BACKGROUND: Fast track strategy in the management of patients undergoing intra-abdominal surgery of various types has emerged as a landmark approach to reduce surgical stress and accelerate recovery. This study was to evaluate the effect of fast track strategy on patients subjected to pancreaticoduo-denectomy (PD) from an individual unit during transit from low to a high volume center. METHODS: A total of 142 PD patients who had been subjected to fast track strategy between June 2008 and September 2012 were compared with 46 patients who had received convention-al surgery between January 2006 and May 2008. Comparative analysis was made of postoperative complications, postopera-tive recovery, length of hospital stay and patient readmission requirement. RESULTS: The patients subjected to fast track strategy had a faster recovery and a shorter hospital stay than those who were treated conventionally (7.8 vs 12.1 days). The intraoperative events like operative blood loss (417.9±83.8 vs 997.4 ±151.8 mL, P CONCLUSIONS: This preliminary analysis suggests that the fast track approach might be beneifcial to the well-being of the patients after PD, for it accelerates the immediate clinical recovery of patients and signiifcantly shortens their length of hospital stay.

Morpho-tectonic study plays an important role in deciphering the effects of tectonic activity in the geomorphic evolution of the drainage *** watershed forms one of the major watersheds of the intermontane Karewa Basin of Kashmir *** Karewa sediments are characterized by glacio-fluvio-lacustrine deposits capped by the aeolian *** geomorphic,morphometric and lithostratigraphic studies of these cap deposits have been carried out to elucidate the effect of tectonics on the geomorphic evolution of Romushi *** mapping was carried out using GPS measurements,DEM at 30m resolution,Topographic Position Index(TPI) model,topographic maps,LANDSAT TM Imagery and field *** and morphotectonic analyses in GIS environment were used to calculate various geomorphic indices(Mountain Front Sinuosity Index,Bifurcation Ratio,Asymmetry Factor,River Profile,etc).These indices reveal that the tectonic uplift observed in the region due to Himalayan orogeny coupled with mass movement and aeolian deposition have dominated the landscape evolution of intermontane Karewa Basin of Kashmir throughout the Late Quaternary *** data from lithostratigraphic measurements were analyzed to understand the geomorphic evolution of intermontane Karewa *** data revealed that the basin has experienced differential uplift and erosion rates from time to time in the geological *** was corroborated by the results from the morphometric and morphotectonic analysis.

Quantitative glacial chronologies of past glaciations are sparse in the Himalaya, and mostly absent in the Kashmir Himalaya. We used cosmogenicBe exposure dating, and geomorphological mapping to reconstruct glacial advances of the Thajwas Glacier(TG) in the Great Himalayan Range of the Kashmir Himalaya. FromBe exposure dating of ten moraine boulders, four glacial stages with ages ~20.77 ±2.28 ka, ~11.46 ± 1.69 ka, ~9.12 ± 1.39 ka and ~4.19 ± 0.78 ka, were identified. The reconstructed cosmogenic radionuclide ages confirmed the global Last Glacial Maximum(g LGM), Younger Dryas, Early Holocene, and Neoglaciation episodes. As per area and volume change analyses, the TG has lost 51.1 km~2 of its area and a volume of 2.64 km~3 during the last 20.77 ± 2.28 ka. Overall, the results suggested that the TG has lost 64% of area and 73% of volume from the Last glacial maximum to Neoglaciation and about 85.74% and 87.67% of area and volume, respectively, from Neoglaciation to the present day. The equilibrium line altitude of the TG fluctuated from 4238 m a.s.l present to3365 m a.s.l during the g LGM(20.77 ± 2.28 ka). The significant cooling induced by a drop in mean ambient temperature resulted in a positive mass balance of the TG during the g LGM. Subsequently the melting accelerated due to the continuing rise of the global ambient temperature. Paleo-glacial history reconstruction of the Kashmir Himalaya, with its specific geomorphic and climatic setting, would help close the information gap about the chronology of past regional glacial episodes.

Background： Objectives were to examine the effects of selenium （Se） supply and maternal nutritional plane during gestation on mammary gland growth, cellular proliferation, and vascularity at parturition and d 20 of lactation. Rambouillet primiparous ewes （n = 84） were allocated to treatments in a 2 x 3 factorial. Factors were dietary Se （adequate Se [ASe, 11.5 μg/kg BW] or high Se [HSe, 77.0 μg/kg BVV]） and nutritional plane （60% IRES], 100% [CON], or 140% [EXC]）. At parturition, lambs were removed and 42 ewes （7/treatment） were necropsied. Remaining ewes were fed a common diet meeting requirements for lactation and mechanically milked twice daily until necropsy on d 20. At both necropsy periods, mammary glands were dissected and tissues harvested. Samples were analyzed for RNA, DNA, and protein content, cell proliferation, and vascularity. Where interactions were present （P 〈 0.05）, least squares means from the highest-order interaction are presented. Results： Final body weight of ewes was least （P 〈 0.002） in RES, intermediate for CON, and greatest for EXC, regardless of stage of the ewe at necropsy （parturition or d 20 of lactation）. In ewes necropsied at parturition, mammary glands were heavier （P = 0.02） in EXC compared to RES, with CON intermediate. Concentration of RNA （rag/g） was decreased （P= 0.01） in EXC compared to CON at parturition. There was a tendency （P= 0.07） for a Se by nutrition interaction in percentage of cells proliferating where ASe-EXC ewes had greater （P_〈 0.02） number of proliferating cells then all other treatments. Mammary vascular area tended （P = 0.08） to be affected by a Se by nutrition interaction where ASe-CON had less （P= 0.007） vascular area than HSe-CON ewes. In ewes necropsied at d 20 of lactation, the number of alveoli per area was decreased （P 〈- 0.05） in RES compared to CON and EXC-fed ewes. We introduce a new family of classes of operators termed as *p-paranormal operator, classes *A(p,p);p > 0 and *A(p,q);p, q > 0, parallel to p-paranormal operator and classes A(p,p);p> 0 and A(p,q);p, q > 0 introduced by M. Fujii, D. Jung, S. H. Lee, M. Y. Lee and R. Nakamoto [1]. We present a necessary and sufficient condition for p-hyponormal operator T∈B(H)to be *p-paranormal and the monotonicity of *A(p,q). We also present an alternative proof of a result of M. Fujii, et al. [1, Theorem 3.4].

Imaging department is an important department of a hospital contributing directly to patient care, providing diagnostic support to all specialties which cannot practice efficiently without their support. Hospital administrators are looking for newer tools to control costs without affecting the quality of patient care. It is well known that the escalation of costs for advanced technology has been dramatic and it has been labeled as one of the culprits for great increase in healthcare costs. A prospective study for a period of six months was carried out for calculation of unit cost of radiological investigations CT head, CT chest, CT abdomen and MRI. Unit costs were computed under direct and indirect costs. The actual cost incurred by the hospital on CT head was Rupees 581.40 (US $10.89), CT abdomen Rupees 2339.20 (US $43.83), CT chest Rupees 2339.20 (US $43.83), and MRI Rupees 4497.50 (US $84.28). However, in the hospital patients are charged Rupees 900 (US $16.86) for CT head, Rupees 1200 (US $22.48) for CT abdomen, Rupees 1200 (US $22.48) for CT chest and Rupees 2500 (US $46.85) for MRI. There is a substantial loss of revenue because of subsidies provided to patients in a tertiary care teaching hospital which needs revision of charges.

Nonunion of the humerus in a severely osteoporotic bone is a likely event especially if the fracture is transverse. The management of such a combination is a challenge. Most of the conventional fixation methods are unlikely to succeed as the bone failure precedes implant failure in osteoporosis. The challenge is further compounded in severe osteoporosis when the cortical thickness is affected more severely. We used a coi＇nbination of an intramedullary fibula with a locking plate in 5 cases. The results show that it may be a good combination in such situations as the bone strength is augmented and the plate pullout is less likely.
