical designs of the gusset plate connection provide satisfactory ductility for the steel *** structural section(HSS)braces fractured at a story drift smaller than that found using wide flange *** nonlinear fine element method(FEM)program ABAQUS was used to simulate the responses of the *** base shear versus the story drift relationships obtained from the tests and the FEM analytical results are quite agreeable in various levels of lateral frame *** analytical results confirm that the severe out-of-plane buckling of the braces can be accurately *** analyses also illustrate that the steel moment resisting frame takes about 40%story shear when the inter-story drift is greater than 0.02 radians.
