Objective： Differentiated thyroid carcinomas （DTCs） are classified into papillary thyroid carcinoma （PTC） and follicular thyroid carcinoma （FTC）. DTCs are analyzed as a single group in clinical studies that investigated the prognostic factors and prognosis of these malignancies. However, the biological behaviors of these carcinomas significantly differ. In the present study, we aimed to detect differences in the outcomes between PTC and FTC in Mansoura University Hospital in Egypt. Methods： A total of SS8 patients with histologically proven thyroid carcinomas from January 2003 to December 2012 were retrospectively enrolled. The clinical and pathological data of patients were reviewed. Results： Large primary tumor size, lymph node involvement, extrathyroid extension, and distant metastasis were significant poor prognostic factors for overall survival （OS） in old PTC patients. Cox hazard analysis showed that the patient＇s age, extra thyroid extension, and distant metastasis were the only independent prognostic factors. In FTC patients, only the distant metastasis and degree of tumor invasion were significant poor prognostic factors in OS univariate analysis. However, these factors were nonsignificant in multivariate analysis. The 10-year OS rates were 97% and 89% for PTC and FTC, respectively （P=0.003）. The 10-year disease-free survival （DFS） rates were 77.2% in PTC vs. 65% in FTC （P=0.179）. Conclusion： The significant prognostic factors vary between the two types of DTCs. Therefore, PTC and FTC patients need to be analyzed and reported independently. PTC survival is widely and significantly affected by age, extrathyroid extension, and distant metastasis. By contrast, these factors were nonsignificant in FTC, which showed poorer survival than PTC.

The use of unmanned aerial vehicles(UAVs) in the collection of data from wireless devices, sensor nodes, and the Internet of Things(IoT) devices has recently received significant attention. In this paper, we investigate the data collection process from a set of smart meters in advanced metering infrastructure(AMI) enabled by UAVs. The objective is to minimize the total annual cost of the electric utility by jointly optimizing the number of UAVs, their power source sizing, the charging locations as well as the data collection trip planning. This is achieved while considering the energy budgets of batteries of UAVs and the required amount of collected data. The problem is formulated as a mixed-integer nonlinear programming(MINLP), which is decoupled into two sub-problems where a candidate UAV and a number of buildings are first grouped into trips via genetic algorithms(GAs), and then the optimum trip path is found using a traveling salesman problem(TSP) branch and bound algorithm. Simulation results show that the battery capacity or the number of UAVs increases as the coverage area or the density increases.

This study aimed to assess the preventive effects of thyme oil and thymol on doxorubicin(DOX)-induced renotoxicity,cardiotoxicity,and oxidative stress in Wistar *** oil was subjected to GC-MS analysis,which indicated that thymol was the major constituent representing 33.896%.Rats intraperitoneally injected with DOX at a dose of 2 mg/kg b.w./one per week for 7 weeks were co-treated with thyme oil and its major constituent,thymol,at doses 250 and 100 mg/kg b.w./every other day,respectively,by oral gavage for the same *** oil and thymol markedly ameliorated the raised levels of serum urea,uric acid,and creatinine in DOX-administered *** also reduced the elevated activities of serum CK-MB and *** oil was more effective than thymol in decreasing the elevated serum creatinine level and serum CK-MB activity in DOX-administered rats,thereby reflecting its more potent effect on kidney and heart *** peroxidation significantly decreased while GSH level and GST and GPx activities significantly increased in kidney and heart of DOX-administered rats treated with thyme oil and *** DOX-induced perturbed kidney histological changes including congestion of glomerulus tuft,inflammatory cells infiltration,protein cast in lumina of the renal tubule,and thickening of the parietal layer of Bowman’s capsule were remarkably ameliorated as a result of treatment with thyme oil and thymol;thyme oil was more *** addition,DOX-induced deleterious heart histological alterations,including intramuscular infiltration of inflammatory cells,focal necrosis of cardiac myocytes,and edema,were remarkably reduced by treatment with thyme oil and ***,it can be concluded that DOX could induce marked toxicity in kidney and heart,and the treatment with thyme oil or thymol produced potential improvement of kidney and heart function and histological integrity via repression of oxidative stress and enhancement of antioxidant defense mechanisms.

Transient elastography compared to liver biopsy and morphometry for predicting fibrosis in pediatric chronic liver disease: Does etiology matter?

AIM: To evaluate transient elastography (TE) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver ***: A total of 90 children [50 with chronic hepatitis C virus (HCV), 20 with autoimmune hepatitis (AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement (LSM) using TE. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction (FAF) using digital image analysis (morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was also performed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiver-operating characteristic curve. Cut-off values with optimal clinical performance (optimal sensitivity and specificity simultaneously) were ***: The majority of HCV group had minimal activity (80%) and no/mild fibrosis (72%). On the other hand, the majority of AIH group had mild to moderate activity (70%) and moderate to severe fibrosis (95%) and all Wilson disease group had mild to moderate activity (100%) and moderate to severe fibrosis (100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter (r = 0.839 vs 0.879, P < 0.0001 for both). LSM discriminated individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH (16.15 ± 7.23 kPa) compared to Wilson disease (8.30 ± 0.84 kPa) and HCV groups (7.43 ± 1.73 kPa). OBJECTIVE:To investigate the efficacy of Fig fruit powder and olive on hepatic,renal and splenic injury induced by 2-nitropropane(2-NP)in mice,especially if they were used in ***:A total of 40 adult BALB/c male mice weighting 25-30 g/*** were categorized into five groups(8 each).Group 1 as negative *** 2 as positive control group intraperitoneally injected with 2-NP(100 mg/kg b.w.)3 times/weekly for eight *** 3 injected with 2-NP and were orally supplemented with Fig(300 mg/kg).Group 4 injected with 2-NP and were orally supplemented with olive(100 mg/kg).Group 5 injected with 2-NP and were orally supplemented with mixture of Fig and olive(3∶1 respectively).RESULTS:Histopathological observation of liver in mice treated with 2-NP showed cellular degeneration,pyknosis,and congestion of the portal *** kidney there were disorganization of the cortical tissues,cellular necrosis and plenty of inflammatory lymphocytic *** elevations in liver function parameters(alanine aminotransferase and aspartate aminotransferase),m RNA expression levels of tumor necrosis factor-α,nicotinamide adenine dinucleotide phosphate oxidase and cyclooxygenase were detected as anti-inflammatory markers and 5-lipoxygenase,interleukin-1βand interleukin-6 as inflammatory biomarkers for liver and spleen,also significant elevations was detected in lipid peroxidation *** levels of antioxidants,glutathione,glutathione peroxidase,catalase and superoxide dismutase were significantly ***:our findings indicated that Fig fruit powder and olive protected against hepatic,renal and splenic injury induced with 2-NP in mice,especially if they were used in combination.

3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GSH) content made other melanoma cells resistant to 3BP. Concurrent use of a GSH depletor with 3BP killed resistant melanoma cells. Survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. Diagnostic and Prognostic Value of Serum Tissue Polypeptide Antigen in Advanced Lung Cancer

Lung cancer is a lethal malignancy, however, no serum marker is routinely recommended till now. Prospectively two groups of patients included: Group I: Patients with advanced lung cancer. Group II: patients with benign lung disease as control. Serum Tissue Polypeptide Antigen (TPA) levels were measured by ELISA technique before the first line chemotherapy. The TPA cutoff taken was 1800 pg./ml. End points were comparison of high TPA in cases and controls and correlation between high TPA and disease progression (PD), progression free survival (PFS) and overall survival (OS). 30 patients with advanced lung cancer (16 non-small and 14 small cell lung cancer) and 15 patients with benign lung disease were included and followed up during the period from October 2008 to October 2011 with median follow-up of 1.5 years. High TPA was found in 50% of lung cancer cases compared to 26% in controls (p = 0.014). High TPA was found in 64% of cases showing PD versus 36% normal TPA (p = 0.08). 1 year PFS in high TPA was 32% versus 39% in normal TPA, (p = 0.2). 1 year OS in high TPA was 46% versus 73% in normal TPA (p = 0.6). Serum TPA is a potential marker for advanced lung cancer.

Thermally modified organic materials commonly known as biochar have gained popularity of being used as a soil *** information, however, is available on the role of biochar in alleviating the negative impacts of saline water on soil productivity and plant growth. This study, therefore, was conducted to investigate the effects of Conocarpus biochar(BC) and organic farm residues(FR) at different application rates of 0.0%(control), 4.0% and 8.0%(weight/weight) on yield and quality of tomatoes grown on a sandy soil under drip irrigation with saline or non-saline water. The availability of P, K, Fe, Mn, Zn and Cu to plants was also investigated. The results demonstrated clearly that addition of BC or FR increased the vegetative growth, yield and quality parameters in all irrigation treatments. It was found that salt stress adversely affected soil productivity, as indicated by the lower vegetative growth and yield components of tomato plants. However, this suppressing effect on the vegetative growth and yield tended to decline with application of FR or BC, especially at the high application rate and in the presence of biochar. Under saline irrigation system, for instance, the total tomato yield increased over the control by 14.0%–43.3% with BC and by 3.9%–35.6% with FR. These could be attributed to enhancement effects of FR or BC on soil properties, as indicated by increases in soil organic matter content and nutrient availability. Therefore, biochar may be effectively used as a soil amendment for enhancing the productivity of salt-affected sandy soils under arid conditions.

Aim:The aim was to assess the impact of hyperglycemia on the recurrence of hepatocellular carcinoma(HCC)as well as evaluate survival after curative ablation by ***:This study,which was conducted retrospectively on 107 chronic hepatitis C(CHC)patients with 159 HCCs,was presented to the Hepatology Unit of Internal Medicine Department at Tanta University *** lesions were curatively treated by radiofrequency ablation(RFA)and the surveillance of HCC recurrence was evaluated radiologically every 3 months for periods between 6 and 36 *** 107 subjects,70 were males and 37 were females,with mean age 50.4±9.4 *** patients were divided according to their glycemic state into the following three groups:Group I,which included 37 type 2 diabetic patients,with adequate maintenance of blood glucose,has 52 HCCs;Group II,which included 25 type 2 diabetic patients with inadequate maintenance of blood glucose,has 43 HCCs;and Group III,which included 45 euglycemic non-diabetic patients,has 64 ***:Our results showed that,there was signifi cant increase in recurrence rate in diabetic patients with inadequate maintenance of blood glucose(Group II)compared to those in Group I and Group III(P<0.0001).Interestingly,there was no signifi cant difference concerning HCC recurrence between diabetic patients with adequate maintenance of blood glucose(Group I)and non-diabetic euglycemic patients(Group III).Our results also identifi ed that,inadequate maintenance of blood glucose in diabetic patients was also a signifi cant predictor of poor ***:Inadequate maintenance of blood glucose in diabetic patients is a signifi cant risk factor for recurrence of HCC and for poor survival after curative RFA therapy in CHC patients.
