Gastric necrosis is a rare condition because of the rich blood supply and the extensive submucosal vascular network of the stomach. "Gas-bloat" syndrome is a well known Nissen fundoplication postoperative complica-tion. It may cause severe gastric dilatation, but very rarely an ischemic compromise of the organ. Other factors, such as gastric outlet obstruction, may concur to cause an intraluminal pressure enough to blockade venous return and ultimately arterial blood supply and oxygen deliver, leading to ischaemia. We report a case of a 63-year-old women, who presented a total gastric necrosis following laparoscopic Nissen fundoplication and a pyloric phytobezoar which was the trigger event. No preexisting gastric motility disorders were present by the time of surgery, as demonstrated in the preop-erative barium swallow, thus a poor mastication(patient needed no dentures) of a high fiber meal(cabbage) may have been predisposing factors for the develop-ment of a bezoar in an otherwise healthy women at the onset of old age. A total gastrectomy with esophagojejunostomy was performed and patient was discharged home after a 7-d hospital stay with no immediate com-plications. We also discuss some technical aspects of the procedure that might be important to reduce the incidence of this complication.

Daily Total Column Ozone (TCO) measurements compiled from Total Ozone Mapping Spectrometer (TOMS) and Ozone Monitoring Instruments (OMI) were used to analyze the global and hemispherical TCO interannual variations. Two periods of TCO measurements were analyzed separately covering full years. For the 1978-1994 period, the TCO showed a global decade decrease rate of 13.45 DU (about -4.3%). For the Northern Hemisphere(NH) the decade decrease rate was of 12.96 DU (-4.0%), while in the Southern Hemisphere (SH) was of 13.57 DU (-4.5%). These decreases in ozone trends, using the totality of TOMS and OMI satellite measurements, are greater than those reported in literature. The 1998-2014 period global TCO decade decrease rate was of 1.56 DU, corresponding 0.94 DU and 0.138 DU for the NH and SH, respectively. The global TCO variations must show a double annual periodicity, the first one with maxima in March due to the Northern Hemisphere (NH) and the second one during September due to the Southern Hemisphere (SH). However, the maxima due to SH TCO interannual variations have gradually vanished. A disturbance in the SH TCO interannual variations has appeared since 1980;graphically the periodicity brakes down and transforms to a double peak from 1985 and on. This effect can be attributed to the hemispheric impact of the ozone hole at the South Pole. Between October 1, 2004 and December 14, 2005 TOMS and OMI have recorded this disturbance unequivocally. We conclude that the disturbance in SH TCO has an irreversible character.

The beet (Beta vulgaris L.) vegetable is of great importance in the Brazilian market, mainly by having high nutritional values in consumer diet. Thus, the objective of this work was to evaluate the effects of organic and mineral fertilization on the production and post-harvest quality of beet. The experiment was conducted in the field, in the municipality of Urutaí-GO. The experimental design was a randomized block (DCB), where treatments were applied to two types of fertilizer: organic (chicken manure) and conventional (mineral fertilizer) with six replicates (using six tubers per plot). The following evaluations were made: horizontal and vertical diameters of the tubers (cm) weight (kg) Protein (g) Lipid (%) Ash humidity (%), electrical conductivity (μS·cm-1), pH, Titratable Acidity and Brix. The organic surpassed conventional treatment for all variables, except for the ash content, wherein the organic treating poultry litter base, showed 1.40% of ash and pH (6.48).

This work aimed to evaluate the effects of different individual heights of cut on structural characteristics of Brachiaria hibrida cv. Mulato I in four periods of the year. The study was carried out at the Fazenda Experimental Vale do Acaraú, in Sobral, Ceará, Brazil, from January to December, 2008. The experimental design was a split plot, with the plots representing four pasture conditions through manual cuts of 10 cm, 20 cm, 30 cm and 40 cm heights, which were done every 30 days. Each treatment had three replications. The subplots were represented by four periods of evaluation: January-February-March, April-May-June, July-August-September and October-November-December. Fertilization was performed with 350 kg of N/ha, divided into monthly applications. Irrigation was conducted in alternate days during the dry season. The variables studied were: mean height before cutting (cm);dry matter content (%);total dry matter production (t/ha);percentage of leaves;percentage of stems;percentage of dead material;leaf blade/stem ratios;number of total leaves per tiller;number of live leaves per tiller;average size of leaves;and tiller density. Cutting at 10 cm caused a decrease in total dry matter production, underestimating the potential production. Cutting heights between 20 and 30 cm contributed positively to increase the total number of leaves, number of green leaves, average size of the leafs and total dry matter production. Under the present experimental conditions, cutting around 40 cm was not recommended because of a decreasing in the leaf/stem ratio and a increasing in stem fraction and dry matter content, compromising the structure of the canopy.

Infective Endocarditis with Negative Cultures:A Defiant Diagnosis

The diagnosis and management of blood culture-negative endocarditis constitute a real clinical challenge and a systemic approach is necessary for a successful outcome. The authors report a case of a female patient aged 26, with previous clinical history of valve disease and heart failure NYHA class II, with decompensation of co-morbidities associated with fever, productive cough, nonselective anorexia and unquantified weight loss with one month of evolution with negative blood cultures. Transesophageal echocardiography revealed the presence of severe pulmonary hypertension and mitral valve vegetations in the context of positive serology for Q fever. Herein, the authors report a case of blood culture-negative endocarditis and present a brief review on the management of this medical condition. We highlighted the diagnostic difficulties of blood culture-negative endocarditis and subacute clinical presentation, which sometimes present with fever of unknown origin and complaints of deterioration of cardiac function, thus creating a challenging differential diagnosis.

Many gastrointestinal complaints and motility disorders are described in patients suffering from different eating disorders. These have a negative impact on refeeding. This report evaluated–on admission-gastric emptying of a standardized solid meal with 13C octanoic acid breath test in anorexic adolescents. The results were compared with age and gender matched controls. Gastric emptying data were related to a subjective symptom score. 21 Anorexic girls and 3 boys (mean age: 15.6 y ± 1.3 y), body mass index (mean 15.6 kg/m2 ± 1.6 kg/m2), weight loss (mean: 22.2% ± 10.7%) were studied. T? of the gastric emptying time exceeded the P95 in 14/24 (58%) patients, classified as having delayed gastric emptying (DGE). One patient (4%) had a T? exceeding P75 and was classified as slow (SGE), whereas 9/24 (38%) patients had normal gastric emptying (NGE). In comparison to NGE, subjects with DGE had significantly higher symptom scores (p = 0.01) and more weight loss. The DGE and SGE patients lost 25.29% and 21.38% of weight respectively, whereas a weight loss of 17.64% was found in anorexics with NGE. In conclusion, gastric emptying is delayed in patients with anorexia. This is associated with more significant gastrointestinal symptoms and higher weight loss.

The prevalence of pediatric obesity continues to increase worldwide,bringing with it various metabolic,functional,social,and psychological complications.1 Both the prevention and treatment of childhood obesity must be based on multidisciplinary approaches combining nutrition,physical activity(PA),and psychological support.2 As an essential element of these multicomponent strategies,regular physical activity has been acknowledged as having beneficial effects on children's and adolescents,body composition,physical fitness,and metabolic profile,as well as on their health-related quality of life,social and psychological health,and academic achievement.3,4 Given that only a relatively small proportion of children and adolescents meet the public health recommendations for PA,5 and given that weight loss exercise-based interventions suffer from a high attrition rate,there is a clear need for appropriate PAs.

A New Biomaterial for Urinary Catheters

Several studies argue that an ideal biomaterial for urinary catheters is utopian. Based in literature review it seems to be true. However, research advances: the biomaterial itself, new designs, new coatings, associated drugs, etc. Once implanted and interacting with urine, two old problems persist: encrustation and bacterial colonization. In this context, an extracellular product from bacterial synthesis on sugarcane molasses biomaterial has been studied in several experimental and clinical studies. Based on its high biocompatibility, the aim of this study is to evaluate its performance in an in vivo model as an endourologic prosthesis implanted in the bladder of Wistar rats. We evaluate physical, chemical and biological phenomena in comparison to an already established biomaterial, polyurethane. Even though it is not a finished product, the sugarcane biopolymer presented similar performance compared to polyurethane in several analyzed parameters and has an important characteristic: low cost.

近年来,为了满足提高高炉利用系数等的需要,高炉炉缸铁水温度有所提高,这就导致了炉缸耐火材料内衬的极度磨损。虽然目前有很多关于炉缸耐火材料及其性能对比试验的报道,但几乎没有关于这些材料的反应机理和进一步变质方面的报道。对比了两种不同碳含量材料实验室实验结果,弄清了炉缸内衬耐火材料的各种反应机理和改善其恶化的措施。根据炉缸内衬的磨损机理,提出一些实际操作措施,以延长高炉寿命。
