Background:Cutaneous leishmaniasis(CL),endemic in Guatemala,mostly affects poor people living in the northern region.A national control program that ineludes surveillanee,diagnose,and treatment offered free of cost by the Ministry of Health(MoH)has been in place since 2003.However,the incidence is increasing and treatment rates are not optimal,suggesting that current efforts are not being effective.This study aimed to understand barriers and facilitators of CL control in Guatemala as experienced and perceived by key stakeholders in order to comprehend what works well and does not and suggest evidence-informed interventions.Methods:The study was conducted in the Coban municipality,the most endemic of Guatemala,situated in the Department of Alta Verapaz.Data were collected during May and June 2019 via focus groups and semi-structured interviews with key stakeholders,including local and national health personnel and residents of four communities of the endemic region.Thematic and content analysis of the collected data was conducted using NVIVO.Results:Three overarching issues hamper the effectiveness of current CL efforts:resource scarcity,treatment challenges,and knowledge-action gaps.Scarce economic resources from the MoH and community residents negatively impact incidence,detection of cases and treatment rates in that preventive action is insufficient and healthcare access is low.In addition,local health workers often lack specialized CL training and access to the national CL control guidelines.With regards to the population living in the study area,misunderstanding of disease causation,shame associated with CL lesions,treatment pain fear,and long(often uncertain)waiting times for diagnose and treatment negatively affect people's willingness to seek help,treatment adherence,and their trust on the healthcare provided.Conclusions:Culturally sensitive CL preventive action must be developed.Given the scarce economic resources available for CL control in the country,the involvement of trained community health workers

AbstractWe extend the （outer） measureγIassociated to an operator ideal I to a measureγJfor bounded bilinear operators.If I is surjective and closed,and J is the class of those bilinear operators such thatγJ（T）=0,we prove that J coincides with the composition bideal I o B.If I satisfies the ∑r-condition,we establish a simple necessary and sufficient condition for an interpolated operator by the real method to belong to J.Furthermore,if in addition I is symmetric,we prove a formula for the measureγJof an operator interpolated by the real method.In particular,results apply to weakly compact operators.

Background: Use of inappropriate amikacin dose is one of the most important factors in inducing toxicity, prolonged hospitalization as well as in increasing patient’s mortality. Objective: The aims of this study are the analysis of amikacin dose, serum level and the examination of the effectiveness of the clinical pharmacologist (CP) therapeutic drug monitoring (TDM) intervention to guarantee the safety of amikacin use. Methods: This is a one-year retrospective observational chart review study, which evaluates amikacin dose, serum drug level, development of adverse effects in patients on amikacin with or without CP TDM consultation. Results: Amikacin was prescribed for 393 complex patients, with median age 83. Amikacin group (AG) included 140 (32%) courses with CP consultation (AG1) and 292 (68%) courses without CP consultation (AG2). The distribution of most study characteristics in both groups was similar including amikacin dose (9-10 mg/kg/day), renal failure (14%) and mortality (12%). Acceptance for CP consultation was in 46% of amikacin courses and dose changes were done in 63% after CP intervention. Prolonged antibiotic course (4.6 ± 1.5 vs 3.8 ± 1.6 days, p < 0.0001) and the patient’s hemodynamic instability (15% vs 7%, p = 0.01) were more frequent in the AG1 compared to the AG2. There was a strong association between CP consultation and prolonged hospitalization (p = 0.005), while no association between it and amikacin adverse effects, renal failure or mortality. Conclusions: There was no trend to reducing amikacin toxicity, days of hospitaliza tion or mortality in patients with CP consultation. CP TDM intervention was more in the management of complicated clinical situations. However, it is necessary to optimize it.

Biodegradable nanoparticles such as chitosan nanoparticles (CSNPs) are used in sustainable agriculture since theyavoid damage to the environment;CSNPs have positive effects such as the accumulation of bioactive compoundsand increased productivity in plants. This study aimed to investigate the impact of applying CSNPs on lettuce,specifically focusing on enzymatic activity, bioactive compounds, and yield. The trial was conducted using a completelyrandomized design, incorporating CSNPs: 0, 0.05, 0.1, 0.2, 0.4, and 0.8 mg mL−1. The doses of 0.4 mg mL−1improve yields up to 24.6% increases and 0.1 mg mL−1 of CSNPs increases total phenols by 31.2% and antioxidantcapacity by 34.6%. In addition, when low concentrations of CSNPs (0.05 and 0.1 mg L−1) were applied, anincrease in catalase was determined. The CSNPs represent a good alternative to be used as a biostimulant in sustainableagriculture because they improve the yield and quality of lettuce by increasing the bioactive compounds.

Background Obesity is a multifactorial chronic disease with a high,increasing worldwide prevalence.Genetic causes account for 7%of the cases in children with extreme obesity.Data sources This narrative review was conducted by searching for papers published in the PubMed/MEDLINE,Embase and SciELO databases and included 161 articles.The search used the following search terms:"obesity","obesity and genetics","leptin","Prader-Willi syndrome",and"melanocortins".The types of studies included were systematic reviews,clinical trials,prospective cohort studies,cross-sectional and prospective studies,narrative reviews,and case reports.Results The leptin-melanocortin pathway is primarily responsible for the regulation of appetite and body weight.However,several important aspects of the pathophysiology of obesity remain unknown.Genetic causes of obesity can be grouped into syndromic,monogenic,and polygenic causes and should be assessed in children with extreme obesity before the age of 5 years,hyperphagia,or a family history of extreme obesity.A microarray study,an analysis of the melanocortin type 4 receptor gene mutations and leptin levels should be performed for this purpose.There are three therapeutic levels:lifestyle modifications,pharmacological treatment,and bariatric surgery.Conclusions Genetic study technologies are in constant development;however,we are still far from having a personalized approach to genetic causes of obesity.A significant proportion of the affected individuals are associated with genetic causes;however,there are still barriers to its approach,as it continues to be underdiagnosed.

Diseases caused by ischemia are one of the leading causes of death in the world.Current therapies for treating acute myocardial infarction,ischemic stroke,and critical limb ischemia do not complete recovery.Regenerative therapies opens new therapeutic strategy in the treatment of ischemic disorders.Mesenchymal stem cells(MSCs)are the most promising option in the field of cell-based therapies,due to their secretory and immunomodulatory abilities,that contribute to ease inflammation and promote the regeneration of damaged tissues.This review presents the current knowledge of the mechanisms of action of MSCs and their therapeutic effects in the treatment of ischemic diseases,described on the basis of data from in vitro experiments and preclinical animal studies,and also summarize the effects of using these cells in clinical trial settings.Since the obtained therapeutic benefits are not always satisfactory,approaches aimed at enhancing the effect of MSCs in regenerative therapies are presented at the end.

Evidence-Based Complementary and Alternative Medicine Effects of Capybara Oil on Cardiac Remodeling of C57bl/6 Mice

Cardiovascular diseases are the main cause of morbidity and mortality in the world, and obesity and the metabolic syndrome are risk factors for its development. One of the therapies to reduce cardiovascular risk is the use of polyunsaturated fatty acids. In Brazil, a source of such acid is the oil extracted from the fat of the capybara. The objective of this work is to study the effects of the capybara oil on lipid and glucose metabolism, as well as its effects on the adipose tissue and cardiac remodeling. We assessed the effects of capybara oil treatment on body mass, lipid and carbohydrate metabolism, systolic blood pressure, adipose tissue and cardiac remodeling, and performed an ultrastructural evaluation of the myocardium in C57Bl/6 mice treated with high-fat diet. Treatment with capybara oil reduced total cholesterol and triglyceride levels, systolic blood pressure, visceral and subcutaneous adipose tissue, and adipocyte diameter. In addition, cardiac remodeling was attenuated, preserving cardiomyocytes, increasing vascularization, reducing cardiomyocyte hypertrophy and the extracellular matrix, and preserving the morphological integrity of mitochondria. Capybara oil has several beneficial effects on the cardiovascular and metabolic system, and further studies are needed to better understand its role in the prevention or treatment of cardiovascular diseases.

Background:The molecular heterogeneity of Alzheimer’s amyloid-β(Aβ)deposits extends well beyond the clas-sic Aβ1-40/Aβ1-42 dichotomy,substantially expanded by multiple post-translational modifications that increase the proteome diversity.Numerous truncated fragments consistently populate the brain Aβpeptidome,and their homeo-static regulation and potential contribution to disease pathogenesis are largely unknown.Aβ4-x peptides have been reported as major components of plaque cores and the limited studies available indicate their relative abundance in Alzheimer’s disease(AD).Methods:Immunohistochemistry was used to assess the topographic distribution of Aβ4-x species in well-char-acterized AD cases using custom-generated monoclonal antibody 18H6-specific for Aβ4-x species and blind for full-length Aβ1-40/Aβ1-42-in conjunction with thioflavin-S and antibodies recognizing Aβx-40 and Aβx-42 proteo-forms.Circular dichroism,thioflavin-T binding,and electron microscopy evaluated the biophysical and aggregation/oligomerization properties of full-length and truncated synthetic homologues,whereas stereotaxic intracerebral injections of monomeric and oligomeric radiolabeled homologues in wild-type mice were used to evaluate their brain clearance characteristics.Results:All types of amyloid deposits contained the probed Aβepitopes,albeit expressed in different proportions.Aβ4-x species showed preferential localization within thioflavin-S-positive cerebral amyloid angiopathy and cored plaques,strongly suggesting poor clearance characteristics and consistent with the reduced solubility and enhanced oligomerization of their synthetic homologues.In vivo clearance studies demonstrated a fast brain efflux of N-termi-nally truncated and full-length monomeric forms whereas their oligomeric counterparts-particularly of Aβ4-40 and Aβ4-42-consistently exhibited enhanced brain retention.Conclusions:The persistence of aggregation-prone Aβ4-x proteoforms likely contributes to the process of amyloid formation,self-perpetuating the amyloido

On the installation of an in situ large-scale vertical SEALing (VSEAL) experiment on bentonite pellet-powder mixture

Recently,the Institute for Radiological protection and Nuclear Safety(IRSN)has launched VSEAL(Vertical SEALing)project to investigate the impact of gas migration on the long-term performance of bentonite based vertical sealing systems(VSS).The first VSEAL in situ test was emplaced in IRSN’s Underground Research Laboratory(URL)in Tournemire(France)in 2019 and was equipped with 76 wired and wireless sensors.The test is still in progress,but the collected set of data provides already valuable information of the hydro-mechanical behavior of VSS during hydration.The swelling core consists of a mixture of highdensity pellets and powder of MX80 bentonite in a ratio of 80/20(in dry mass).An innovative method was adopted to drill a 1-m diameter and w10-m deep shaft in order to minimize the rock perturbation at the sidewalls.Because a specific protocol was adopted to install the bentonite mixture together with a careful characterization of the core during construction,VSEAL 1 constitutes the unique in situ sealing test with a well-known initial structural distribution of the pellets and the powder.Some heterogeneities occurred within the experiment during the installation process:a damaged zone developed around the shaft walls due to the interruption of the installation operations caused by COVID19 lockdown in France;a technological gap with a variable thickness between the last pellets layer and the top confining lid and a heterogeneous distribution of the bentonite powder at some layers inducing large inter pellets voids close to the bentonite-rock interface.Artificially injected water volume,relative humidity,water content and swelling pressure in both radial and axial directions were monitored.Comparison of the results showed that the presence of installation-induced heterogeneities led to the generation of preferential flow paths that influenced the swelling pressure evolution at radial and axial directions.
