Objective:Long-term survivors(LS)of non-small cell lung cancer(nSCLC)without driver alterations,displaying an overall survival(OS)of more than 3 years,comprise around 10%of cases in several series treated with *** are classical prognosis factors for these cases[stage,Eastern Cooperative Oncology Group(ECOG),etc.],but more data are required in the *** this multi-center study,we focused on LS of advanced nSCLC with OS above 36 months to perform a clinical-pathological and molecular ***:In the first step,we conducted a clinical-pathological characterization of the ***,we carried out a genetic analysis by comparing LS to a sample of short-term survivors(SS)(with an OS less than 9 months).We initially used whole-genome RnA-seq to identify differentiating profiles of LS and SS,and later confirmed these with reverse transcription-polymerase chain reaction(RT-PCR)for the rest of the ***:A total of 94 patients were included,who were mainly men,former smokers,having adenocarcinoma(AC)-type nSCLC with an ECOG of *** obtained an initial differential transcriptome expression,displaying 5 over-and 33 under-expressed genes involved in different pathways:namely,the secretin receptor,surfactant protein,trefoil factor 1(T FF1),serpin,Ca-channels,and Tolllike receptor(TLRs)***,RT-PCR analysis of 40(20 LS/20 SS)samples confirmed that four genes(surfactant proteins and SFTP)were significantly down-regulated in SS compared to LS by using an analysis of covariance(AnCOVA)model:SFTPA1(P=0.023),SFTPA2(P=0.027),SFTPB{P=0.02),and SFT PC(P=0.047).Conclusions:We present a sequential genetic analysis of a sample of nSCLCLS with no driver alterations,obtaining a differential RnA-seq/RT-PCR profile showing an abnormal expression of SF genes.

AIM： To determine prevalence of H pylori infection in symptomatic children in Buenos Aires, Argentina, and to investigate factors associated with HpyloH positivity. METHODS： A total of 395 children with upper gastrointestinal symptoms referred to the Gastroenterology Unit of the Children Hospital ＂Sor Maria Ludovica＂ were evaluated for the presence of H py/or/by the ^13C-Urea Breath Test （^13C-UBT）. A questionnaire was applied to the recruited population. RESULTS： Prevalence of H pylo H infection was 40.0% in this population （mean age 9.97 ± 3.1 years）. The factors associated with H py/or/positivity were number of siblings （P 〈 0.001）, presence of pet cats （P = 0.03） and birds （P = 0.04） in the household, and antecedents of gastritis among family members （P = 0.01）. After multivariate analysis, number of siblings [Odds ratio （OR） = 1.39; 95% CI, 1.20-1.61] and contact with pet cats （OR = 1.76; 95% CI, 1.00-3.09） remained as variables associated with H pylori infection. COnCLUSIOn： The prevalence of H pylori infection in children with upper gastrointestinal symptoms in Argentina was similar to that reported in developed countries. Children from families with a higher crowding index and presence of pet cats have a higher risk of being colonized with Hpylori.

A simple method for the estimation of the wavelength of a fiber laser system is proposed. The method is based on the use of a high-birefringence-fiber loop mirror(HBFLM).The HBFLM exhibits a periodic transmission/reflection spectrum whose spectral characteristics are determined by the length and temperature of the high-birefringence fiber(HBF).Then,by the previous characterization of the HBFLM spectral transmission response,the central wavelength of the generated laser line can be *** using a photodetector,the *** of the laser line is estimated during an HBF temperature scanning by measuring the temperature at which the maximum transmitted power of the HBFLM is *** proposed method is demonstrated in a linear cavity tunable Er/Yb fiber *** method is a reliable and low-cost alternative for laser wavelength determination in short wavelength ranges without the use of specialized and expensive equipment.

Cenozoic Age Counterclockwise Rotation in the northwest End of the Sierras Pampeanas, Argentina

Investigations into the Andean orocline revealed a counterclockwise rotation of about 37° in the north and a clockwise rotation of about 29° in the south. This rotation would have started in the Eocene because the nazca and South American plates converged. The transition zone between the Puna and the Sierras Pampeanas has a clockwise rotation pattern. Our new data show that the nE convergence of the nazca and South American plates caused the counterclockwise rotation around the nW end of the Sierras Pampeanas. The temperature rise during a magmatic activity at 13 Ma would have favored a counterclockwise rotation of the mountain blocks of about 20° on a detachment zone within 10 to 15 km of depth. These range rotations generated local stress tensors trending nE and nW, facilitating the development of valleys, basins, mineralized dikes, mineral deposits, and alluvial fans separated from their origin. The Atajo fault shows both ductile and brittle characteristics. A mylonitic belt from the Sierra de Aconquija was juxtaposed on the rocks of the Ovejería Block and the Farallón negro Volcanic Complex by reverse vertical displacement, and a dextral horizontal component of displacement resulted in curvatures that gave rise to pull-apart basins and step over features. The santa Maria Valley, Campo del Arenal, Hualfín Valley, and Pipanaco salt flat most likely constituted a vast early Miocene basin rarely interrupted by low feature relief.

Objective:Minimally invasive midvastus approach(mini—midvastus) has been widely used in total knee arthroplasty(TKA).However,the clinical effects still remains *** meta-analysis was based on randomized controlled trials(RCTs) aiming to quantitatively analyze the clinical efficacy of mini—midvastus versus standard parapatellar approach in ***:This meta-analysis was performed according to the PRISMA guidelines.A literature search for the eligible RCTs was carried out in the databases of PubMed,the Cochrane library,EMBASE and Web of *** independent reviewers independently completed the study selection,data extraction,and the assessment of methodological ***-analysis was conducted by the RevMan 5.2 ***:A total of 18 RCTs(937 patients with 1093 TKAs) published from 2007 to 2013 were *** meta-analysis suggested that the mini—midvastus approach significantly improved knee range of motion(rom) and decreased visual analog score(VAS) at postoperative 1- 2 weeks(p,0.05),and there were no statistical differences in terms of knee society score(KSS)(6 weeks to 1 year),VAS(6 weeks to 6 months),rom(6 weeks to 6 months),lateral retinacular release,blood loss,straight leg raise,hospital stay and postoperative complications between the mini—midvastus and standard parapatellar approach(p.0.05).However,the operative time was significantly longer when performing the mini—midvastus group than the parapartellar approach(p,0.05).Conclusion:This meta-analysis found that compared with the standard parapatellar approach,the mini—midvastus approach had early advantages in the VAS and rom,but had the disadvantage in the operative *** of Evidence:Therapeutic study Level I.

The alkali halide crystals exhibit significant TL （thermoluminescence） properties when exposed to ionizing radiation. The defects in crystals can be produced in high concentration by means of chemical impurities, radiation, mechanical work and others processes. This study is concerned with the TL properties of quaternary mixtures of alkali halide crystals when they are irradiated with beta and gamma-rays. The samples were made mixing KCI, KBr, RbCI, RbBr salts doped with divalent europium （Eu2＋）, by the Czochralski method sintering at 400 ~C during 6 hours in a n2 atmosphere. The samples were exposed to beta and gamma rays. We present the TL glow curves, demonstrating that the nature of the defects in the crystals can be produced by trapping states and recombination mechanisms. The highest temperature TL curves are the most interesting due to possible applications in dosimetry and optical energy storage.

An Unusual Case of a Pulmonary-Renal Syndrome with negative Anti-GBM and AnCA Antibodies

Pulmonary renal syndrome (PRS) is a high mortality, rare disorder presenting with diffuse alveolar hemorrhage and progressive acute glomerulonephritis. This syndrome is often caused by autoimmune entities, the most frequent being AnCA positive vasculitis and anti-GBM disease. We report a case of a 34-year-old Chilean woman, who initially presents with anemia and after a few days of inpatient management, starts with progressively worsening dyspnea, decrease in renal function and hematuria. The patient is initially diagnosed with pneumonia, but further evaluation using Thorax CT scan and renal biopsy confirms the suspicion of PRS. The case is of particular interest due to the lack of extensive bibliography on anti-GBM and AnCA negative pulmonary-renal syndrome, an uncategorized subtype of this syndrome with unknown optimal management.

Estimation of Contraction Coefficient of Gorlin Equation for Assessment of Aortic Valve Area in Aortic Stenosis

Background: The Gorlin equation is the reference method for the assessment of aortic valve area in aortic stenosis and is calculated using a constant, called the coefficient of contraction, which is empirically assumed to be 1. This coefficient is the ratio of effective aortic area to anatomic aortic area, and a value of 1 indicates that both are the same. The purpose of this study was to estimate the actual coefficient of contraction in patients with aortic stenosis and to evaluate its impact on aortic area as calculated by the Gorlin equation. Methods: We studied 17 patients with moderate to severe aortic stenosis. Effective aortic area was calculated using the continuity equation. Anatomic aortic area was obtained by planimetry with transesophageal echocardiography. Aortic valve area by the Gorlin equation was calculated from echocardiography data. The coefficient of contraction was derived as above. Results: The coefficient of contraction was inversely related to the pressure recovery. Effective area was correlated with anatomic area (r = 0.86, P 2). Aortic area by the Gorlin equation was not correlated with anatomic area, but the correlation became significant when the Gorlin equation was corrected for coefficient of contraction and pressure recovery. Conclusions: Using a coefficient of contraction of 1 in the Gorlin equation gives a poor correlation with anatomic area. Using the calculated coefficient of contraction for each patient and the mean gradient for pressure recovery improves the correlation with anatomic area. These facts could be taken in account when Gorlin equation is considered as the reference method.

Approach to Massive Bleeding in Obstetrics: Changing Management Paradigms Oriented by Early Goals [Early Goal Therapy in Postpartum Hemorrhage]

Recent findings: Approximately 830 women die every day worldwide as a result of complications related to pregnancy or childbirth with obstetric hemorrhage that remains a major cause of maternal morbidity and mortality. A systematic analysis by the World Health Organization [WHO] on the global causes of maternal deaths from 2003 to 2009 found that bleeding was the leading direct cause of maternal mortality followed by hypertensive disorders and sepsis. Overall, bleeding accounted for 27.1% of all maternal deaths worldwide. The need to implement strategies that mitigate the impact of bleeding in this clinical scenario is increasingly growing. Purpose of review: The purpose of this review is to propose a management strategy based on goals established through dynamic coagulation assessment. Summary: Effective management of obstetric hemorrhage requires rapid recognition, rapid response and mobilization of the multidisciplinary team. Volumetric resuscitation, hemodynamic stability, as well as the simultaneous identification and treatment of the source of bleeding and the resolution of coagulopathy in the shortest possible time remain the cornerstones of the management of Postpartum Hemorrhage (PPH).

RDG System in Remote Areas with Utility or Local Feed Topology and Operation Strategy

In remote regions with availability of wind energy, a RDG （renewable distributed generation） system is an advantageous alternative to increase the provision of electrical supply. Usually, these systems are structured on the basis of a connection to an existing weak grid. When the grid is out of service, the system may operate in islanding mode, if the RDG configuration allows it, continuing the provision of energy with standard voltage and frequency values. Facing the latter situation, a wind-diesel/gas generation system is proposed, with a conversion and control strategies based on a variable speed wind turbine employing a DFIG （doubly fed induction generator）, a SC （ultracapacitor） storage system and a SG （synchronous generator） driven by a diesel/gas engine.
