Respiratory evaluation of patients requiring ventilator support due to acute respiratory failure

This review, based on relevant published evidence and the authors` clinical experience, presents how to evaluate a patient with acute respiratory failure requiring ventilatory support. This patient must be carefully evaluated by nurses, physiotherapists, respiratory care practitioners and physicians regarding the elucidation of the cause of the acute episode of respiratory failure by means of physical examination with the measurement of respiratory parameters and assessment of arterial blood gases analysis to make a correct respiratory diagnosis. After the initial evaluation, the patient must quickly receive adequate oxygen and ventilatory support that has to be carefully monitored until its discontinuation. When available, a noninvasive ventilation trial must be done in patients presenting desaturation during oxygen mask and or PaCO2 retention, especially in cases of cardiogenic pulmonary edema and severe exacerbation of chronic obstructive pulmonary disease. In cases of noninvasive ventilation trial-failure, endotracheal intubation and invasive protective mechanical ventilation must be promptly initiated. In severe ARDS patients, low tidal ventilation, higher PEEP levels, prone positioning and recruitment maneuvers with adequate PEEP titration should be used. Recently, new modes of ventilation should allow a better patient-ventilator interaction or synchrony permitting a sufficient unloading of respiratory muscles and increase patient comfort. Patients with chronic obstructive pulmonary disease may be considered for a trial for early extubation to noninvasive positive pressure ventilation in centers with extensive experience in noninvasive positive pressure ventilation.

The calculation of apparent resistivity in electrical methods is relatively simple and consolidated in the literature. Furthermore, in the case of DC methods, the apparent resistivity values are for the most part, intuitive and enable an initial interpretation of the results. On the other hand, in TEM method the apparent resistivity values are not very intuitive and interpretation based only on the apparent resistivity values cannot be done reasonably. In this way, this paper presents a discussion about the reasons and effects of the negatives values of apparent resistivity in TEM soundings. The main objective of this paper is to clarify the meanings of these negatives values and the possible effects in TEM inversion.

Study on the Attack of Molten Silicates on Plasma-Sprayed Thermal Barrier Coatings

Thermal barrier coating (TBC) revolutionized the industry by allowing higher operating temperatures for equipment, such as gas turbines in the aeronautical industry. However, at high temperatures, the TBC is exposed to the attack of molten silicates, known as CMAS (Calcium-Magnesium-Alumino-Silicate), which are particles from the environment that infiltrate the TBC, causing delamination. In this study, samples coated with TBC by thermal spray and covered with CMAS were evaluated at temperatures of 1200˚C and 1250˚C. For each temperature, exposure times of 1 h and 5 h were used. Samples with longer exposure time had a considerable volume increase. The main contribution of this work was to demonstrate the non-wettability of the CMAS, even in the 5-h heat treatments, which prevented its infiltration in the deeper regions. The conditions to guarantee the formation of the silicate and its consequent wettability are also discussed.

PNPLA3 and TM6SF2 polymorphisms in Brazilian patients with nonalcoholic fatty liver disease

BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is becoming the most common chronic liver disease worldwide,with significant morbidity associated with nonalcoholic steatohepatitis(NASH).Genome-wide association studies demonstrated that the variants rs738409 C/G in the PNPLA3 and rs58542926 C/T in the TM6SF2 genes are determinants of inter-individual and ethnicity-related differences in hepatic fat content and NAFLD *** To investigate PNPLA3 and TM6SF2 genotype frequency and their association with NAFLD development and progression in Brazilian *** This cross-sectional case-control study enrolled 285 individuals from the Gastroenterology and Hepatology clinics at a university hospital in *** case patients(n=148)were confirmed to have NAFLD by the identification of hepatic steatosis on ultrasonography and exclusion of other causes of liver *** to the clinical protocol,patients underwent liver biopsy when at high risk for NASH and/or advanced fibrosis(n=65).Steatohepatitis was confirmed in 54 *** who did not have biopsy indication or NASH on histology were considered to have simple steatosis(n=94).The control group(n=137)was selected among patients that attended the Intestinal Disease clinic and was composed of subjects without abnormalities on abdominal ultrasonography and normal liver biochemical *** individuals underwent PNPLA3 and TM6SF2 genotype *** PNPLA3 CC,CG and GG genotype frequencies were 37%,44%and 19%,respectively,in NAFLD patients and were 58%,31%and 10%in controls(P<0.001).In a model adjusted for gender,age,body mass index and type 2 diabetes mellitus,the G allele increased the chance of NAFLD(OR=1.69,95%CI:1.21-2.36,P=0.002)and NASH(OR=3.50,95%CI:1.84-6.64,P<0.001).The chance of NASH was even higher with GG homozygosis(OR=5.53,95%CI:2.04-14.92,P=0.001).No association was found between G allele and the features of metabolic *** histological assessment,PNPLA3 genoty

This study evaluated the effect of enriched artemia nauplii with commercial probiotic for angelfish larvae determining growth perfomance,survival and modulation of the intestinal ***,it experiment occurred in completely randomized design with five treatments(T1-0.0,T2-1.5,T3-3.0,T4-4.5 and T5-6.0g of commercial probiotic)and four replaces during 20 *** larvaculture,post larvae passed by biometric procedures to determine productive performance and then microbiological *** reduction of total heterotrophic bacteria while increased lactic acid bacteria in the intestinal tract from the post larvae for treatments T3,T4 and *** commercial probiotic also increased the survival and performance as final weight,weight gain and specific growth *** these reasons,the use of 3g of commercial probiotic promotes greater performance and intestinal modulation for angelfish larvae.

A Simple Two-Phase System Involving the Commercial Organophosphorous Extractant Cyanex 471x for the Preparation of Silver Sulphide Nanoparticles

The facile chemical synthesis of silver sulphide nanocrystals from metal-loaded organic media, containing a silver-selective organophosphorous ligand as extractant, is reported. The method involves the phase-transfer of silver species from aqueous nitrate media to organic solution using the commercial extractant Cyanex? 471x (tri-isobutylphosphine sulphide, Cytec Co.) as extractant, followed by precipitation stripping using ammonium sulphide as strip reagent. The nanoparticles were structurally characterized, and some aspects of the synthetical process, are briefly discussed. Under the conditions studied, the extractant Cyanex? 471x was able to act as stabilizer adsorbing on the particles surface, maintaining the size of the particles nanometrical.

Studies about the parasite fauna of marine fish highlights as an important problem for public health with zoonotic parasites or affecting the fish ***,this study evaluated the parasite fauna of Lutjanus synagris commercialized in the fish market from Braganç*** laboratory,58 fish were measured,weighted and conducted to parasitological analysis to determine parasitological indexes and relative *** parasite was fixed and identified until to the lowest taxonomic *** the parasitological analysis,it found Cymothoidae,Digenea,Cucullanus *** Procamallanus(Spirocamallanus)sp.,with total prevalence 67.24%.Digenea showed the highest prevalence and mean intensity *** nematode,Cucullanus *** the greater prevalence and relative dominance,while Procamallanus(Spirocamallanus)*** the greater mean intensity and *** showed the lowest prevalence and mean intensity *** conclusion,the parasite fauna of *** has been noted with low diversity,reporting the nematode occurrence Procamallanus(Spirocamallanus).

Fibrinolytic enzymes have received attention regarding their medicinal potential for thrombolytic diseases, a leading cause of morbidity and mortality worldwide. Various natural enzymes purified from animal, plant and microbial sources have been extensively studied. The aim of this work was to produce fibrinolytic protease by solid state fermentation using agro industrial substrates. Rhizopus arrhizus var. arrhizus UCP 1295 and Mucor subtillissimus UCP 1262 filamentous fungi species isolated from soil of Caatinga-PE, Brasil, were used as producer microorganisms. Wheat bran was shown to be the best substrate for the production of the enzyme and by using a 23 full factorial design the main effects and interactions of the quantity of the substrate wheat bran, moisture and temperature on the fibrinolytic enzyme production and protease were evaluated. The best results for fibrinolytic and protease activities, 144.58 U/mL and 48.33 U/mL, respectively, were obtained with Mucor subtillissimus UCP 1262 using as culture medium 3 g wheat bran, 50% moisture at a temperature of 25°C for 72 hours. The optimum temperature for the produced enzyme was 45°C and most of its original activity was retained after being subjected to 80°C for 120 min. The protease activity was enhanced by K+, Ca+ and Mn+;but with Cu+ there was an inhibition. The specificity to chromogenic substrate and the inhibition by PMSF indicates that it is a chymotrypsin-like serine protease. Presented results suggest that this enzyme produced by solid-state fermentation is an interesting alternative as a candidate for thrombolytic therapy.
