Hematogenous Osteomyelitis by Acinetobacter Baumannii: Case Report and Literature Review

Skeletal infection with Acinetobacter baumanii is a rare condition and found mainly among soldiers injured in war. Multidrug resistant (MDR) Acinetobacter baumanii (A. baumanii) osteomyelitis is difficult to treat and requires long course of intravenous antibiotics. Most of reported cases in the literature are the consequences of direct inoculation of the pathogen. Here in, we report the first case of A. Baumannii osteomyelitis disseminated through hematogenous route and the therapeutic approach for this rare infection. Clinical Presentation: A 46 year old African-American male patient with human immunodeficiency virus (HIV) and end- stage renal disease on hemodialysis, who developed persistent MDR A. baumanii bacteremia in the hospital, thought to be secondary to the hemodialysis catheter, necessitating replacement of the catheter. Three months after his discharge to a skilled nursing facility (SNF), he developed left leg swelling without noticeable pain or fever. MRI revealed findings consistent with chronic osteomyelitis of left tibia and intra operative bone culture grew MDR A. baumannii. The patient had good outcome after three surgical debridements and prolonged period (7 Months) of dual antimicrobial therapy. Discussion: While in most documented cases of A. baumannii osteomyelitis, entry appears to require direct inoculation;our case suggests that this pathogen can seed into bone hematogenously in the setting of immunosuppression, persistent bacteremia and possibly in the presence of underlying bone infarcts. Clinicians need to be aware of this rare possible consequence of A. baumannii bacteremia. In conclusion, combination of multiple surgical debridements and dual antimicrobial therapy for a long period may result in a good outcome.

Modeling Solid Waste Minimization Performance at Source in Dar es Salaam City, Tanzania

Municipal solid waste generation is strongly linked to rising human population and expanding urban areas, with significant implications on urban metabolism as well as space and place values redefinition. Effective management performance of municipal solid waste management underscores the interdisciplinarity strategies. Such knowledge and skills are paramount to uncover the sources of waste generation as well as means of waste storage, collection, recycling, transportation, handling/treatment, disposal, and monitoring. This study was conducted in Dar es Salaam city. Driven by the curiosity model of the solid waste minimization performance at source, study data was collected using focus group discussion techniques to ward-level local government officers, which was triangulated with literature and documentary review. The main themes of the FGD were situational factors (SFA) and local government by-laws (LGBY). In the FGD session, sub-themes of SFA tricked to understand how MSW minimization is related to the presence and effect of services such as land use planning, availability of landfills, solid waste transfer stations, material recovery facilities, incinerators, solid waste collection bins, solid waste trucks, solid waste management budget and solid waste collection agents. Similarly, FGD on LGBY was extended by sub-themes such as contents of the by-law, community awareness of the by-law, and by-law enforcement mechanisms. While data preparation applied an analytical hierarchy process, data analysis applied an ordinary least square (OLS) regression model for sub-criteria that explain SFA and LGBY;and OLS standard residues as variables into geographically weighted regression with a resolution of 241 × 241 meter in ArcMap v10.5. Results showed that situational factors and local government by-laws have a strong relationship with the rate of minimizing solid waste dumping in water bodies (local R square = 0.94).

Early mortality and safety after transcatheter aortic valve replacement using the SAPIEN 3 in nonagenarians

Background Transcatheter aortic valve replacement （TAVR） has been performed for many elderly patients with severe aortic stenosis （AS）. The SAPIEN 3 is one of the latest balloon-expandable prosthesis. This study aimed to investigate the early clinical outcomes after TAVR using the SAPIEN 3 in nonagenarians. Methods A total of 97 consecutive patients underwent TAVR for severe AS between De- cember 2015 and December 2016. Of these, 85 consecutive patients who underwent TAVR using the SAPIEN 3 were included. According to the age, patients were classified into age 〉 90 years group （17 patients） or age 〈 90 years group （68 patients）. The clinical outcomes in- cluding all-cause mortality and composite endpoint of early safety at 30 days were evaluated. Results The Society of Thoracic Surgeons score in age 〉 90 years group was higher than age 〈 90 years group （12.3 ±6.1% vs. 8.5 ± 5.1%, P 〈 0.01）. There was no significant difference in 30-day mortality between the two groups. However, the life-threatening bleeding and major vascular complications in age 〉 90 years group were greater than age 〈 90 years group （11.8% vs. 1.5%, P = 0.04 and 11.8% vs. 1.5%, P = 0.04, respectively）. The composite endpoint of early safety at 30 days was similar between the two groups. Multivariate logistic regression analysis showed that prior myocardial infarction was an independent predictor of the composite endpoint of early safety （odds ratio： 4.76, 95% confidence interval： 1.02-22.21, P = 0.047）. Conclusions The early mortality and safety after TAVR using the SAPIEN 3 in nonagenarians were similar and acceptable despite of higher operative risk.

Background:Soccer match-play is typically contested over 90 min;however,in some cup and tournament scenarios,when matches are tied,they proceed to an additional 30 min,which is termed "extra-time^(ET).This systematic review sought to appraise the literature available on 120-min of soccer-specific exercise,with a view to identifying practical recommendations and future research ***:The review was conducted according to the PRISMA *** researchers performed a systematic search of PubMed,CINAHL,and PsycINFO in May 2019,with the following keywords entered in various combinations:"soccer","football","extra-time","extra time","extratime","120 minutes","120 min","additional 30 minutes",and "additional 30 min".Results:The search yielded an initial 73 *** the screening process,11 articles were accepted for *** were subsequently organized into the following 5 categories:movement demands of ET,performance responses to ET,physiological and neuromuscular response during ET,nutritional interventions,and recovery and *** results highlighted that during competitive match-play,players cover5%-12% less distance relative to match duration(i.e.,meters per minute) during ET compared to the preceding 90 *** in technical performance(i.e.,shot speed,number of passes and dribbles) were also observed during ***,carbohydrate provision may attenuate and improve dribbling performance during ***,objective and subjective measures of recovery may be further compromised following ET when compared to 90 ***:Additional investigations are warranted to further substantiate these findings and identify interventions to improve performance during ET.

Polarization variability arising from clumps in the winds of Wolf-Rayet stars

Polarimetric and photometric variability of Wolf-Rayet （WR） stars as caused by clumps in the winds is revisited. In our model, which is improved from Li et al., radial expansion of the thickness is accounted for, but we retain dependence on the β velocity law and stellar occultation effects. We again search for parameters that can yield results consistent with observations in regards to the mean polarization p, the ratio R = σp/σphot of polarimetric to photometric variability and the volume filling factor fV. Clump generation and spatial distribution are randomized by the Monte Carlo method so as to produce clumps which are, in the mean, distributed uniformly in space and have time intervals that obey a Gaussian distribution. The generated clumps move radially outward with a velocity law determined by a β index, and the angular size of clumps is assumed to be fixed. By fitting the observed σp/σphot and the volume filling factor fv, clump velocity taw index β （- 2） and clump ejection rate .N （- 1） are inferred, and are found to be well constrained. In addition, the subpeak features of broad emission lines seem to support the clump ejection rate. Meanwhile, the fraction of total mass loss rate that is contained in clumps is obtained by fitting observed polarization. We conclude that this picture of the clumps＇ properties produces a valuable diagnostic of WR wind structure.

Assessment of energy utilization and leakages in buildings with building information model energy

Given the ability of building information models （BIM） to serve as a multidisciplinary data repository, this study attempts to explore and exploit the sustainability value of BIM in delivering buildings that require less energy for operations, emit tess carbon dioxide, and provide conducive living environments for occupants. This objective was attained by a critical and extensive literature review that covers the following：（1） building energy consumption, （2） building energy performance and analysis, and （3） BIM and energy assessment. Literature cited in this paper shows that linking an energy analysis toot with a BIM model has helped project design teams to predict and create optimized energy consumption by conducting building energy performance analysis utilizing key performance indicators on average thermal transmitters, resulting heat demand, lighting power, solar heat gains, and ventilation heat losses. An in-depth analysis was conducted on a completed BIM integrated construction project utilizing the Arboleda Project in the Dominican Republic to validate the aforementioned findings. Results show that the BIM-based energy analysis helped the design team attain the world＇s first positive energy building. This study concludes that linking an energy analysis tool with a BIM model helps to expedite the energy analysis process, provide more detailed and accurate results, and deliver energy-efficient buildings. This study further recommends that the adoption of level 2 BIM and BIM integration in energy optimization analysis must be demanded by building regulatory agencies for all projects regardless of procurement method （i.e., government funded or otherwise） or size.

Although monocytes and macrophages are key mediators of the innate immune system,the focus has largely been on the role of the adaptive immune system in the context of human immunodeficiency virus(HIV)*** more attention and research work regarding the innate immune system-especially the role of monocytes and macrophages during early HIV-1 infection-is *** monocytes and tissue macrophages are both susceptible targets of HIV-1 infection,and the early host response can determine whether the nature of the infection becomes pathogenic or *** example,monocytes and macrophages can contribute to the HIV reservoir and viral persistence,and influence the initiation/extension of immune activation and chronic *** the expansion of monocyte subsets(classical,intermediate and non-classical)provide an increased understanding of the crucial role they play in terms of chronic inflammation and also by increasing the risk of coagulation during HIV-1 *** review discusses the role of monocytes and macrophages during HIV-1 pathogenesis,starting from the early response to late dysregulation that occurs as a result of persistent immune activation and chronic *** changes are also linked to downstream targets such as increased coagulation and the onset of cardiovascular diseases.
