The r.cognition of p-glucans by dectin-1 has been shown to mediate cell activation, cytokine pr.duction and a var.ety of antifungal r.sponses. Her., we r.por. that the functional activity of dectin-1 in mucosal immunity to Candida albicans is influenced by the genetic backgr.und of the host. Dectin-1 was r.quir.d for.the pr.per.contr.l of gastr.intestinal and vaginal candidiasis in C57BIJ6, but not BALB/c mice; in fact, the latter.showed incr.ased r.sistance in the absence of dectin-1. The susceptibility of dectin-l-deficient C57BL/6 mice to infection was associated with defects in IL-17A and ar.l hydr.car.on r.ceptor.dependent IL-22 pr.duction and in adaptive Th I r.sponses. In contr.st, the r.sistance of dectin- 1-deficient BALB/c mice was associated with incr.ased IL-17A and IL-22 pr.duction and the skewing towar.s Thl/Tr.g immune r.sponses that pr.vide immunological memor.. Dispar.te canonical/ noncanonical NF-KB signaling pathways downstr.am of dectin-1 wer. activated in the two differ.nt mouse str.ins. Thus, the net activity of dectin-1 in antifungal mucosal immunity is dependent on the host＇s genetic backgr.und, which affects both the innate cytokine pr.duction and the adaptive Th1/Th17 cell activation upon dectin-1 signaling.

Cystic dystr.phy of the duodenal wall is a r.r. for. of the disease which was descr.bed in 1970 by Fr.nch author. who r.por.ed the pr.sence of focal pancr.atic disease localized in an ar.a compr.sing the C-loop of the duodenum and the head of the ***-man author. have defined this ar.a as a"gr.ove".We r.por. our.r.cent exper.ence on cystic dystr.phy of the par.duodenal space and systematically r.view the data in the liter.tur. r.gar.ing the alter.tions of this space.A MEDLINE sear.h of paper. published between 1966 and 2010 was car.ied out and 59 paper.wer. consider.d for.the pr.sent study;ther. wer. 19 cohor. studies and 40 case *** major.ty of patients having gr.ove pancr.atitis wer. middle *** age was significantly higher.in patients having gr.ove *** diagnosis of cystic dystr.phy of the duodenal wall can now be assessed by multi-detector.computer.tomogr.phy,magnetic r.sonance imaging and endoscopic *** latter.two techniques may also add mor. infor.ation on the involvement of the r.maining pancr.atic gland not involved by the duodenal malfor.ation and they may help in differ.ntiating"gr.ove pancr.atitis"fr.m "gr.ove adenocar.inoma".In conclusion,chr.nic pan-cr.atitis involving the entir. pancr.atic gland was pr.sent in half of the patients with cystic dystr.phy of the duodenal wall and,in the major.ty of them,the pan-cr.atitis had calcifications.

Since it has been r.cognized that sar.oidosis(SA) is not an exclusive disor.er.of the lungs but can also affect other.or.ans such as the liver.and spleen, effor.s have been made to define specific imaging cr.ter.a for.the diagnosis of the single or.an involvement, and the concept has been r.infor.ed that the exclusion of alter.ative causes is impor.ant to achieve the cor.ect diagnosis. Ultr.sound(US)is a useful tool to evaluate patients with suspected abdominal SA, such as of the liver. spleen, kidney, pancr.as and other.or.ans, showing findings such as or.anomegaly, focal lesions and lymphadenopathy. While the diagnosis of abdominal SA is mor. pr.dictable in the case of involvement of other.or.ans(e.g.,lungs), the pr.blem is mor. complex in the case of isolated abdominal SA. The r.cent use of contr.st-enhanced ultr.sound and endoscopic ultr.sound elastogr.phy has pr.vided additional infor.ation about the enhancement patter.s and tissue r.gidity in abdominal SA. Her. we cr.tically r.view the r.le of US in abdominal SA, r.por.ing typical findings and limitations of cur.ent evidence and by discussing futur. per.pectives of study.
