Impact of pediatric inflammatory bowel disease diagnosis on exercise and sports participation: Patient and parent perspectives

BACKGROUND Inflammatory bowel disease(IBD)may limit physical activity due to intestinal or extraintestinal manifestations,fatigue,or exercise *** sought to evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric *** compared patient-reported and parent-reported *** To evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric *** Consecutive IBD outpatients(aged 10-18 years)and their parents completed parallel voluntary surveys.A validated,patient-reported functional activity scale,the Hospital for Special Surgery Pediatric Functional Activity Brief Scale(HSS Pedi-FABS)was used to assess children’s activity *** There were 149 completed surveys(75%response rate)with mean participant age of 16.5 years[standard deviation(SD)=4.0]and mean age at IBD diagnosis of 11.8 years(SD=3.4).most patients(77%)were diagnosed within 12 mo of symptom *** athletic participation was reported in 65%across 65 *** was greatest before(40%)rather than after(32%)IBD diagnosis,with no reported change in 28%.IBD negatively impacted play/performance in 45%but did not change play/performance in 44%.IBD treatment improved patients’desire to exercise(70%)and subjective capacity for aerobic exercise(72%).Patients and parents agreed that IBD subjects demonstrate normal capacity for aerobic exercise(0.40,95%CI:0.22-0.58)and that treatment improved both participatory desire(0.33,95%CI:0.12-0.54)and capacity for aerobic exercise(0.52,95%CI:0.31-0.71).Almost all(99%)viewed exercise as healthy,and most encouraged its *** patients demonstrated normal mean HSS Pedi-FABS *** After pediatric IBD diagnosis,most patients continue exercise and sports,with mean activity levels comparable to American *** positively impacts participatory desire and aerobic *** favor the role of exercise in IBD.

Tumor necrosis factor-α inhibitor therapy and fetal risk:A systematic literature review

Tumor necrosis factor-α inhibitors (anti-TNFs) are effective in the treatment of inflammatory bowel disease (IBD) recalcitrant to conventional medical therapy. As the peak incidence of IBD overlaps with the prime reproductive years, it is crucial to establish pharmacologic regimens for women of childbearing age that achieve effective disease control without posing significant fetal harm. A systematic literature review was performed to identify all human studies with birth outcomes data after maternal exposure to infliximab, adalimumab, or certolizumab pegol within 3 mo of conception or during any trimester of pregnancy. Live births, spontaneous abortions or stillbirths, preterm or premature births, low birth weight or small for gestational age infants, and congenital abnormalities were recorded. Fifty selected references identified 472 pregnancy exposures. The subsequent review includes general information regarding anti-TNF therapy in pregnancy followed by a summary of our findings. The benefits of biologic modalities in optimizing disease control during pregnancy must be weighed against the potential toxicity of drug exposure on the developing fetus. Although promising overall, there is insufficient evidence to prove absolute safety for use of anti-TNFs during pregnancy given the limitations of available data and lack of controlled trials.

Primary Biliary Cirrhosis and Primary Sclerosing Cholangitis:a Review Featuring a Women's Health Perspective

Primary biliary cirrhosis (PBC) and primary sclerosing cho-langitis (PSC) are two major types of chronic cholestatic liver disease. each disorder has distinguishing features and variable progression, but both may ultimately result in cirrhosis and hepatic failure. The following offers a review of PBC and PSC, beginning with a general overview of disease etiology, pathogenesis, diagnosis, clinical features, natural course, and treatment. In addition to commonly associated manifestations of fatigue, pruritus, and fat-soluble vitamin deficiency, select disease-related topics pertaining to women's health are discussed including metabolic bone disease, hyperlipidemia and cardiovascular risk, and preg-nancy-related issues influencing maternal disease course and birth outcomes. This comprehensive review of PBC and PSC highlights some unique clinical considerations in the care of female patients with cholestatic liver disease.

Fault diagnosis of HVAC system with imbalanced data using multi-scale convolution composite neural network

Accurate fault diagnosis of heating,ventilation,and air conditioning(HVAC)systems is of significant importance for maintaining normal operation,reducing energy consumption,and minimizing maintenance ***,in practical applications,it is challenging to obtain sufficient fault data for HVAC systems,leading to imbalanced data,where the number of fault samples is much smaller than that of normal ***,most existing HVAC system fault diagnosis methods heavily rely on balanced training sets to achieve high fault diagnosis ***,to address this issue,a composite neural network fault diagnosis model is proposed,which combines SmOTeTomek,multi-scale one-dimensional convolutional neural networks(m1DCNN),and support vector machine(SVm).This method first utilizes SmOTeTomek to augment the minority class samples in the imbalanced dataset,achieving a balanced number of faulty and normal ***,it employs the m1DCNN model to extract feature information from the augmented ***,it replaces the original Softmax classifier with an SVm classifier for classification,thus enhancing the fault diagnosis *** the SmOTeTomek-m1DCNN-SVm method,we conducted fault diagnosis validation on both the ASHRAe RP-1043 dataset and experimental dataset with an imbalance ratio of 1:*** results demonstrate the superiority of this approach,providing a novel and promising solution for intelligent building management,with accuracy and F1 scores of 98.45%and 100%for the RP-1043 dataset and experimental dataset,respectively.

An effective bimetallic oxide catalyst of RuO_(2)-Co_(3)O_(4) for alkaline overall water splitting

enhanced quantum resources via two distant atom-cavity systems under the influence of atomic dissipation

Quantum resources such as entanglement and coherence are the holy grail for modern quantum technologies. Although the unwanted environmental effects tackle quantum information processing tasks, suprisingly these key quantum resources may be protected and even enhanced by the implementation of some special hybrid open quantum systems. Here, we aim to show how a dissipative atom-cavity-system can be accomplished to generate enhanced quantum *** do so, we consider a couple of dissipative cavities, where each one contains two effective two-level atoms interacting with a single-mode cavity field. In practical applications, a classical laser field may be applied to drive each atomic subsystem. After driving the system, a Bell-state measurement is performed on the output of the system to quantify the entanglement and coherence. The obtained results reveal that the remote entanglement and coherence between the atoms existing inside the two distant cavities are not only enhanced, but can be stabilized, even under the action of dissipation. In contrast, the local entanglement between two atoms inside each dissipative cavity attenuates due to the presence of unwanted environmental ***, the local coherence may show the same behavior as the remote ***, the system provides the steady state entanglement in various interaction regimes,particularly in the strong atom-cavity coupling and with relatively large detuning. more interestingly, our numerical analyses demonstrate that the system may show a memory effect due to the fact that the death and revival of the entanglement take place during the interaction. Our proposed model may find potential applications for the implementation of long distance quantum networks. In particular, it facilitates the distribution of quantum resources between the nodes of large-scale quantum networks for secure communication.
