Dendritic cell vaccination in glioblastoma after fluorescenceguided resection

AIM: To assess whether the addition of a customized, active immunotherapy to standard of care including fluorescence-guided surgery, may provide hints of an improved survival for patients with poor-prognosis, incurable glioblastoma multiform. METHODS: Preliminary to our ongoing, phase-Ⅱ clinical trial, we conducted a small pilot study enrolling five consecutive patients with resectable glioblastoma. In terms of Recursive Partitioning Analysis, four patientswere class Ⅴ and one was class Ⅳ. In all five cases, fluorescence-guided surgery was employed, followed by rapid steroid discontinuation. Patients were then treated with a combination of standard radio-chemotherapy with temozolomide and tumor lysate-pulsed, mature dendritic cell-based ***: Though all five patients ultimately progressed, with any further treatment left to the sole decision of the treating oncologist, active immunotherapy was very well tolerated and induced specific immune responses in all three patients for whom enough material was available for such an assessment. Median progression-free survival was 16.1 mo. Even more important, median and mean overall survival were 27 mo and 26 mo, respectively. Three patients have died with an overall survival of 9 mo, 27 mo and 27.4 mo, while the other two are still alive at 32 mo and 36 mo, the former receiving treatment with bevacizumab, while the latter has now been off therapy for 12 mo. Four of five patients were alive at two ***: Active immunotherapy with tumor lysate-pulsed, autologous dendritic cells is feasible, safe, well tolerated and biologically efficacious. A phase-Ⅱ study is ongoing to possibly improve further on our very encouraging clinical results.

The goal of this work is to perform the risks analysis of the mission nAnOSATC-BR1 and nAnOSATC-BR2 and then compare them linearly. The nAnOSATC-BR1 and nAnOSATC-BR2 are the first and the second satellite, respectively. They belonged to the project nAnOSATC-BR—-development of CubeSats, which is performed in the facilities built by the partnership between the national Institute of Space Research and the Technological Center from Federal University of Santa Maria. The project focuses on the development of a scientific instrumentation and, simultaneously, the design development, construction, qualification and launch of a national scientific nanosatellite, in a cube shape with 100 mm of edge and near to 1.33 kg of mass, per unit (U). The risk analysis was held to identify and minimize the project"s risks of failure, due to its complexity, assuring the mission success, preventing extra pays and rework. The software, CubeSat Mission Design Software Tool for Risk Estimating, which uses statistical regression methods, was used. So, we were capable to measure the project"s most critical steps assuring its success. The nAnOSATC-BR1 was launched in June 19 and it is orbiting the Earth in a nominal regime and the nAnOSATC-BR2 has been scheduled to be launched in 2016.

The literature has shown the efficiency of exercise in the control of type 2 diabetes(T2D), being suggested as one of the best kinds of non-pharmacological treatments for its population. Thus, the scientific production related to this phenomenon has growing exponentially. However, despite its advances, still there is a lack of studies that have carried out a review on the acute effects of physical exercise on metabolic and hemodynamic markers and possible control mechanisms of these indicators in individuals with T2 D, not to mention that in a related way, these themes have been very little studied today. Therefore, the aim of this study was to organize and analyze the current scientific production about the acute effects of physical exercise on metabolic and hemodynamic markers and possible control mechanisms of these indicators in T2 D individuals. For such, a research with the following keywords was performed:-exercise; diabetes and post-exercise hypotension; diabetes and excess post-exercise oxygen consumption; diabetes and acute effects in PUBMED, SCIELO and HIGHWIRE databases. From the analyzed studies, it is possible to conclude that, a single exercise session can promote an increase in the bioavailability of nitric oxide and elicit decreases in postexercise blood pressure. Furthermore, the metabolic stress from physical exercise can increase the oxidation of carbohydrate during the exercise and keep it, in high levels, the post exercise consumption of O2, this phenomenon increases the rate of fat oxidation during recovery periods after exercise, improves glucose tolerance and insulin sensitivity and reduces glycemia between 2-72 h, which seems to be dependent on the exercise intensity and duration of the effort.

A Fuzzy Inference System for Increasing of Survivability and Efficiency in Wireless Sensor networks

The nodes of a WSns (wireless sensors network) are composed of small devices capable of sensing and transmitting data related to some phenomenon in the environment. These devices, named sensor nodes, have severe constraints, such as lower processing and storage capacity, and mainly they have severe constraints related to battery energy. Therefore, the developing of strategies to reduce the power consumption is one of the main challenges in WSns, and thereby helping to increase the survivability and efficiency of these networks. This paper proposes a new approach to help multi-path routing protocols to choose the best route based on Fuzzy Inference Systems and ACO (ant colony optimization). The Fuzzy System is used to estimate the degree of the route quality, based on the number of hops and the lowest energy level among the nodes that form the route. The ACO algorithm is used to adjust the rule base of the fuzzy system in order to improve the classification strategy of the route, and hence increasing the energy efficiency and the survivability of the network. The simulations showed that the proposal is effective from the point of view of the energy, the number of received messages, and the cost of received messages when compared against other approaches.

近年来,经自然腔道取标本手术(nOSES)治疗结直肠癌已引起了广泛关注。nOSES应用于结直肠手术的潜在优势包括减轻术后疼痛,减少伤口并发症,减少对术后镇痛的需求,加快肠道功能的恢复,缩短住院时间,更好的美容效果,以及给患者带来了更佳的心理状态。尽管nOSES的手术创伤显著减轻,然而该技术也存在一些潜在问题。尤其随着这项新技术的兴起,人们对其无菌的把控、肿瘤学结果和病例选择等问题表示出关切。因此,迫切需要一部操作指南来规范nOSES手术在结直肠癌中的开展。经过国际nOSES联盟全体成员的三轮讨论,最终形成了该部共识。这一共识有助于nOSES在全球范围内的长远发展。

Role of Cognitive Behavioral Therapy in Fibromyalgia: A Systematic Review

A literature search of articles from 2002-2019 was performed using Medline, Embase, Cochrane, LILACS, IBECS, CRD, and Epistemonikos databases, to analyze the effects of Cognitive-Behavioral Therapy (CBT) in the treatment of fibromyalgia. Twenty-seven articles were selected in which CBT was performed exclusively by specialist physicians, associated or not with conventional pharmacological treatment and/or physical exercise. In most articles, CBT worked with self-knowledge and cognitive restructuring, attempting to reduce pain perception, and it showed a general improvement in daily activities by decreasing patient’s limitations, such as morning stiffness. The literature showed significant correlations of CBT in pain processing over time. When CBT was compared to conventional pharmacological therapy, a certain superiority of CBT could be observed concerning the quality of life, catastrophizing, and acceptance of pain. However, when they were simultaneously applied, this improvement in quality of life was not observed.

AIM:To determine whether topical lidocaine benefits esophagogastroduoduenoscopy(EGD) by decreasing propofol dose necessary for sedation or procedurerelated ***:The study was designed as a prospective,single centre,double blind,randomised clinical trial and was conducted in 2012 between January and May(nCT01489891).Consecutive patients undergoing EGD were randomly assigned to receive supplemental topical lidocaine(L;50 mg in an excipient solution which was applied as a spray to the oropharynx) or placebo(P;taste excipients solution without active substance,similarly delivered) prior to the standard propofol sedation *** propofol was administered as a bolus intravenous(iv) dose,with patients in the L and P groups receiving initial doses based on the patient’s American Society of Anaesthesiologists(ASA) classification(ASAⅠ-Ⅱ:0.50-0.60 mg/kg;ASA Ⅲ-Ⅳ:0.25-0.35 mg/kg),followed by 10-20 mg iv dose every 30-60 s at the anaesthetist’s *** signs,anthropometric measurements,amount of propofol administered,sedation level reached,examination time,and the subjective assessments of the endoscopist’s and anaesthetist’s satisfaction(based upon a four point Likert scale) were *** statistical tests were performed by the Stata statistical software suite(Release 11,2009;StataCorp,LP,College Station,TX,United States).RESULTS:no significant differences were found between the groups treated with lidocaine or placebo in terms of total propofol dose(310.7 ± 139.2 mg/kg per minute vs 280.1 ± 87.7 mg/kg per minute,P = 0.15) or intraprocedural propofol dose(135.3 ± 151.7 mg/kg per minute vs 122.7 ± 96.5 mg/kg per minute,P = 0.58).Only when the L and P groups were analysed with the particular subgroups of female,nd lower anaesthetic risk level(ASA Ⅰ-Ⅱ) was a statistically significant difference found(L:336.5 ± 141.2 mg/kg per minute vs P:284.6 ± 91.2 mg/kg per minute,P = 0.03) for greater total propofol requirements).The total i
