Success of susceptibility-guided eradication of Helicobacter pylori in a region with high secondary clarithromycin and levofloxacin resistance rates

BACKGROUND Resistance to clarithromycin(CLA)and levofloxacin(LFX)of Helicobacter pylori(H.pylori)is increasing in severity,and successful eradication is essential.Presently,the eradication success rate has greatly declined,leaving a large number of patients with previous treatment histories.AIM To investigate secondary resistance rates,explore risk factors for antibiotic resistance,and assess the efficacy of susceptibility-guided therapy.METHODS We recruited 154 subjects positive for Urea Breath Test who attended The First Affiliated Hospital of China Medical University between July 2022 and April 2023.Participants underwent a string test after an overnight fast.The gastric juice was obtained and transferred to vials containing storage solution.Subsequently,DNA extraction and the specific DNA amplification were performed using quantitative polymerase chain reaction(qPCR).Demographic information was also analyzed as part of the study.Based on these results,the participants were administered susceptibility-guided treatment.Efficacy was compared with that of the empiric treatment group.RESULTS A total of 132 individuals tested positive for the H.pylori ureA gene by qPCR technique.CLA resistance rate reached a high level of 82.6%(n=109),LFX resistance rate was 69.7%(n=92)and dual resistance was 62.1%(n=82).Gastric symptoms[odds ratio(OR)=2.782;95%confidence interval(95%CI):1.076-7.194;P=0.035]and rural residence(OR=5.152;95%CI:1.407-18.861;P=0.013)were independent risk factors for secondary resistance to CLA and LFX,respectively.A total of 102 and 100 individuals received susceptibility-guided therapies and empiric treatment,respectively.The antibiotic susceptibility-guided treatment and empiric treatment groups achieved successful eradication rates of 75.5%(77/102)and 59.0%(59/411)by the intention-to-treat(ITT)analysis and 90.6%(77/85)and 70.2%(59/84)by the per-protocol(PP)analysis,respectively.The eradication rates of these two treatment strategies were significantly different in both ITT(P=0.001)and PP(P=0.012)ana

Constructed wetlands(CWs)are widely used to treat secondary effluent.However,simultaneously removing ammonia(NH_(4)^(+)-N)and nitrate(NO_(3)^(–)–N)is challenging because of insufficient oxygen and carbon sources.In this study,a novel composite material(MPCM)comprising MnO2 and polycaprolactone was developed as a substrate for CWs to enhance the synchronous removal of NH_(4)^(+)–N and NO_(3)^(–)–N.The CWs with a higher MPCM content(H-CW),lower MPCM content(LCW),and controlled CW(C-CW)exhibited average NH_(4)^(+)–N removal efficiencies of 75.69%,70.49%,and 52.40%,respectively.The 15N isotope tracking technique showed that NH_(4)^(+)–N removal was attributed to anaerobic ammonia oxidation mediated by MnO2 reduction(Mnammox),which accounted for 17.16%–27.24%of the NH_(4)^(+)–N removal in the composite material layers(0–20 cm)of the H-CW and L-CW.The richness of ammonia oxidizers in the upper layers(40–50 cm)of the HCW and L-CW further facilitated NH_(4)^(+)–N removal.Moreover,the average total nitrogen(TN)removal efficiencies of the H-CW and L-CW were 1.99 and 1.59 times that of C-CW,respectively,owing to enhanced denitrification by MPCM.Furthermore,N2O emissions were reduced by 81.31%and 70.83%in the H-CW and L-CW,respectively.This study provides an effective approach for improving nitrogen removal and reducing N2O emissions during the treatment of secondary effluent by CWs.

As a highly invasive carcinoma,esophageal cancer(EC)was the eighth most prevalent malignancy and the sixth leading cause of cancer-related death worldwide in 2020.Esophageal squamous cell carcinoma(ESCC)is the major histological subtype of EC,and its incidence and mortality rates are decreasing globally.Due to the lack of specific early symptoms,ESCC patients are usually diagnosed with advanced-stage disease with a poor prognosis,and the incidence and mortality rates are still high in many countries,especially in China.Therefore,enormous challenges still exist in the management of ESCC,and novel strategies are urgently needed to further decrease the incidence and mortality rates of ESCC.Although the key molecular mechanisms underlying ESCC pathogenesis have not been fully elucidated,certain promising biomarkers are being investigated to facilitate clinical decision-making.With the advent and advancement of highthroughput technologies,such as genomics,proteomics and metabolomics,valuable biomarkers with high sensitivity,specificity and stability could be identified for ESCC.Herein,we aimed to determine the epidemiological features of ESCC in different regions of the world,especially in China,and focused on novel molecular biomarkers associated with ESCC screening,early diagnosis and prognosis prediction.

In order to study the influence of various sintering variables more intuitively and clearly,electromagnetic field analysis and computational fl uid dynamics are used to analyze the infl uence of gas flow,heating power and other process parameters on the temperature range of medium-frequency sintering in this paper.The results indicate that the uniformity of temperature fi eld is determined by the gas flow rate and heating power.When the heating power is 50 kW,the stable temperature drops about 10 K for every 1 m^(3)·h^(−1)increase of hydrogen flow.The peak value of the maximum temperature diff erence on the tungsten rods gradually increases with the increase of the hydrogen flow rate,and it appears slightly later.The temperature distribution of tungsten rods in furnace is similar with diff erent hydrogen flow rate.At the same time,the closer to the furnace wall,the higher the temperature of the tungsten rod.The temperature diff erence between tungsten rods mainly occurred within 1 h after the beginning of heating.Increasing the heating power can improve the stable temperature,but has little eff ect on the heating time.Increasing heating power can increase the peak value of the maximum temperature diff erence on the tungsten rods,but has little eff ect on the occurrence time of the peak value of the maximum temperature diff erence on the tungsten rods.Increasing the heating power will increase the temperature diff erence of a single tungsten rod,but the temperature diff erence of a single tungsten rod never exceeds 65 K.This study can be used to guide the process optimization design.Combined with numerical simulation calculation,the sintering parameters with low energy consumption and high temperature uniformity can be quickly and eff ectively found,thus improving the microstructure and properties of sintered tungsten products.

Multidrug-resistant(MDR)Enterobacteriaceae critically threaten duck farming and public health.The phenotypes,genotypes,and associated mobile genetic elements(MGEs)of MDR Enterobacteriaceae isolated from 6 duck farms in Zhejiang Province,China,were investigated.A total of 215 isolates were identified as Escherichia coli(64.65%),Klebsiella pneumoniae(12.09%),Proteus mirabilis(10.23%),Salmonella(8.84%),and Enterobacter cloacae(4.19%).Meanwhile,all isolates were resistant to at least two antibiotics.Most isolates carried tet(A)(85.12%),blaTEM(78.60%)and sul1(67.44%)resistance genes.Gene co-occurrence analysis showed that the resistance genes were associated with IS26 and integrons.A conjugative IncFII plasmid pSDM004 containing all the above MGEs was detected in Proteus mirabilis isolate SDM004.This isolate was resistant to 18 antibiotics and carried the blaNDM-5 gene.MGEs,especially plasmids,are the primary antibiotic resistance gene transmission route in duck farms.These findings provide a theoretical basis for the rational use of antibiotics in farms which are substantial for evaluating public health and food safety.

Wood,a readily available and sustainable natural resource,has found widespread use in construction and furniture.However,its inherent flammability poses a potential fire risk.Although intumescent fire-retardant coatings effectively mitigate this risk,achieving high transparency in such coatings presents a significant challenge.In our approach,we employed a cross-linked network of phytic acid anion and N-[3-(trimethoxysilyl)propyl]-N,N,N-trimethylammonium cation to create a transparent"three-in-one"intumescent coating.The collaborative P/N/Si flame-retardant effect markedly improved the intumescent char-forming capability,preventing the wood from rapid decomposition.This resulted in a substantial reduction in heat release(13.9%decrease in THR)and an increased limiting oxygen index(LOI)value of 35.5%.Crucially,the high transparency of the coating ensured minimal impact on the wood's appearance,allowing the natural wood grains to remain clearly visible.This innovative approach provides a straightforward method for developing transparent intumescent flame-retardant coatings suitable for wooden substrates.The potential applications extend to preserving ancient buildings and heritage conservation efforts.

BACKGROUND Atherosclerosis(AS),a chronic inflammatory disease of blood vessels,is a major contributor to cardiovascular disease.Dental pulp stem cells(DPSCs)are capable of exerting immunomodulatory and anti-inflammatory effects by secreting cytokines and exosomes and are widely used to treat autoimmune and inflam-mation-related diseases.Hepatocyte growth factor(HGF)is a pleiotropic cytokine that plays a key role in many inflammatory and autoimmune diseases.AIM To modify DPSCs with HGF(DPSC-HGF)and evaluate the therapeutic effect of DPSC-HGF on AS using an apolipoprotein E-knockout(ApoE-/-)mouse model and an in vitro cellular model.METHODS ApoE-/-mice were fed with a high-fat diet(HFD)for 12 wk and injected with DPSC-HGF or Ad-Null modified DPSCs(DPSC-Null)through tail vein at weeks 4,7,and 11,respectively,and the therapeutic efficacy and mechanisms were analyzed by histopathology,flow cytometry,lipid and glucose measurements,real-time reverse transcription polymerase chain reaction(RT-PCR),and enzyme-linked immunosorbent assay at the different time points of the experiment.An in vitro inflammatory cell model was established by using RAW264.7 cells and human aortic endothelial cells(HAOECs),and indirect co-cultured with supernatant of DPSC-Null(DPSC-Null-CM)or DPSC-HGF-CM,and the effect and mechanisms were analyzed by flow cytometry,RT-PCR and western blot.Nuclear factor-κB(NF-κB)activators and inhibitors were also used to validate the related signaling pathways.RESULTS DPSC-Null and DPSC-HGF treatments decreased the area of atherosclerotic plaques and reduced the expression of inflammatory factors,and the percentage of macrophages in the aorta,and DPSC-HGF treatment had more pronounced effects.DPSCs treatment had no effect on serum lipoprotein levels.The FACS results showed that DPSCs treatment reduced the percentages of monocytes,neutrophils,and M1 macrophages in the peripheral blood and spleen.DPSC-Null-CM and DPSC-HGF-CM reduced adhesion molecule expression in tumor necrosis factor-αstimulated HAOECs and regul

Integrating electrochemical reduction of CO_(2)and electrochemical oxidation to recycle degraded superalloys is a promising solution to ease resource scarcity and safeguard environmental sustainability.Herein,we propose an electrochemical technique for the conversion of bulk superalloy scraps and CO_(2)into oxide powder at the anode and solid carbon at the cathode,respectively.In particular,a borax-modifi ed CaCl_(2)-based molten salt electrolyte is used for enhancing the electrochemical oxidation of superalloy scraps.At a temperature of 700℃and a voltage of 2.8 V,90.55 wt.%of alloy scraps were oxidized in a molten CaCl_(2)–NaCl–CaCO_(3)–Na_(2)B_(4)O_(7)with an acid–base ratio(K_(a/b))of 1.The synergy of Cl−and B_(4)O_(7)2−of electrolyte prevents the passivation of the alloy anode and enables continuous oxidation.Furthermore,the Ni and Co in the anode products are recovered by sulfation roasting with recovery efficiencies of 85.58%and 95.27%for Ni and Co,respectively.Overall,modulating the alkalinity of the electrolyte for enhancing oxidation/pulverization of alloy scrap anode provides new insight into electrochemically recovering superalloy scraps.

Analysis on characteristics of extreme precipitation indices and atmospheric circulation in Northern Shanxi

This article utilizes daily precipitation data from 28 national meteorological stations in northern Shanxi Province spanning from 1972 to 2020, and the US NCEP/NCAR monthly average reanalysis and ERA5 monthly average reanalysis data. The study employs techniques such as empirical orthogonal function(EOF) decomposition, MannKendall mutation and other methods to investigate the spatiotemporal distribution of extreme precipitation index in northern Shanxi and their correlation with atmospheric circulation. The research results show that: the absolute index, relative index, intensity index and sustained dry period index(CDD) in the continuous index appear from southwest to northeast. The spatial distribution characteristics of the central region decrease, while the continuous wet period(CWD) decreases from the central to the east and west. The three indices Rx1day, Rx5day, and CWD mutated in 1978, 1975, and 1983 respectively, and other extreme precipitation indices all appeared in a sudden change from a low-value period to a high-value period occurred around 2010. In the high-value years of the summer extreme precipitation index, there is a significant negative anomaly in the height field in the mid-high latitude regions of Eurasia. Northern Shanxi is controlled by a broad low-pressure trough in the Lake Baikal area.Water vapor transported via the east, west, and south routes converges in the northern Shanxi region and encounters cold air from the north. There is a strong upward motion anomaly at 500 hPa in the troposphere, and the dynamic conditions of upper-level divergence and lower-level convergence lead to more summer extreme precipitation in the northern Shanxi region. Conversely, in the low-value years of the summer extreme precipitation index, northern Shanxi is affected by a strong high-pressure ridge north of Lake Baikal.
