Expression of Toll-Like Receptor-2 and -4 in Amniochorion Membranes of Preterm Delivery in the Presence of Histologic Chorioamnionitis

Expression of Toll-like receptors at the maternal-fetal interface during normal and complicated pregnancies has aroused interest in the last few years. However, despite the importance of TLR-2 and TLR-4, which recognizes most microorganisms presenting in the amniotic cavity infections associated with prematurity, comparison of the expressions of these receptors is rare in the literature. Thus, the purpose of this study was to compare the gene expression between TLR-2 and TLR-4 in amniochorion membranes of pregnant women with preterm delivery in the presence of histologic chorioamnionitis (HCA). Amniochorion membranes were collected from 40 pregnant women with preterm delivery;20 presented HCA and 20 did not. Fragments of the membranes were submitted to total RNA extraction, followed by cDNA production by reverse transcription. Real time quantitative PCR was used to quantify the gene expression of the TLRs. mRNA concentrations between TLR-2 and TLR-4 were compared using the nonparametric Mann-Whitney test. TLR-2 expression was higher than TLR-4 expression in the presence of HCA. No difference was observed between TLR-2 and TLR-4 expression in membranes in the absence of inflammatory infiltrate. In conclusion, amniochorion membranes express TLR-2 and TLR-4 and higher TLR-2 expression in the presence of histologic chorioamnionitis suggests that microorganisms recognizable by TLR-2 play an important role in the physiopathology of preterm labor.

Impact of gastroesophageal reflux control through tailored proton pump inhibition therapy or fundoplication in patients with Barrett's esophagus

AIM To determine the impact of upwards titration of proton pump inhibition(PPI) on acid reflux, symptom scores and histology, compared to clinically successful *** Two cohorts of long-segment Barrett's esophagus(BE) patients were studied. In group 1(n = 24), increasing doses of PPI were administered in 8-wk intervals until acid reflux normalization. At each assessment, ambulatory 24 h p H recording, endoscopy with biopsies and symptom scoring(by a gastroesophageal reflux disease health related quality of life questionnaire, GERD/HRLQ) were performed. Group 2(n = 30) consisted of patients with a previous fundoplication. RESULTS In group 1, acid reflux normalized in 23 of 24 patients, resulting in improved GERD/HRQL scores(P = 0.001), which were most pronounced after the starting dose of PPI(P < 0.001). PPI treatment reached the same level of GERD/HRQL scores as after a clinically successful fundoplication(P = 0.5). Normalization of acid reflux in both groups was associated with reduction in papillary length, basal cell layer thickness, intercellular space dilatation, and acute and chronic inflammation of squamous epithelium. CONCLUSION This study shows that acid reflux and symptom scores co-vary throughout PPI increments in long-segment BE patients, especially after the first dose of PPI, reaching the same level as after a successful fundoplication. Minor changes were found among GERD markers at the morphological level.

Vadose Zone Heterogeneity Effect on Unsaturated Water Flow Modeling at Meso-Scale

The understanding of unsaturated flow in heterogeneous formations is a prerequisite to the understanding of pollutant transfer in the vadose zone and the proper management of infiltration basins settled over such heterogeneous formations. This study addresses the effect of lithological heterogeneity of a glaciofluvial deposit on flow in the vadose zone underneath an infiltration basin settled in the Lyon suburbs. The basin had already been the subject of several previous studies, some of which demonstrated the impact of soil heterogeneity. But all of them were only based on the sedimentological study of a trench and no study addressed the potential spatial variability of results due to the spatial variability of soil heterogeneity. In this study, we model flow in the vadose zone for several case studies, including drainage, water infiltration during a rainfall event, and a complete meteorological chronic. These calculations were conducted for several sections, previously characterized in the basin using GPR and sedimentological study and compared with a blank (homogeneous section). The results clearly show that heterogeneity impacts unsaturated flow and that these impacts depend upon the section considered. Some geometrical architectural and textural parameters were proposed to explain the spatial variability and effect of the soil heterogeneity on unsaturated flow, thus establishing the first step towards modeling unsaturated flow in the basin at the meso-scale.

Seaweeds have been a food source since ancient times(600 BC)and are still widely used in Asia,mainly in traditional Chinese medicine and Japanese folk ***,seaweed compounds and extracts have been gaining interest from the biomedical and pharmaceutical market *** have been referenced as feasible solutions in finding new potential compounds and therapies in prevention,control,and reduction of cancer development due to the multirole of some bioactive components(e.g,phenolic compounds and sulphated polysaccharides).Moreover,seaweeds are rich in important health-promoting molecules[such as poly and highly unsaturated fatty acids(PUFAs and HUFAs),essential amino acids,vitamins,and dietary fibers]and minerals(calcium,iron,iodine,magnesium,phosphorus,potassium,zinc,copper,manganese,selenium,and fluoride).In this review,the potential therapeutic effects of seaweed in the prevention and treatment of cancer are approached,as well as nutraceutical properties of seaweed to promote cell homeostasis.

This documentary research aimed to describe the results of an epidemiological survey that sought to determine the prevalence of Infections Related to Health Care (IRHC) in newborns admitted to a Neonatal Intensive Care Unit (NICU) at the city of Rio de Janeiro. For methodology, the existing data in the original research database were used as the primary source. The method used was the case study in a quantitative approach. The data have been statistically arranged by odds ratio, which measured the possibility of IRHC development at the 102 newborns, by exposition at the risk factors. Results: IRHC prevalence with pneumonias, sepsis, navel’s infection and ocular infections predominance rises. Prematures are 4.10 more susceptible. Twelve hours after amniotic rupture there are 1.77 more chances of infections. Babies with more than 10 days of hospitalization are 4.70 more susceptible than those with 10 days. The registers examined were incipient at the IRHC’s characterization, beginning and duration, which make the IRHC’s diagnostic/notification difficult.

Fish populations are sensitive to environmental impact resulting from many factors, as an introduction of exotic species, industrial or residual waste, oil spills, pesticides or other agents that can directly affect the ecology and species survival. Astyanax altiparanae is pelagic and active swimmer specie and can have serious implications on its way of life as changes in muscle structure of fish may occur since the presence of pollutants in the water. In this study we analyze the alterations in Muscle and collagen fibers of the pectoral muscles and pollutants exposed flows contained in water with biodegradable detergents, and water coming from the Blue Lake, River clear-SP (2o24'39"S, 47o33'39"E) diluted. Effects of these exposures were analyzed during the period of seven and thirty days. Compared with acclimatized fish (Astyanax altiparanae) with pure water from an artesian well, the muscle fibers morphology and collagen concentration on them have changed due to the exposure to urban pollutants and biodegradable detergents.

The Action of p-Synephrine on Lipid Metabolism in the Perfused Rat Liver

p-synephrine and p-octopamine were found to increase lipolysis in adipocytes. The present study approaches the question if these compounds, natural products of the bitter orange (Citrus aurantium fruit), increase lipolysis and fatty acid oxidation in the liver. Experiments were done in the perfused rat liver. Non-recirculating hemoglobin-free perfusion was done using the Krebs/ Henseleit-bicarbonate buffer (pH 7.4) as perfusion fluid. Both p-synephrine and p-octopamine, at the concentrations of 100 μM, were found to stimulate the hepatic triacylglycerol lipase by 40% and 51%, respectively. These seem to be the maximal stimulations possible in the liver. In the perfused liver, p-synephrine, when present at an initial concentration of 500 μM, was able to increase the non-esterified fatty acid release after one hour of recirculating perfusion. The effects of p-synephrine on the oxidation of exogenously supplied [1-14C]octanoate and [1-14C]oleate were minimal. Only oxygen uptake, already stimulated by octanoate or oleate, was additionally increased by the infusion of p-synephrine. These results contrast with those obtained in a previous study with p-octopamine, which increased the production of 14CO2 from both [1-14C]octanoate and [1-14C]oleate. Apparently only the oxidation of endogenous fatty acids is stimulated by p-synephrine. On the other hand, both p-synephrine and p-octopamine stimulate the hepatic triacylglycerol lipase to a much lesser extent than the adipocyte lipase. It can be concluded that p-synephrine affects much more carbohydrate metabolism in the liver than lipid metabolism.

Do Cytokines Play a Role in Predicting Some Features and Outcome in Infective Endocarditis?

Objective: This study aimed to measure the serum concentrations of several cytokines in patients with infective endocarditis (IE), and correlate them with the C-reactive protein serum levels, echocardiographic findings and infecting microorganisms. Methods: Forty-five patients with definite IE according to the Duke’s criteria were included in the study. Ten healthy blood donors formed the control group. Serum levels of interleukin (IL) 1β, IL-6, IL-8, IL-10, IL-12 and tumor necrosis factor-α (TNF-α) were measured and compared between the groups, and also with some clinical and laboratory parameters of IE. Results: Patients with IE had significantly higher serum concentrations of the inflammatory mediators than the control patients. Median IL-12 and IL-1β levels were higher in staphylococcal than streptococcal IE. Except for TNF-α, the levels of all the other cytokines correlated with the C-reactive protein concentrations. In multivariate analysis, IL-10 and IL-12 serum levels remained as independent factors associated with the C-reactive protein concentrations. There was a significant correlation between IL-10 concentration and vegetation length. Conclusion: The inflammatory serum cytokines levels were elevated in IE in comparison to healthy controls. C-reactive protein concentrations correlated with cytokines levels. IL-1β and IL-12 serum concentrations were higher in staphylococcal when compared to streptococcal IE. IL-10 levels were associated with vegetation size.

We aimed to compare Gleason score and tumor laterality between transrectal ultrasound-guided biopsy of the prostate （TRUSBX） and radical prostatectomy （RP）. Some factors that could cause a discrepancy in results between these two procedures were also evaluated. Among the 318 cases reviewed, 191 cases were selected for inclusion in this comparative Study, We divided the patients into two groups using the Gleason score： an intermediate/high-grade group （≥7） and a low-grade group （〈6）. Exploratory analyses were conducted for comparisons between groups. We also performed comparisons between TRUSBX and RP for tumor laterality. TRUSBX overestimated 6% and underestimated 24% cases in comparison with RP for Gleason score, and overestimated 2.6% and underestimated 46% cases compared with RP for tumor laterality. Biopsy specimens were slightly smaller in TRUSBX cases with underestimated tumor laterality （P〈 0.05）, and no relationship between the biopsy specimen size and underestimated Gleason score in TRUSBX was found. Prostatic volume showed no statistical correlation with the likelihood of under or overestimation （P 〉 0.05）. Thus, our study showed that TRUSBX has a high likelihood of underestimating both the Gleason score and tumor laterality in prostate cancer （PCa）. The size of the fragment appears to be an important factor influencing the likelihood of laterality underestimation and Gleason score overestimation via TRUSBX. Due to the high likelihood of underestimation of the Gleason score and tumor laterality by 12-core prostate biopsy, we conclude that this type of biopsy should not be used alone to guide therapy in Pca.

Diversity of Filamentous Fungi of Area from Brazilian Caatinga and High-Level Tannase Production Using Mango (<i>Mangifera indica</i>L.) and Surinam Cherry (<i>Eugenia uniflora</i>L.) Leaves under SSF

Tannase is a biotechnologically important enzyme that can be produced during fungal fermentation of organic matter. The Caatinga is an exclusive Brazilian ecosystem that has been largely unexplored by science, particularly its filamentous fungal diversity. This study evaluated the diversity of filamentous fungi in the Caatinga soils of Pernambuco, Brazil, and their potential for tannase production by solid-state fermentation (SSF) of mango (Mangifera indica L.) and Surinam cherry (Eugenia uniflora L.) leaves. A total of 4711 isolates were obtained, 2090 during the rainy seasonand 2621 during the dry season. The isolates belonged to 18 genera and 66 species, with Aspergillus and Penicillium having the highest species richness. The dry season had a higher diversity index. Aspergillus was the dominant genus, and A. flavus, A. sclerotiorum, and A. ochraceus the most abundant species. A representative of each species was tested for tannase production using dried mango and Surinam cherry leaves as substrates;the leaves contained 14.28 and 7.0 g/L tannin, respectively. Most fungal species produced tannase, but the highest yields were obtained when mango leaves were used as substrate for Penicillium restrictum (accession URM 6044), Aspergillus flavofurcatus (URM 6142), and A. stromatoides (URM 6609), which produced 104.16, 87.51, and 81.83 U/mL tannase, respectively. These yields exceeded previously published reports. Filamentous fungi from Caatinga soils have great potential for producing tannase by SSF, and low-cost mango leaves make excellent substrate.
