Endoscopic identification of endoluminal esophageal landmarks for radial and longitudinal orientation and lesion location

AIM To characterize esophageal endoluminal landmarks to permit radial and longitudinal esophageal orientation and accurate lesion *** Distance from the incisors and radial orientation were estimated for the main left bronchus and the left atrium landmarks in 207 consecutive patients using white light examination. A sub-study was also performed using white light followed by endoscopic ultrasound(EUS) in 25 consecutive patients to confirm the *** scope orientation throughout the exam was maintained at the natural axis,where the left esophageal quadrant corresponds to the area between 6 and 9 o'clock. When an anatomical landmark was identified, it was recorded with a photograph and its quadrant orientation and distance from the incisors were determined. The reference points to obtain the distances and radial orientation were as follows: the midpoint of the left main bronchus and the most intense pulsatile zone of the left atrium. With the video processor system set to moderate insufflation, measurements were obtained at the end of the patients' air *** The left main bronchus and left atrium esophageal landmarks were identified using white light in 99% and 100% of subjects at a mean distance of 25.8 cm(SD2.3), and 31.4 cm(SD 2.4) from the incisors, respectively. The left main bronchus landmark was found to be a tubular, concave, non-pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. The left atrium landmark was identified as a round, convex, pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. Both landmarks were identified using white light on the anterior esophageal quadrant. Endoscopic cryotherapy is a technique utilized for the ablation of target tissue within the gastrointestinal tract.A cryotherapy system utilizes the endoscopic application of cryogen such as liquid nitrogen,carbon dioxide or liquid nitrous *** leads to disruption of cell membranes,apoptosis,and thrombosis of local blood vessels within the target *** trials utilizing cryotherapy for Barrett’s esophagus(BE)with variable dysplasia,gastric antral vascular ectasia(GAVE),esophageal carcinoma,radiation proctitis,and metastatic esophageal carcinomas have shown safety and *** recently,liquid nitrogen cryotherapy(cryodilation)was shown to be safe and effective for the treatment of a benign esophageal stricture which was refractory to dilations,steroid injections,and ***,liquid nitrogen cryotherapy is associated with less post procedure pain as compared to radiofrequency ablation in BE with comparable ablation *** patients with GAVE,cryotherapy was found to be less tedious as compared to argon plasma *** events from cryotherapy most commonly include chest pain,esophageal strictures,and *** perforations did occur as well,but less *** summary,endoscopic cryotherapy is a promising and growing field,which was first demonstrated in BE,but the use now spans for several other disease *** randomized controlled trials are needed before its role can be established for these different diseases.

Transformation of leather waste into microwave-activated and N-doped carbon for Li-S batteries

The use of carbon from waste biomass has the potential to buffer the drawbacks of Li-S batteries and improve their overall performance. Chrome-Tanned-LeatherShavings(CTLS) are undesirable wastes, highly available for transformation into porous carbon. Microwave activation is an efficient state-of-the-art procedure to obtain unique porous structures and nitrogen incorporation can improve the performance of porous carbons. This study proposes the transformation of CTLS into a microwave activated porous carbon, and then was modified to introduce nitrogen. A microporous activated carbon with a specific surface area of 556 m2g-1 with honeycomb-like structure was obtained. Both urea thermal decomposition and ethanolamine impregnation improves the nitrogen and amine presence of surface groups. However,the results indicated that only urea treatment was effective in improving the initial capacity of the cell(1363 mAhg-1), which is probably linked with the sorption of long-chain polysulfides. This investigation confirms that it is possible to obtain carbon materials from CTLS, for use as sulfur-host cathodes in Li-S batteries, and improve their performance with urea thermal decomposition. Furthermore, a Radial basis function neural network was fitted to provide statistical support to the experimental results. The trained model corroborated the importance of nitrogen content of the carbons in the discharge capacity of the cells.

Profitability Analysis of Various Maize Value-Added Products in the North-West Region of Cameroon

Maize value-added products play a crucial role in reducing post-harvest losses, enhancing food security, and generating income. While extensive research has focused on maize production in Cameroon, the exploration of its value-added products and their profitability in the North-West Region remains underexplored. This study examined the profitability of maize value-added products in Mezam Division, with the objectives to: 1) identify various maize-based products, 2) assess the diversity of these products, 3) conduct a cost-benefit analysis of selected products, 4) examine the relationship between profitability and product diversity, and 5) identify key constraints impacting profitability. To achieve these objectives, structured questionnaires were administered to 500 small-scale maize entrepreneurs randomly selected from five subdivisions. Descriptive statistics were used to analyze objective 1 and 5, while the Shannon Diversity Index was employed to assess product diversity. Additionally, a cost-benefit analysis was conducted on four selected products namely pap, parched corn, peeled parboiled corn, and corn beer, and a correlation analysis was used to examine objective 4. In total, 13 maize value-added products were identified, with a diversity index of 4.4. The total cost of processing the four selected products per entrepreneur using 18 kg of maize per product was FCFA 83631.5 (US $132.75), while the total revenue was FCFA 121864.5 (US $193.43), resulting in an economic profit of FCFA 38,233 (US $60.69). Pap emerged as the most profitable product, with an economic profit of FCFA 27,875 (US $44.24), while corn beer was the least profitable, with an economic profit of FCFA 2133.46 (US $3.39). The correlation analysis revealed a strong negative relationship between product diversity and profitability (r = −0.91), indicating that entrepreneurs can maximize profitability by focusing on a few high-demand products like pap and parched corn. The cubic spline integration scheme has been successfully applied for the first time to obtain solutions for the two-dimensional shallow water wave and transport equations. Its principal advantages are its high accuracy, low computational cost, and flexibility of application. The study quantitatively analyzed and predicted the variation in time and space of a solid pollutant in water environment. The infiltration, adsorption, solubility, advection, and diffusion related factors at different spatial and temporal scales were taken into consideration in the model. Experiments were conducted in this study to examine the capabilities of the cubic spline scheme, The correspondence between the numerical results and the experimental data was fair, The results were also compared with the numerical results obtained using finite difference schemes with good agreement.

Hepatic organ protection:From basic science to clinical practice

Hepatic ischemia and reperfusion (I/R) injury during liver surgery is still the main cause of postoperative liver failure and the subsequent rise of mortality in these patients. During the last few years, a multitude of underlying mechanisms have been extensively characterized and many different protective approaches have been evaluated under experimental conditions. Some of them have already found their way into small sized clinical trials. In this Topic Highlight series of articles, we present recent insights into promising protective concepts including the regulation and optimization of hepatic blood flow, molecular mechanisms of preconditioning and pharmacological approaches with the aim of limiting hepatic I/R injury. Leading international experts present the latest experimental evidence in their fields stressing clinically relevant ideas, which are now on the edge of entering clinical practice.

Do people communicate about their whereabouts? Investigating the relation between user-generated text messages and Foursquare check-in places

The social functionality of places(***,restaurant)partly determines human behaviors and reflects a region’s functional *** descriptions of places are thus valuable to a range of studies of humans and geographic *** their potential impacts on human verbalization behaviors,one possibility is to link the functions of places to verbal representations such as users’postings in location-based social networks(LBSNs).In this study,we examine whether the heterogeneous user-generated text snippets found in LBSNs reliably reflect the semantic concepts attached with check-in *** investigate Foursquare because its available categorization hierarchy provides rich a-priori semantic knowledge about its check-in places,which enables a reliable verification of the semantic concepts identified fromuser-generated text snippets.A latent semantic analysis is conducted on a large Foursquare check-in *** results confirm that attached text messages can represent semantic concepts by demonstrating their large correspondence to the official Foursquare venue *** further elaborate the representativeness of text messages,this work also performs an investigation on the textual terms to quantify their abilities of representing semantic concepts(i.e.,representativeness),and another investigation on semantic concepts to quantify how well they can be represented by text messages(i.e.,representability).The results shed light on featured terms with strong locational characteristics,as well as on distinctive semantic concepts with potentially strong impacts on human verbalizations.

Taking advantage of drug resistance, a new approach in the war on cancer

Identification of the driver mutations in cancer has resulted in the development of a new category of molecularly targeted anti-cancer drugs. However, as was the case with conventional chemotherapies, the effectiveness of these drugs is limited by the emergence of drug-resistant variants. While most cancer therapies are given in combinations that are designed to avoid drug resistance, we discuss here therapeutic approaches that take advantage of the changes in cancer cells that arise upon development of drug resistance. This approach is based on notion that drug resistance comes at a fitness cost to the cancer cell that can be exploited for therapeutic benefit. We discuss the development of sequential drug therapies in which the first therapy is not given with curative intent, but to induce a major new sensitivity that can be targeted with a second drug that selectively targets the acquired vulnerability. This concept of collateral sensitivity has hitherto not been used on a large scale in the clinic and holds great promise for future cancer therapy.
