A critical review of the fatigue life prediction of asphalt mixtures and pavements

New pavement construction techniques and the increased use of recycled materials have led to unexpected and premature pavement failure in recent *** pavement’s exposure to daily and seasonal extreme temperature and repeated vehicular loads accumulate *** cracking occurs once the cumulative damage surpasses the material’s cracking *** crack is the most common pavement cracking *** the past four decades,researchers have carried out numerous experiments and analyses to understand pavement *** paper aims to provide an overview of fatigue cracking and discuss various fatigue test methods for characterizing asphalt concrete *** article also discusses the most common phenomenological and mechanistic models for predicting the fatigue life of asphalt concrete pavements based on different fatigue test *** paper details the implementation of the commonly used numerical models found in numerical simulation software and their prediction ability for the fatigue life of a pavement *** major flaws in current evaluation methods are the sensitivity of experimental results and the lack of reliability of some predictive *** asphalt material characterization is the ongoing practice for determining the most appropriate performance evaluation ***,proceeding with future research objectives is unrealistic until the accuracy of the tests and reliability of the predictions can be verified against actual field *** critical review of the fatigue life predictions of asphalt mixtures and pavements should help to refine or redefine the right course of action for future research.

Construction quality acceptance performance-related specifications for chip seals

The research described herein details the establishment of a framework for construction quality acceptance performance-related specifications(PRS) for chip seal surface treatments. This paper establishes acceptance quality characteristics(AQCs) and relationships between the AQCs and related chip seal performance measures. This work is a critical step in developing PRS for constructed chip seal treatments and establishing suitable guidelines for the PRS. The main objective of this paper is to determine appropriate test methods to evaluate each defined AQC. The relationships that are established between the AQCs and chip seal performance are used to define performance ranges and threshold values for a particular performance measure. These performance ranges and threshold values then can be used to define pay factors for the constructed chip seal section. The threshold values and pay factors for each AQC described herein are preliminary recommendations and require further validation prior to implementation. However, this research demonstrates how these PRS would be utilized to ensure acceptable chip seal construction quality in the *** developed construction quality acceptance PRS framework uses the percent within limits(PWL) concept to determine whether a chip seal lot passes the PRS threshold values and whether the contractor receives full pay, is subject to a pay penalty, or fails the construction PRS and must correct the chip seal within the first year in service, which constitutes the typical warranty period for contracted chip seal ***, recommendations are made as to the next steps in the development and validation of these construction quality acceptance PRS that are needed for implementation by contractors and state roadway maintenance agencies.

Management of borderline resectable pancreatic cancer

Pancreatic cancer is the fourth most common cause of cancer death in the United States. Surgery remains the only curative option; however only 20% of the patients have resectable disease at the time of initialpresentation. The definition of borderline resectable pancreatic cancer is not uniform but generally denotes to regional vessel involvement that makes it unlikely to have negative surgical margins. The accurate staging of pancreatic cancer requires triple phase computed tomography or magnetic resonance imaging of the pancreas. Management of patients with borderline resectable pancreatic cancer remains unclear. The data for treatment of these patients is primarily derived from retrospective single institution experience. The prospective trials have been plagued by small numbers and poor accrual. Neoadjuvant therapy is recommended and typically consists of chemotherapy and radiation therapy. The chemotherapeutic regimens continue to evolve along with type and dose of radiation therapy. Gemcitabine or 5-fluorouracil based chemotherapeutic combinations are administered. The type and dose of radiation vary among different institutions. With neoadjuvant treatment, approximately 50% of the patients are able to undergo surgical resections with negative margins obtained in greater than 80% of the patients. Newer trials are attempting to standardize the definition of borderline resectable pancreatic cancer and treatment regimens. In this review, we outline the definition, imaging requirements and management of patients with borderline resectable pancreatic cancer.

Neo-adjuvant therapy for hepatocellular carcinoma before liver transplantation:Where do we stand?

Liver transplantation (LT) for hepatocellular carcinoma (HCC) within Milan criteria is a widely accepted optimal therapy. Neo-adjuvant therapy before transplantation has been used as a bridging therapy to prevent dropout during the waiting period and as a down-staging method for the patient with intermediate HCC to qualify for liver transplantation. Transarterial chemoembolization and radiofrequency ablation are the most commonly used method for locoregional therapy. The data associated with newer modalities including drug-eluting beads, radioembolization with Y90, stereotactic radiation therapy and sorafenib will be discussed as a tool for converting advanced HCC to LT candidates. The concept “ablate and wait” has gained the popularity where mandated observation period after neo-adjuvant therapy allows for tumor biology to become apparent, thus has been recommended after down-staging. The role of neo-adjuvant therapy with conjunction of “ablate and wait” in living donor liver transplantation for intermediate stage HCC is also discussed in the paper.

Application of viscoelastic continuum damage approach to predict fatigue performance of Binzhou perpetual pavements

For this study, the Binzhou perpetual pavement test sections constructed in Shandong Province, China, were simulated for long-term fatigue performance using the layered viscoelastic pavement analysis for critical distresses （LVECD） finite element software package. In this framework, asphalt concrete was treated in the context of linear visco- elastic continuum damage theory. A recently developed unified fatigue failure criterion that defined the boundaries of the applicable region of the theory was also incorporated. The mechanistic modeling of the fatigue mechanisms was able to accommodate the complex temperature variations and loading conditions of the field pavements in a rigorous manner. All of the material models were conveniently characterized by dynamic modulus tests and direct tension cyclic fatigue tests in the laboratory using cylindrical specimens. By comparing the obtained damage characteristic curves and failure criteria, it is found that mixtures with small aggregate particle sizes, a dense gradation, and modified asphalt binder tended to exhibit the best fatigue resistance at the material level. The 15 year finite element structural simulation results for all the test sections indicate that fa- tigue performance has a strong dependence on the thickness of the asphalt pavements. Based on the predicted location and severity of the fatigue damage, it is recommended that Sections 1 and 3 of the Binzhou test sections be emoloved for perpetual pavement design.

Prognostic value of pre-treatment F-18-FDG PET-CT in patients with hepatocellular carcinoma undergoing radioembolization

AIM To evaluate the value of pre-treatment 18F-FDG PET/CT in patients with HCC following liver *** We identified 34 patients with HCC who underwent an FDG PET/CT scan prior to hepatic radioembolization at our institution between 2009 and 2013. Patients wereseen in clinic one month after radioembolization and then at 2-3 mo intervals. We assessed the influence of FDG tumor uptake on outcomes including local liver control(LLC), distant liver control(DLC), time to distant metastases(DM), progression free survival(PFS) and overall survival(OS).RESULTS The majority of patients were males(n = 25, 74%), and had Child Pugh Class A(n = 31, 91%), with a median age of 68 years(46-84 years). FDG-avid disease was found in 19(56%) patients with SUVmax ranging from 3 to 20. Female patients were more likely to have an FDG-avid HCC(P = 0.02). Median follow up of patients following radioembolization was 12 months(1.2-62.8 mo). FDG-avid disease was associated with a decreased 1 year LLC, DLC, DM and PFS(P < 0.05). Using multivariate analysis, FDG avidity predicted for LLC, DLC, and PFS(all P < 0.05).CONCLUSION In this retrospective study, pre-treatment HCC FDGavidity was found to be associated with worse LLC, DLC, and PFS following radioembolization. Larger studies are needed to validate our initial findings to assess the role of F-18-FDG PET/CT scans as biomarker for patients with HCC following radioembolization.

Higher radiation dose with a shorter treatment duration improves outcome for locally advanced carcinoma of anal canal

AIM： To assess whether radiation dose and duration of treatment influence local control and survival of patients with locally advanced anal cancer treated with definitive chemoradiation. METHODS： Twenty-eight consecutive patients who were treated with definitive radiation therapy for bulky anal cancers （〉 5 cm in size） were reviewed. Nineteen patients had T3 lesions, 8 patients had T4 lesions, and 15 patients had lymph node involvement. The median tumor size was 7.5 cm. All but one patient received concurrent chemoradiation. The median radiation dose was 54 Gy. The median duration of treatment was 58 d. RESULTS： With a median follow-up of 2.5 years in all patients and 7.8 years in living patients, the 2-year local recurrence-free probability was 570 and overall survival rate was 67%. Neither radiation dose nor duration of treatment alone was predictive of either time to local failure or overall survival. However, longer treatment breaks can potentially mask an advantage over higher radiation doses. Therefore, we examined those patients who received ≥ 54 Gy within 60 d, comparing them to the rest of the patients. Of patients who received ≥54 Gy within 60 d, local progression-free probability was 890 versus 420 for the rest of the group （P = 0.01）. CONCLUSION： Local failure is a significant problem in locally advanced carcinomas of-the anal canal. Higher radiation doses with limited treatment breaks may offer an increase in local control and survival.
