Inter-hospital transfers and door-to-balloon times for STEMI: a single centre cohort study

Background Key performance indices such as door-to-balloon times have long been recognized as quality metrics in reducing time to care for patients with acute coronary syndromes(ACS). In the situation where patients do not present to a facility capable of 24/7 percutaneous coronary interventions(PCI) delays in time to therapy can exceed the recommendation of 90 min or less. This study aimed to evaluate the impact of transfers on performance indices for patients diagnosed with ST-segment elevation myocardial infarction(STEMI). Methods Over a seven month collection period, all patients presenting with symptoms suggestive of ACS and admitted for PCI were studied. Patients were divided into dichotomous groups of direct presentations or transfers from a secondary non-PCI capable hospital with key times recorded, including symptom-onset, first hospital and PCI-capable hospital arrival and balloon inflation times to evaluate time of treatment for STEMI patients. Results Of the 87 patients diagnosed with STEMI, transferred patients experienced statistically significant delays in symptom-onset to the PCI-capable hospital(PCI-h) arrival(215 vs. 95 min, P hospital arrival to balloon inflation times(106 vs. 56 min, P hin 90 min from first hospital arrival, while 60%(n = 19) did within 120 min, although all received balloon inflation within 90 min from arrival at the PCI-h. After controlling for confounding factors of socio-economic status, presentation date/time and diagnostic category, transferred patients experienced an average 162% longer delays from symptom-onset to PCI-h door arrival, and 98% longer delays in symptom-onset to balloon inflation;compared to patients who present directly to the PCI-h. No statistically significant differences were noted between transferred and direct patients when measured from PCI-

二叠纪镁铁-超镁铁质岩体在北山造山带广泛分布,其中多发育铜镍硫化物矿化,并集中在新疆北山坡北和甘肃北山柳园地区。本文对柳园地区二叠纪骆驼山(283Ma)和西南山(277Ma)矿化岩体中早期结晶的造岩矿物开展矿物化学分析,并对甘-新北山地区矿化岩体的主要造岩矿物进行系统对比研究。骆驼山和西南山岩体主要由单辉橄榄岩、二辉橄榄岩、橄榄辉石岩、橄榄辉长岩和辉长岩组成,主要造岩矿物为橄榄石、单斜辉石、斜方辉石、斜长石和角闪石,及少量铬尖晶石和金云母。铬尖晶石Cr~#值范围为52.2~70.4,TiO_(2)含量为1.1%~4.0%,Al_(2)O_(3)含量为8.1%~18.9%。橄榄石Fo值范围为78~86,Ni含量为856×10^(-6)~2121×10^(-6),Ca含量为71.3×10^(-6)~720×10^(-6)。斜方辉石主要为顽火辉石和少量古铜辉石;单斜辉石为透辉石和普通辉石,Mg~#范围为86~88,Al~Ⅳ/Al~Ⅵ比值范围为1.42~3.49。矿物成分计算结果表明岩体的母岩浆为高Mg拉斑玄武质岩浆,结晶温度范围为1132~1323℃,结晶压力为0.23~0.25GPa,氧逸度范围为FMQ+1.0~+1.7,具有富集LREE以及Nb-Ta-Zr-hf负异常的特征。富含含水矿物及母岩浆含水量反演结果显示骆驼山和西南山岩体单斜辉石结晶时其岩浆含水量分别为3.4%和3.6%,表明岩体原始岩浆形成于富水源区,可能与俯冲交代作用有关。与坡北地区岩体对比,骆驼山和西南山岩体母岩浆结晶温度略低,较高的单斜辉石Al~Ⅳ/Al~Ⅵ值以及压力计算表明岩体形成于相对低压环境,岩浆氧逸度范围略高于坡北地区岩体,可能与含水的镁铁质岩浆分异演化作用相关。结合区域地质资料认为柳园地区二叠纪岩体形成于碰撞后伸展构造环境,母岩浆所具有的岛弧岩浆特征来源于地幔源区曾经历的俯冲板片来源熔/流体的改造作用。

Geochemistry of the Rare Earth Elements in Natural Terrestrial Waters:A Review of What Is Currently Known

The range of observed Chemical compositions of natural terrestrial waters varies greatly especially when compared to the essentially constant global composition of the *** concentrations of the REEs in natural terrestrial waters also exhibit more variation than what Was reported in seawater. In terrestrial waters, ph values span the range from acid up to alkaline. In addition, terrestrial waters can range from very dilute waters through to highly concentrated brines. The REE concentrations and their behavior. in natural terrestrial waters reflect these compositional ranges. Chemical weathering of rocks represents the source of the REEs to natural terrestrial waters and, consequently, the REE signature of rocks can impart their REE signature to associated waters. In addition, because of the typical low solubilities of the REEs, both surface and solution complexation can be important in fractionating REEs in aqueous solution. Both of these processes are important in all natural terrestrial waters, howev-er, their for relative importance varies as a function of the overall solution composition. In alkaline waters, for example, solution complexation of the REEs with carbonate ions appears to control their aqueous distributions whereas in acid waters, the REE signature of the labile fraction of the REEs is readily leached from the rocks. In circumneutral ph waters, both processes appear to be important and their relative significance has not yet been determined.

New Methods to Quantify Canopy Structure of Leafless Boreal Birch Forest from hemispherical Photographs

hemispherical photography has been used for many years to measure the physical characteristics of forests, but most related image processing work has focused on leafy canopies or conifers. The boreal forest contains large areas of deciduous trees that remain leafless for over half the year, influencing surface albedo and snow dynamics. hemispherical photographs of these sparse, twiggy canopies are difficult to acquire and analyze due to bright bark and reflections from snow. This Note presents new methods for producing binary images from hemispherical photographs of a leafless boreal birch forest. Firstly, a thresholding method based on differences between colour panes provides a quick way to remove bright sunlit patches on vegetation. Secondly, an algorithm for joining up fragmented pieces of tree after thresholding ensures a continuous canopy. These methods reduce the estimated hemispherical sky view fraction by up to 6% and 3%, respectively. Although the processing remains subjective to some degree, these tools help to standardize analysis and allow the use of some photographs that might have previously been considered unsuitable for scientific purposes.
